A few years ago, one of this blog’s frequent commentators mentioned taking Finasteride for six days a week and the much stronger Dutasteride for one day a week. This would theoretically limit the side effects from taking the much more potent Dutasteride for all seven days of the week.

I blew off this idea as pointless, since Dutasteride by itself has such a long 5-week half-life (Finasteride has a 6-hr or so half-life). So one might as well just take Dutasteride once or twice a week and completely ignore taking Finasteride to keep things simple. In fact I myself have been taking 0.5mg Dutasteride every two days rather than daily for a couple of years now.

However, a short time after the above commentator made the suggestion, another blog reader made the following comment:

I had been on Propecia alone for 15 years. Noticed hair started to thin again in 2016. Added Avodart in Jan 2017 and I can honestly say my hair is better and thicker than all the years on just Propecia. I take Propecia every day and one 0.5 Avodart once a week. Never had any sides at all. I did have a shed when I added the Avodart, but stick with it and within about 4 months I could feel and see new hairs. My hairline is better than I can remember. It has taken one full year of this dosage to fully appreciate just how effective it has been. I will continue until things change and then reassess, but hopefully that won’t be for a very long time.

I have read some similar suggestions which is why I initially went with 1x per week Dut. Also, just wanted to test how I'd react to the drug. I do agree with @LookMaxx that you don't want to get carried away with over blocking DHT. But the paranoia of losing more hair especially when you read about how Fin loses it's efficacy over the course of time got me to at least try Dut as a safety measure. @LookMaxx got me thinking that maybe I can get away with Dut just a couple of times a month instead. Where the growth comes into play is that all the downstream effects that play into blocking DHT help promote the WNT pathway as well. DHT will suppress the WNT pathway so it's kind of like a double whammy. Which is why Fin and Minoxidil work so well. Fin unblocks the WNT pathway by blocking DHT and the 5-AR inflammatory markers that are associated with it. Minoxidil will then stimulate the growth of the hair with nothing now getting in the way of the WNT pathway. This is why it is paramount to start with these 2 modalities for your hairloss treatments before anything else. 

Even if it's true that efficacy goes down with fin, so does ones DHT when you hit 50 years old. I suppose I'd probably be fine just taking Dut a couple of times month instead of once a week. Something to think about.

It's tough because everyone's physiology is different and there is no hard and fast rule on dosage. All one can do is try and see what works. Problem is if you aim to low on dosage, you may experience the death by 1000 cuts where the change is so gradual with the thinning that you never notice the loss until it's too late. Which is why I've always aimed higher on dosage and if tolerated, continue on. I do agree however, that you do eventually hit a bell curve where there is a point of diminishing returns. Perhaps for me that may be taking Dutasteride too often. I'll research it more, but I may move to once every couple of weeks.

Too bad we can't meet up @ZeeMan. I went very cheap on it. Got a LLLT bulb for $40, then hung a utility light cord above my head with the bulb oriented about 6cm above my head. Works like a charm, but definitely not as good as the laser caps.

Wow @MazAB! That new design is very cool I must say, though I'll be sad to see your current design go as it's so brilliant already. Those new temple points are going to be epic!

Will definitely do that Ryan! Thanks for the stamp of approval! It means a lot coming from a guy like you as I know we are both very particular when it comes to drawing the lines to have perfect framing of the face.

I think design is very good. It looks similar to this : 

Personally i didn't chose V-shape for myself, because i think it doesnt suit my face, i sticked to hairline which i had before balding. 

V-shape gives this more agressive look and due to fact that you are going to the gym too, you will look badass.

New design will make your forehead something about 1/3 of face - red line 1/3, blue line 2/3, so it's textbook proportion for men's face. Touching up the temples will provide narrower forehead and less roundness, which is good. Remember to double check if temple points that i tagged with green circles to be at same level. Eugenix sometimes like to make this green points at different level to add "asymmetrical naturalness" and i dont agree with this approach. Like i agree that face is not symmetrical and they are doing great job with little asymmetricality here and there, but this green points should be at the same level, because if not, it looks weird. Yeah after 1st day of surgery i noticed that they are at different level(one green point was higher then second) and incisions were already made. But guess what, at day 2 i asked to make more incisions on the temple, so the level of green points will be the same

Green point at this level will provide nice tunnel (red arrow). Your temples project is very similar to mine, i approve :)

Completely agree! You should have irregularities that are in the symmetry, not outside of it. It is so important to keep that balance. I'm actually having the doc balance mine out because there definitely was a slight imbalance. It was very subtle and nobody ever noticed, but this is exactly what these sessions are for. Refining, reshaping and rebalancing is the name of the game especially when you have an already brilliant procedure that preceded it. 

@GeneralNorwood thank you for being totally awesome and breaking down the design for me! You confirmed what I was thinking about how it frames my face and why I chose the design to begin with. I know it is an aggressive approach, but the one thing I learned  the from first experience is that the hairline and temples drawn always look more aggressive than how they turn out with Eugenix. Typically this is because of the soft singles they use in the first couple of rows of the hairline, and even more so with the super fine hair they use on the temple points. I know I got a long way to go especially having not sat on the operating table yet. It'll take a while before this new look takes over, but until then I'll keep day dreaming about it, haha!😎

With the reset button officially hit, I’m 2 days post OP! As a reminder my 1st procedure was 2700 grafts and after a very very long day on the operating table yesterday, I had 2500 grafts extracted and a total of 2700 slits.

