Senior Member Dr. Sheldon Kabaker Posted November 25, 2014 Senior Member Share Posted November 25, 2014 This 51 year old women, with a very high forehead, has always had a wish to have it lowered to give the appearance of a smaller more feminine forehead. She had no history of hair loss or prior surgery of the scalp or face. She is shown before surgery, and on the table from the surgeons view at the top. This view shows her before (after having scalp expansion) and immediately after the definitive hairline lowering operation. She is shown before and after one day after surgery, in the last 3 pairs of pictures. She has had the equivalent of 4000 follicular units being moved. Senior Member Loags79 Posted November 26, 2014 Senior Member Share Posted November 26, 2014 To my eyes, she appeared more attractive before her transplant. I think the hairline doesn't match her facial structure and features at all. my HT thread: http://www.hairrestorationnetwork.com/eve/175267-dr-radha-fut-3261-grafts-photo-word-heavy.html

I think the hairline doesn't match her facial structure and features at all. I couldn't agree more.. Okay she had a high hair line but she also has a mop of hair & as far i can see the hairline looks to low & she looked a lot better before HT. If she's happy oh well good...not my cup of tea tho. Senior Member Cryingoutloud Posted November 26, 2014 Senior Member Share Posted November 26, 2014 No, KO. They cut out the skin and pulled the scalp down towards the eyes, then stitched her up. She looks weird now. Her high forehead really suited her facial features but I'd imagine she's probably regretting the surgery now

Senior Member Rootz Posted November 26, 2014 Senior Member Share Posted November 26, 2014 I think this is a good example of a lower hairline not necessarily being better than a higher one. I think the ideal position would have been about in the middle from where she started and ended up. That said the result pics are immediately post op and I think this makes the result seem a bit worse as the hair is dried together and very unflattering. With the right hair style I bet she will still look better than preop.

Senior Member Dr. Sheldon Kabaker Posted November 28, 2014 Author Senior Member Share Posted November 28, 2014 I agree with many of these observations. I am happy to see the interest in this subject and this case. The hairline was placed lower than what might be considered average for this facial structure. However, it is where the patient wanted it. We needed to expand the scalp for four weeks in order to get the hairline this low. Much time was spent designing the height and shape of the new hairline. Hairlines as low as this one do naturally occur in many women. The one day result with the the sutures in place may make the hairline appear 1-2 mm lower than the final long term result. However, it still will be on the low side. The patient is extremely happy with the result after having been very concerned about a high hairline for over 30 years. I will be sure to post later results on this case. Sheldon S. Kabaker MD. Senior Member KO Posted November 28, 2014 Senior Member Share Posted November 28, 2014 Thanks for the response...what are the red/blue dot like marks on the hairline? Furthermore, are the before pictures before the scalp reduction or after? 3382 FUE Lupanzula http://www.hairrestorationnetwork.com/eve/185463-3382-grafts-lupanzula.html

Senior Member Dr. Sheldon Kabaker Posted November 28, 2014 Author Senior Member Share Posted November 28, 2014 Medication, such as Dutasteride, can work nicely where there are dormant hair follicles as seen in male or female pattern baldness. In this case, with no prior hair loss, there were never any hair follicles in the forehead that could respond to medication. This case can be described as a forehead reduction or a hairline lowering. Some call it an anterior scalp reduction. No grafts were done . This type of operation moved at least 4000 folliculr units to the lower position desired by the patient. The left sided picture on the operating table view shows the effect of 4 weeks of expansion of the front half of the hair bearing scalp, a prior procedure in this case. The view next to it shows the result one hour later. The paired pictures show the patient before any work paired with the result only one day after the definitive surgery.
