Dr. Thiago Bianco Leal 3361 Grafts - Full Year Progression

Well, it has been a year, and as many people will say, 1 year is a great time to get an idea of what the final results are for a hair transplant.  I've updated my original thread once per month at least, and that thread has grown to 11 total pages, so it's quite hard to really go through in a time efficient manner.  This thread is to put everything into one place, the full progression, from day 1 to the final result. 

Important information for those who want it:
Clinic:  Dr. Thiago Bianco Leal (Brazilian Clinic)
Graft count:  3361 grafts
Hairs per graft : average of 2.2
Density between 75-81 grafts/cm2

Medication:  I didn't take any medication prior to the transplant.  After the transplant, I was on a unique cocktail of cialis/viagra/arginine/minoxidil (4 separate vasodilators).  My understanding is that this is the my doctor's go to approach for densities as high as mine, as we all know, it is assumed that grafts have to fight for blood flow, and at such high densities, there is risks involved (up to potentially necrosis).  This is one of the things my doctor does to get as much blood flow into the scalp as possible.

Also important to mention that I have no male patter balding, as I've had the same hairline for almost 20 years.  Putting this many grafts in such a small area makes sense in my case, but not for anyone who is actually losing hair.  Grafts are a limit resource, and if you will have more loss in the future, it is important to use them sparingly.

I wanted to make this thread for a while now, to have everything in one place, and also to have information such as the Dr. who did the procedure in the title, for easier search options for new and existing users.  I don't think there's much need to do any more picture updates after this, as after this point, I doubt there will be any changes, and I'm quite confident this is the final result.

Looks great, but could you also include pics of donor area also, as that is quite an important aspect of a HT

Yeah, huge texture improvements.  At 6-7 months, I could still see what hair was transplanted vs natural, due to texture.

Looks great! My timeline is really similar to yours, I'd be thrilled if I had a similar result. I went with Hasson and Wong and noticed they used almost exclusively doubles and triples for my hairline. Did your doctor do this? I heard that singles are necessary at the hairline to prevent a "pluggy" look. My hair is wavy and has always been styled down, so not sure that's an issue, but I was curious nonetheless.

They use doubles and triples? Are you sure?

I don't have the exact distribution of hairs, but the average hairs per follicle they used on me was 2.2  

Yours seems to be 2 exactly.  So in my case, they used more 2s-4s than in your case percentage wise.  

For a natural hairline, only the first row or two are singles, then behind that, they pack on the doubles and tripples to make it nice and thick, and so that the hairline isn't transparent.  Those 300 are probably more than enough to get that looking great (I'd trust H&W to know what they're doing).

I think it's a really cool protocol he uses.  It's a fact (at least a generally agreed upon one) that the higher density you go, the more starved the grafts get for blood flow due to competition from other grafts.  You need to do something about this to guarantee graft survival.  Cialis and Viagra are very well understood and they do increase blood flow by a lot.  I'm pretty sure this also applicable to blood flow to skin (because when I got on the pill he gave me (combo of Viagra and Cialis) my face was kinda pink/flushed for the first 3 days (blood flow).

 

He doesn't use that protocol for normal density or even dense packing (50-60), only for what he calls ultra density (75+).

What an incredibly unique case, and an extremely aesthetically pleasing, dense NW1 hairline. If donor supply wasn't finite, I think many people would aim to get results like you. Perhaps if future verteporfin trials prove to be effective in regenerating donor hair, results like this may become more achievable for more people.

@Fue3361 I'm very curious to hear the full protocol, including dosages and times taken per day. I'm surprised that you were on both Cialis and Viagra. Their mechanism of action is pretty much the same, and Viagra lasts much shorter. So I'm wondering why the doctor didn't go with Cialis alone.

Yeah.  My results are even better now that we're almost 2 years in.  It's looking great.

 

As for protocol, was just one pill a day, for 3 months.  The pill, if I'm not mistaking had 2.5mg of Cialis, 2.5mg of Viagra, and it also had 5mg of minoxidil + arginine.  If you really want to know, I can look for what the Arginine dosage was, although I think it was 5mg.