I’m very happy with how everything turned out! The donor looked unbelievable coming right out of the procedure which I never would have expected. The donor management and techniques in the extraction process were so meticulous and well thought out, could not have asked for more on that front. Since the nature of the procedure was refinement, many soft singles were extracted behind the ears, nap of neck, as well as lower bottom just above the nap. This easily made up over 1300 grafts for both the temple points and hairline. The remaining extractions were in the dominant portions of the donor for the multi hair grafts which made up a combo of 2’s, 3’s and 4’s for a total of 1200 to be added behind the soft singles in the new hairline and distributed strategically throughout the frontal 3rd for added density.

I was told if I did ever need to go back in the future, I’d have plenty of scalp donor to work with. I truly believe that my donor was exceptionally well managed and is set up for great success down the line if I ever  need to dip back into the donor pool.

Here is the breakdown:

Temple points - 700 grafts - 350 on each side.

Hairline - 1000 single and double hair grafts

Back of hairline and frontal 3rd - 800 grafts - both 3 and 4 hair

Eugenix has been right on the money with how they conduct their business. I can assure you that the quality of work has not gone down in any way since I was there last year. A lot of new faces in a lot more areas of the business, but it was not disorganized or chaotic in anyway.

Dr Pradeep really crushed it with my design. Can’t say enough about how detailed he was in the planning, and how creative he was performing the slits to lock this down as the perfect framing  both horizontally and vertically for me.

The procedure was a total of 13 hours. Most of the time was taken having a gradient build up on the temple points and hairline. Very detail oriented process that I completely underestimated. Very hard to believe that after my initial procedure on the temple points, I had roughly 200 grafts done on each side. This time around it is absolutely astonishing that the doctor was able to get 350 additional grafts implanted on each temple point leading up to the hairline.

Crazy to think that in 2 procedures this adds up to 1100 graft total between both temporal regions. It is everything I ever wanted and more. Doctor Pradeep has definitely put me in a great position, so now it is time to just sit back and have patience with the process. 

I’ll remain in India until the 21st and look forward to making this the well deserved time off and vacation that I've been wanting since my first trip here to India.

Just looking at the recipient area compared to the donor, i'm genuinely shocked at how it doesn't look touched at all for that many grafts. 

The hairline design and temple points in particular are very transformative. Honestly, i would go so far as to say, if i put a photo of you pre-op from the first procedure, directly post-op and now post-op on your 2nd procedure, you look like different people. Genuinely that much is how the new hairline design has changed your look imo. 

I can't wait to see it grow in and i think you 100% have got the best treatment and attention one could have hoped for. It should be an amazing result. Your temple points are now more significantly bolstered than what i had. 330 grafts on my right temple and 400 on my left so it will be amazing to see what your near 1100 grafts just on the temple points will look like.

OMG, This is fu**ing Guiness World Record Temple Hair Transplant. I had 720 grafts transplanted for the temples and i thought that i did a lot, but 1100 total? You became LEGENDARY member of this forum :D

Hairline looks epic, during your HT journey you decided to chose more agressive approach, but this is good, i approve. However as Uncle Ben once said, with great power(hairline) comes great responsibility. So leaving dutasteride behind is not an option in my opinion. Dutasteride once a week is minimum in this situation. You have to reduce DHT in scalp to maintain upcoming superb effect. This is not paranoia, Dutasteride is clinically proven to be better then finasteride for hairloss. One thing that you can consider is to adding topical dutasteride to your regime instead of only pills approach.

Yeah, that's surprising to see and looks very different from 1st procedure. Maybe Armen can elaborate on that. I guess Dr. Sethi became so skilled(level 99) that he can create grafts with his mind power.

I’m very happy with how everything turned out! The donor looked unbelievable coming right out of the procedure which I never would have expected. The donor management and techniques in the extraction process were so meticulous and well thought out, could not have asked for more on that front. Since the nature of the procedure was refinement, many soft singles were extracted behind the ears, nap of neck, as well as lower bottom just above the nap. This easily made up over 1300 grafts for both the temple points and hairline. The remaining extractions were in the dominant portions of the donor for the multi hair grafts which made up a combo of 2’s, 3’s and 4’s for a total of 1200 to be added behind the soft singles in the new hairline and distributed strategically throughout the frontal 3rd for added density.

I was told if I did ever need to go back in the future, I’d have plenty of scalp donor to work with. I truly believe that my donor was exceptionally well managed and is set up for great success down the line if I ever  need to dip back into the donor pool.

Here is the breakdown:

Temple points - 700 grafts - 350 on each side.

Hairline - 1000 single and double hair grafts

Back of hairline and frontal 3rd - 800 grafts - both 3 and 4 hair

Eugenix has been right on the money with how they conduct their business. I can assure you that the quality of work has not gone down in any way since I was there last year. A lot of new faces in a lot more areas of the business, but it was not disorganized or chaotic in anyway.

Dr Pradeep really crushed it with my design. Can’t say enough about how detailed he was in the planning, and how creative he was performing the slits to lock this down as the perfect framing  both horizontally and vertically for me.

The procedure was a total of 13 hours. Most of the time was taken having a gradient build up on the temple points and hairline. Very detail oriented process that I completely underestimated. Very hard to believe that after my initial procedure on the temple points, I had roughly 200 grafts done on each side. This time around it is absolutely astonishing that the doctor was able to get 350 additional grafts implanted on each temple point leading up to the hairline.

Crazy to think that in 2 procedures this adds up to 1100 graft total between both temporal regions. It is everything I ever wanted and more. Doctor Pradeep has definitely put me in a great position, so now it is time to just sit back and have patience with the process. 

I’ll remain in India until the 21st and look forward to making this the well deserved time off and vacation that I've been wanting since my first trip here to India.
