  1. The four pre - operative views of balding 68 year old male before transplants. Paired pictures show before and seven months after 2400 FUT grafts. Awaiting further growth and coverage. The patient is quite pleased with the natural result.
  2. This 25 year old female was concerned with her high hairline. She had a hairline lowering procedure in March 2010 that lowered the hairline 2.6cm. Six months later she had 500 grafts to thicken the corners of the hairline. See the surgeons eye view before and after surgery. The first two pairs of photos show before and one day after. The last three pairs show her now, five years later. This case is presented to show early and long term results of the hairline lowering operation. No other hair operation moves as much hair in such a short period of time with early cosmetic results, styling and long term results with the hairline exposed. This operation is best for females with good scalp flexibility though some males with out family history of balding occasionally have this procedure.
  3. This 68 year old male presenting with frontal baldness thinning mid scalp and crown desired better groomed appearance. (Strip harvest) 2400 follicular unit grafts done on March 4, 2015 with follow up eight months later. Patient considering a second session. The first photograph shows patient before surgery. The next photograph shows the patient after 2400 graphs to frontal area. He is shown in the last three comparative photo's before and eight months after surgery.
  4. This 28 year old female has always had a high hairline. Her original hairline was approximately 8.5 cm (shown in the first photograph) The hairline was lowered 2.6 cm, which is equivalent to approximately 3500 follicular unit grafts of density packed hair. She is shown in the second photographs before and immediately after surgery. The following three side by side photo's show the patient before and one day after surgery. She traveled internationally for this procedure and was able to fly home after her Post Operative appointment. This case will be updated in the coming months once her current photographs are received.
  5. This 25 year old female presented with a hereditary high hairline. The first photograph shows a side by side view surgeons view before and immediately after surgery. Her Hairline was lowered 2.75 cm, which is equivalent to approximately 3500 follicular unit grafts. The patient is seen in the last three comparison photographs before, next day and 14 months post operation. She may choose to have future grafts to the right side cowlick. This occurs in approximately 10% of cases.
  6. This 43 year old African-American female has always had a high hairline. Her hairline was lowered 2.5 cm, which is equivalent to approximately 2500 follicular unit grafts. The first photo was taken from the surgeons view before and immediately after surgery. The following four paired photographs show the patient before and one day after surgery. She returned to work after 4 days with her hair combed over the sutures which were removed three days later.
  7. Last year I posted a one day result of a hairline lowering operation on a female patient. An astute observer commented that the hairline looked too low. In deed, he was correct as the suture line made the hairline look disproportionate to the patients facial features also some stretch back of the forehead and scalp was anticipated since this was an expansion case. Over time the result had the anticipated improvement. The first photograph shows the surgeon's view of the patient before and after the hairline lowering operation in October 2013. Following this, are the picture we posted of the one day post op results. Next, are two pictures taken by the patient at ten weeks. The last three side by side groupings are before, one day after and nine months afterwards. I welcome any further discussion.
  8. This 20 year old male who for two years has been extremely concerned with his high hairline and large forehead which is a familial characteristic. There was no history of male pattern baldness and the patient and his father had always had high hairlines since puberty. After thorough counseling, it was agreed that we would lower the hairline with a scalp advancement operation. This operation would be the only way to get the hairline lowered 3 1/2 cm with full density and have a result that is almost immediate. If there is hair loss in the future, he would have either hair transplants or wear a hair system. The first two pictures show his preoperative condition, the third picture shows the before and after on the table from the surgeons viewpoint, and three paired pictures show him four days after surgery with sutures still in place. It is uncommon to do this operation on young men. However, there can be personal extenuating circumstances such as this patient, who needs to look a certain way to get on with his goals in life. We hope to have six month follow up pictures to submit to HTN. This is the equivilant of at least 4500 follicular unit grafts being moved. This operation is best done for young women, or the exceptional male who is not going to lose hair.
  9. This 36 year old female has always had a high hairline. She wished to have the hairline lowered by greater than 3 cm. The first photograph shows patient before surgery. The first side by side image is shown from top of head just before and immediately after surgery. The last three side by side images show patient before and one day after 3.2 cm lowering. Approximately 4200 follicular units moved.
  10. Thank you. I plan to post a 6 month or later result. I hope I was able to show how quickly a result can be achieved for women with hereditary high hairlines and no hair loss history.
  11. Thank you for your comment. I do over 60 similar female cases per year. I hope to post a longer term result.
  12. This 23 year old actress was concerned with high hairline. She has no history of hair loss. This pattern of hairline/ tall forehead runs in her family. She wanted her hair line brought down to where the green wavy line is drawn on her forehead in the first photograph. Her hairline was lowered approximately 3.5 cm. Which is the equivalent of over 4500 follicular unit grafts for someone with this degree of hair density. The first comparative photograph shows her on the operative table immediately before and after surgery. The next photo shows her before and one day after surgery. The following two pairs show her before and six days after. The last two triple comparative views show her before, one and six days after her procedure.
  13. First photo shown presented frontal baldness and thinning eyebrows before his procedure. He has had a total of 4000 grafts (400 to eye brows). The following three comparative views show patient at 1 year, before 2nd session and two years later. He has mentioned his eye brows need a trim every three weeks. He will follow up annually.
