PATIENT'S DETAILS

 

33 years old, Diffuse hair loss pattern (Ludwig II), losing hair since the age of 23, with a hair loss history in his family (father NW VI).

 

✓ Started taking Minoxidil 5% since 1 month before the surgery.

 

Medium caliber hair.

 

Normal follicular units.

 

Normal % of hair in telogen phase.

 

 

 

RECOMMENDED TREATMENT

 

This 33-year-old patient presented a diffuse thinning affecting the whole of his scalp.

 

Despite that, he is a very good candidate for a hair transplant procedure: his hair is slightly curly and his donor area is excellent. Moreover, he had started taking Minoxidil 5% since 3 months before the surgery - even though the medication turned out to be not very effective for the patient.

 

We agreed upon performing an FUT procedure and the patient accepted to shave the whole recipient area, which allowed for a perfect work. In this first session, we decided to focus on the frontal area and midscalp restoration, leaving the vertex untouched at this stage. The procedure was carried out in June 2017, when he received 3556 FUT grafts.

 

As you can see in the pictures below, the patient has experienced a quite fast regrowth, since at the 7th post-op month mark the change in his look was already apparent.

 

The patient will come back for his second session next month of April. His second procedure will consist in a combo FUT + FUE during two consecutive days with the aim of covering the vertex area. We will not hesitate to share with you the pictures of his second surgery as soon as they will be available.

This patient has just undergone his second surgery, a combo 2550 FUT + 1052 FUE (3602 grafts in total, some of which have been extracted via BHT from his beard donor) which gives a total of 7158 grafts extracted thus far.

 

After this second procedure his donor appears still in good conditions: if he will need a further procedure in the future, I'm confident that we could extract another 1000/1500 grafts (via FUE and/or BHT) without any problem.

 

The pictures and documentation of his second surgery will be posted shortly.

As anticipated in the previous post, the patient came back in April to undergo his second surgery with the aim of covering the vertex and reinforce the density on the adjacent areas.

The procedure consisted in a Combo FUT + FUE, during two consecutive days, thanks to which we have been able to harvest a total of 3602 grafts (2550 FUT + 1052 FUE)

The patient has recently reached his 6th post-operative month and has sent in a set of pictures showing his current results. He is enthusiastic about the regrowth rate and the cosmetic improvements achieved so far.  

On a side note, it is interesting to notice how the lighting conditions can significantly alter the perception of density (cf. the last two pictures at 6 months post-op, taken with flash, with the previous ones, taken under natural light).     

 

SURGERY DETAILS

3602 grafts

Technique: Combo FUT (2550 FUs) + FUE (1052 FUs)

FUE scoring and extractions executed with my WAW system, now widely used in the sphere of FUE. It consists of a very precise pedal that enables to control the movement of the punch, that I also designed myself. The main advantage is to reduce substantially the number of damaged and transected hair, thus to raise the quality of the procedure. The system is now used worldwide and several dozens of surgeons or centres are already using it.

Incisions: 35-40/cm2

Grafts Breakdown:
☞ 369 Single FUT      +   472 Single FUE                 = 841 Single grafts
☞ 1633 Double FUT  +   445 Double FUE grafts    = 2078 Double grafts
☞ 548 Triple FUT       +   130 Triple FUE grafts      = 678 Triple grafts
☞ 0 Quad. FUT           +   5 Quad. FUE grafts          = 5 Quadruple grafts

Total number of Hair: 5279 (FUT) + 1772 (FUE) =  7051

Average Hair/Graft: 2.07 (FUT) + 1.68 (FUE) = 1.87

Is there any reason strips are never taken vertically as opposed to laterally?

 Melvin, I may be wrong but are you asking why he strip of tissue is never harvested vertically on the scalp instead of laterally at the occipital ridge? If so, then I can  give you an answer to that although there are certainly many reasons. For starters, you can take much longer of a strip laterally then you can vertically since you can essentially goat ear to ear. A longer strip means more follicular units for trans-planting. Secondly, scalp elasticity is greater laterally than it is vertically.  As an exercise, try taking your hands and putting them on the sides and back of your scalp as if you were going to do scalp exercises (do a search on this forum for scalp exercises if you need more information on this)  and then move your scalp up and down versus side to side. You will see that there is more elasticity when you move your scalp up and down. That means you can not only obtain a longer strip laterally, but a wider strip which again equates to more follicular units   

One might argue that you can take several vertical strips as opposed to one horizontal/lateral strip which may equal to the same number of follicular units. However, consider scalp elasticity and how healing will be affected if you take multiple vertical strips.  Simply put, it won’t heal correctly and the patient will be left with several wide vertical scar is running up and down their scalp.    In the old days, surgeons actually used to take several horizontal strips instead of harvesting the old scar with a new strip of tissue containing follicular units during subsequent procedures. But this always lead to less than optimal scarring which is why  surgeons always harvest the same area. It was also found  that the optimal area for harvesting a strip is along the occipital ridge of the scalp running horizontally/laterally. It’s optimal for the simple reason that scarring is nominal compared to other places. 

 I’m sure there are other reasons but these are just a few, assuming I understand your question correctly.
