Many people are afraid of using Finasteride due to the fear of permanent loss of libido or impotence. We have compiled many scientific papers which are published in very reputed journals and tried to give a summary of these papers. Also at the end we have highlighted about our practice of Finasteride in male pattern baldness. Anything which is written on web comes on search in google. Many things are not true. They are anecdotal incidences. What does the scientific data say? The scientific research and data are published in PUBMED, the online library for medical science. This is the most authentic place to know anything in medicine. There are many studies which suggest that Finasteride is nearly safe to use. Baring few studies most of the studies are positive. Also, we don’t have any medicine other than Finasteride and Minoxidil which are effective in preventing baldness and are FDA approved. How effective is Finasteride? Finasteride is capable of preventing the miniaturisation of the balding hair and reversal of thickness of the hair. So keeping the balance of EFFECTS and potential SIDE EFFECTS, we suggest to try Finasteride in every baldness case. In our experience most of the people do well and whenever there is any ambiguity or doubts in the minds of the patients, we ask them to stop it. When re-introduced, the same side effects does not recur! What is the Nocebo effect ? There is a phenomenon called NOCEBO; here those group of people develop the side effects who are informed about the side effects. So, we suggest that the medicine has to be taken with the consultation with the doctor rather than having the fear and phobia. The treating doctor with the vast experience over years should be able to titrate the dosage of needed with the ultimate goal of preventing the baldness.

Every hair transplant patient desires full coverage and density, and our Norwood 7 patients are no different. Most Norwood 7 patients are turned away by other surgeons, as the clinics are not confident in providing the patients with adequate results. However, at Eugenix we’ve been restoring grade 6/7 for almost 10 years. We have been handling grade 6/7 cases as a challenge since 2012. Our target was to give coverage of the total bald area with cosmetically acceptable density. We started using beard hair soon. After many cases we realised that beard is the not that trust worthy. Rest of the body hair are to be ignored. We split large mega-session procedures to ensure we do not over harvest the donor area, and to ensure the donor area maintains a homogeneous appearance.

Is there a difference between Caucasian Hair vs Asian/Indian Hair We have been constantly asked about our experience of Caucasian hair restoration surgeries. We have been getting Caucasians as patients since 2014-15, more so in last 3 years. In 2019 alone 20-25% of our practice constituted of them. Unlike the notion that Caucasian hair restoration is difficult than Indian hair restoration, it is other way round. Baring the dark colour of Indian/Asian hair, most of the features of Caucasian hair outscores the former. Be it the number of hair per graft, number graft per square cm, or thickness of the hair; Caucasian hairs are better. The amount of grafts required to cover the bald area is more in Asians than Caucasians, due to the shape of the scalp and the hairline position. Indian hair features are in between true Asian (mongoloid) and Caucasian hair. The northern and western part of India has most of the population which are similar to Caucasians, baring the colour of the hair. However, once we travel down in India towards south, the characteristics of hair changes. The north-east part of India has more Asian like hair features. So, in India, we do get patients from different genetic pools and carry a huge and varied experience upon different hair qualities. Our extensive experience with Norwood 6/7 patients, and how we approach these complex cases. Since 2010, when we started hair restoration practice, we were asked to cover the TOTAL BALDNESS by patients. They were not happy to only do the front and remain BALD on the mid and crown area! The desires of the hair restoration patients are to get: 1) Natural-looking hair 2) Dense hair 3) Hair forever 4) Complete Coverage We have been working on all these points in the last 10 years and to give solid answers to all our patients. This demanded us to forsake all our Dermatology and Cosmetic practices and focus only upon hair restoration science. With a one-pointed focus, we started delivering results that satisfied most of the points mentioned on the top and most of the patients. There was one particular patient who got close to 16500 grafts from us and his life changed and he became an actor. That kicked us to do more and more grade 6-7 cases and push our selves against our limits. We modified our technique, started using different instruments, worked upon body hair. We soon realized that trusting body hair beyond the beard is foolish. In fact, in the world literate it is difficult to find and gratifying result of chest hair and that lasts long. So, we concentrated only upon beard hair and scalp hair to provide complete coverage of the baldness. We smartly tried to give the GRADED density from front to crown. Scalp hair used on the front and the beard hair used to mix with scalp hair in the mid-scalp and crown. Guys who have extreme baldness has an inversely dense beard. At times, it was difficult to make some people get convinced about beard hair because they wanted to sport a beard! Even we take 3000-4000 beard, still, most of the people could have a great beard to the sport. All these things happened over last 8 years and it majorly started in 2012. The challenge to cover grade 7 baldness has changed the course of our career and we were forced to focus only upon hair. At that time no one in the world was doing grade 7 in one sitting and our works inspired many, all over the globe to attempt these cases. With more than a THOUSAND cases of grade 7 under our belt, we are much better placed to help future patients with grade 6/7 baldness.

Progressive baldness of the mid-30s is difficult to negotiate for two reasons; 1) Baldness is dynamic and we don’t know what the final stage is going to be. 2) The expectations of these young patients are high and they press for a lower hairline. In the current case, we tried initially to keep the hairline up in the first sitting. Somehow he was under peer pressure and wanted a lower hairline. Once he got a good result in the first go, he felt he could lower the hairline a bit. With such discussions, we agreed to lower the hairline by 1-1.5 CM and do some temple job. We did close to 1500 grafts for the second sitting, after 3 years of the first sitting. The hairline consumed 500 single follicle grafts. The outcome after 1 year of the first sitting (4 years of the second sitting) was great. Unfortunately, he is not willing to take Finasteride and we are chasing his baldness! He has a gene of extreme grade 7 baldness. Now we are prepared to do his crown as well. Message: if you’re a young patient, and your hair loss is progressing, Finasteride should be taken to halt further hair loss. As a clinic, we don’t want our patients to be chasing hair loss with surgery.

Mrs. Singh, a 70 years old lady had got frontal fibrosing alopecia, FFA, which was stable. She was diagnosed based upon the clinical history and biopsy. It was stable for ten years. We took the case up and we did 2500 grafts on the front and we did less density than normal - around 25 to 30 grafts per sqm were planted and after 7 months the results started growing and by 1 year it was amazing. We have been following her up for the last 3 years and the results have been consistent. Hair restoration is not only an option for men, but also women. Surgical hair restoration is a great option for women born with naturally high hairlines as well.To summarize, one has to make the right diagnosis and you have to ensure that there is no disease activity for many years. If you do less density and you follow the patient up, then it would ensure a long-lasting result. This is our experience.