  14. This 61 year old female who had an endoscopic brow lift, which greatly raised her hairline. Five years ago she had 800 follicular unit grafts done by another surgeon. She was happy with the thin look, but wanted the hairline an inch lower. Rather than do more grafts which would take a year for growth to be long enough for styling, she had a hairline lowering. The paired pictures show her before and six months after her procedure. Note that hairs grow through and in front of the incision. Further grafting could be done if grater density is desired.
  15. 47 year old female who has always had a large forehead. She had no history of hair loss. She wished to have her forehead shorter in height and the width reduced. In January of 2014 she had the central hairline lowered by removing a two centimeter strip from the top of the forehead and pulling the scalp downward and stitching it in place. In addition, she had 560 follicular unit grafts put in from of the temples. The grafts were taken from the sides of the incision that was in the temples. Therefore, no additional donor scars were created. Picture one shows condition before surgery. Picture two shows the design on the new frontal and temple hairlines. Picture three shows her the day after surgery showing the lower hairline and grafts at the front of the temple hairline. Pictures four, five and six show comparative before and nine months after surgery.
  16. Medication, such as Dutasteride, can work nicely where there are dormant hair follicles as seen in male or female pattern baldness. In this case, with no prior hair loss, there were never any hair follicles in the forehead that could respond to medication. This case can be described as a forehead reduction or a hairline lowering. Some call it an anterior scalp reduction. No grafts were done . This type of operation moved at least 4000 folliculr units to the lower position desired by the patient. The left sided picture on the operating table view shows the effect of 4 weeks of expansion of the front half of the hair bearing scalp, a prior procedure in this case. The view next to it shows the result one hour later. The paired pictures show the patient before any work paired with the result only one day after the definitive surgery.
  17. I agree with many of these observations. I am happy to see the interest in this subject and this case. The hairline was placed lower than what might be considered average for this facial structure. However, it is where the patient wanted it. We needed to expand the scalp for four weeks in order to get the hairline this low. Much time was spent designing the height and shape of the new hairline. Hairlines as low as this one do naturally occur in many women. The one day result with the the sutures in place may make the hairline appear 1-2 mm lower than the final long term result. However, it still will be on the low side. The patient is extremely happy with the result after having been very concerned about a high hairline for over 30 years. I will be sure to post later results on this case. Sheldon S. Kabaker MD. Sent from my iPhone
  18. This 51 year old women, with a very high forehead, has always had a wish to have it lowered to give the appearance of a smaller more feminine forehead. She had no history of hair loss or prior surgery of the scalp or face. She is shown before surgery, and on the table from the surgeons view at the top. This view shows her before (after having scalp expansion) and immediately after the definitive hairline lowering operation. She is shown before and after one day after surgery, in the last 3 pairs of pictures. She has had the equivalent of 4000 follicular units being moved.
  19. 34 year old female concerned with high hairline and pronounced widows peak. She had no history of hair loss. Paired images show patient before and 6 days after one stage hairline lowering procedure, where her hairline was lowered 2 cm. Over 2400 follicular units were moved in this single procedure. The cosmetic result can be appreciated even at this early stage.
  20. This 27 year old patient (now 31) presented with hair loss of frontal forelock and thinning behind it. He had one session of 2084 grafts done in year 2010. The first picture shows a preoperative view from top and a picture taken immediately after the grafts were placed. Subsequent pairs show the preoperative condition in 2010 and the result after four years. The patient also has been on Finasteride which, in this case, has been very effective in preventing further hair loss.
  21. This 24 year old Hispanic male with very sparse facial hair had 2600 follicular unit transplants taken by strip harvest from back of scalp. Three pre operative photos are shown along operative outline and pictures during surgery. He is shown on 8th day post operative before most grafted hair fell out. He was able to shave and went to work the next day. He will realize his final result in three months. He wanted to have appearance of 2 to 3 day growth and stubble. Lack of facial hair is somewhat common is Hispanic and Asian men. Beard/facial transplant is becoming increasing in demand.
  22. This is a 23 year (first picture) old woman who has always had a high hairline. She had no history of hair loss. She is shown after one stage hairline lowering procedure where her hairline was lowered 2.75cm to the lowest edge of the thick marked line. Remaining paired views show one day after surgery where she was able to go out in public with hair covering her sutures. Slight swelling below her hairline represent deep sutures. This swelling subsides within two weeks. She had over 3500 follicular units moved in this one operation.
  23. 47 year old male presented with mild to moderate thinning of the frontal forelock and mid-scalp. He is shown before and after 2450 follicular unit grafts harvested with a strip technique and tricophytic closure. He is shown before and after 18 months.
  24. This patient presented at age 34 with minimal hair in the forelock. In 2006 he had 2200 grafts and then at age 37, in 2009 had another 1238 grafts. Show are comparative views of him at age 34, 37, and 42 demonstrating the progressive nature of male pattern baldness and repeated hair transplant sessions. Initial design of the hair transplant must take into consideration further hair loss when dealing with mail pattern baldness, and future planning of more sessions always needs to be considered. He should have enough donor material for another 3500 grafts, if needed, in the future.
  25. His early result was presented on Hair Transplant Network. This 60 year old male patient presented two years ago with loss of frontal scalp hair and eye brow hairs. He was initially treated with 2380 strip harvested grafts to the scalp and 180 grafts to each eye brow. A year later he had 1020 grafts to thicken the frontal hairline. The first composite shows his pre operative condition followed by pictures immediately after his first procedure in March 2012. Followed by his one year result. The last three pictures show comparative views at one year and after another year showing results of the transplant scalp and eye brows.
