Hello @Pyrat sir, 1.The probability of shock loss of native hair is always present especially if magnitude of implants is huge in an area of pre existing hair. High magnification (6x and 8x) are useful in these cases for proper precision and hence determining the required density per sq cm. Finasteride 1 mg orally after surgery also have helped largely in such cases. 2. Around 5 inches long helps to make it look fuller of course since HT is merely an art of creating an illusion. But shorter or longer works depending on quality and texture of hair. Thank you !

Scar tissue is a compromised tissue in terms of vascularity, uneven thickness and is unfavorable for hair growth. Scar could be due to the 1) primary skin diseases or 2) due to trauma, surgeries, etc. 1) The scar tissue needs to be inactive in terms of disease activity. At least a disease-free period of more than 5 years is required to think of doing a hair procedure upon the scar due to skin diseases. And a test grafting should be done before taking up the full area for the procedure. 2) In case of traumatic scarring or iatrogenic scarring, at least a period of 6 months to 1 year should be waited before doing the procedure. Since the blood supply is less, dense packing of 40-50 grafts per square cm should not be attempted. Around 20-25 grafts should be attempted. If the hair is put at the right depth then they would grow, if the blood supply is adequate. Scar tissue is most of the time emotionally challenging whenever it is on face or scalp. Experts who handle scar well with the experience of growing hair over it can produce highly gratifying results.

He got 3000 Grafts in first sitting for frontal zone then 2000 in second sitting for midscalp. Recently we had done another 2000 grafts on crown where result is awaited. 2000 grafts are taken from his beard. His scalp donor was good and has adequate donor even after 3 sittings.

We describe a case study of 50 yrs old person who did hair transplant (in the past he had done punch grafting hair transplant in 2005 somewhere else) with us 5 years ago. We did FUE of 5000 grafts and gave coverage to frontal and mid-scalp. Second, sitting: he was happy but wanted to cover the crown also. He was also amazed at beard healing and the results of beard transplant. 3 years ago: he was happy with the coverage. He wanted to do something for the punch scars in the donor. So we did scalp micro pigmentation, SMP. 1.5 years ago: he was very happy albeit the sparse coverage of the crown Learnings: If beard has good characteristics then it can serve as an excellent source for coverage of the baldness. Despite poor scalp donor, meticulous extraction and an intelligent mixture of scalp and beard can provide visually satisfying coverage.

REGRETTING HAIR TRANSPLANT ! We have started getting more than we could imagine and handle “Regret Hair Transplantation” cases! These cases are typically 1) Unnatural looks 2) Unnatural hair line position 3) Recognisable “Hair Jobs” 4) Progressive baldness with bald “halo” around the new fake looking hair 5) Abnormal temple points and unnatural angle, direction and “curl” of the hair. It is always difficult to fix these cases, .....most of them have gone to some extent in depression and social recluse....Their initial enthusiasm has turned into Regrets... In last 1 year, we must have done more than 100 repair cases, and we do fear handling these cases for many reasons. The most important being, these are DIFFICULT to do! Extracting the grown hair from the the frontal area and then ensuring an invisible scar on the face is very very challenging. And on pigmented skins along with the risk of post inflammatory hyperpigmentation due to sun exposure, it is much more challenging and difficult to take the responsibility of a great outcome on the “FRONTAL” donor area! We pray, patients do a long research and “Do it Once, but Do it Right!”
