64 years-old patient. No med's. Multiple procedures in the last 30 years (as he say: the HT history), some of them outside of the safe area (fut, punch graft, scalp reduction...).

He think he has a last bullet.

what to do?

He may think he has a bullet left, but the powder is wet and the hammer won't cock. Certainly there are some good haberdasheries close by...

 

Seriously, the donor area is pretty depleted and what there is wouldn't cover much. Shave the dome, grow the sides and back a bit to cover the scars.

I'm serious.  Just look at my face.

I don't know but I'm looking forward to the result. He doesn't have much scalp hair left to work with at all. Looks like his punch grafts have thinned as well (from his donor). I would consider bht as an option in conjunction with any scalp fue he has left. Rare to see someone with such an extensive history.

Extract the strip and plug grafts and use them to fill up the strip and plug scars. Use some beard hair if needed. At least his donor area will appear more natural and he will have a natural norwood pattern this way. Without very extensive body hair transplant(10000+) i cant see this guy getting away with a decent head of hair sadly. So saddening...

What a shame. The only "bullet" he has left resides in his chin and chest hairs. At 64 it's a tough call but if he has the funds and the will, somebody like Umar will be his only course of action. He definitely should let his hair grow in the back to provide some coverage for the punches and strip scars.

shave it and FUE in the scars with remaining donor and also seek some SMP treatment for anything not covered by the FUE grafts

Ok the guy has pulled the trigger, But why wasnt more taken from the beard? as opposed to what looks like a already over harvested donor area. And with all the scars etc in his donor area?! surely it would have been better to leave that alone in the hope it might disguise some of the previous work??

Looks like the guy has got a great beard and im guessing he must have some chest hair also??

Could the guy get smp into his donor??? Would it take on scar tissue??

This poor blokes life has been ruined by hair loss!!

I'm rooting for him!!!

 

That's an excellent suggestion. SMP would be the route I would take with the scars.

 

As far the work so far, he will have a much improved hairline with this added, and the option for more beard hair behind the hairline can be done in Round Two. My first repair was 1500 grafts into the hairline, I had only a bit more scalp available than this guy. The downside was harvesting scalp hair around extensive scarring only makes the scarring more apparent. The addition of around 700 beard into my strip scars made a huge difference, and when I added temporary SMP on top of it, it allows me to cut my hair in back to a #1 guard.

 

Repair on patients like this usually starts at the hairline, then the real work begins. I was in a somewhat similar situation as this chap (without punch graft scars and the scalp reduction) and my crown did not have the scars as his does. When you have this level of damage, you have to know what areas to address, and which ones you have to leave alone. I'm fortunate enough to leave the crown alone, and this guy should probably do the same. I find crowns to be black holes when you need work from the hairline all the way back (and if you have the finances, some folks can go all the way south)

 

Round Two: Assess whether or not beard is needed from hairline to mid-scalp. I think it will be necessary, so add beard grafts to this area. Ultimately I believe he will need, at minimum, a few thousand grafts, I did around 4500 in a similar area, and my hair is much more fine than this guy's, so he would need less. I personally worry about the transection rate when adding a large amount of beard hair to a smaller area of scalp.

 

Round Three: Finish the top and determine whether or not to address the punch scars in the donor region with more beard. Also, consider adding some density to the plug grafts located near the crown. It may be cosmetically the best course of action to leave them there and add hair around them, if this is feasible and not requiring too many grafts. Removal of these plugs will leave more tell-tale scarring.

 

And also I would be consulting with HIS about SMP into those plug scars. Kudos to Dr. Lorenzo for working on such a challenging case, massive improvement thus far!

so he did go ahead with another surgery,,,, mixed feelings here....

wish he'd just shave it and then seek out scar repair with HIS and FUE with remaining donor

 

maybe Lorenzo can make his magic work though, will check in for the next segment

I hope Dr Lorenzo can fix this patient up. The 5 month post-operative shots are showing a pretty significant improvement already...

This is a very ambitious case. Most other surgeons wouldn't touch a case like this. Kudos to guys like Lorenzo, Umar and Bisanga for giving these poor guys a second chance.

Must be costing the poor guy a fortune just disguising the previous poor work that has been done. I still am suprised they are taking from his donor area to be honest, it looks so over harvested as it is.

Wow, quite impressive. What are the future plans? Very nice. The good growth here proves that 'patient physiology' is probably used as an excuse for bad growth more often than it should.

Wow I really feel for this guy and looking at the pre op situation I really wasn't sure what you would be able to do for him. The front looks much better already but what is the plan for the donor area to cover the scars?
