5160 FUE. Unusual non-stable alopecia. Injertocapilar.com-Farjo. Dr. Lorenzo

Unusual case due to the high amount of grafts used and surgeries performed in the repair of the front third and middle area due to a non-stable alopecia despite using med's. Patient who comes in 2007 to our consultation, NW III, in treatment with finasteride, minoxidil and sintron (anticoagulant - background of cardiac arrhythmia). We decide to carry out conservative approach with only 1661 grafts. In 2009 he come back complaining of firewalls at the junction of the transplanted area with native hair. We agreed to insert 1200 units to solve the gap and reinforce the previous area. A year and a half later (2010) concerns about a new depopulation posterior to the transplanted area. The patient started with dutasteride and we performed a new touch-up with 1001 grafts (2023 hairs; 2.02 hair/graft; 0.75 mm punch; 55 microns of hair; 231UF1/ 522UF2/ 244UF3/ 4UF4). The new medication provides hair los stability, but the patient in 2012 beg and insist in a final procedure to increase density in the whole area. His donor area had had a very good recovery for which, with reluctance, we access to do the last touch-up with 1298 units (2656 hairs; 2.04 hair/grat; 0.85 mm punch; 322UF1/ 616UF2/ 343UF3/ 17UF4/ 1UF5). The patient was finally happy with the outcome.

very interesting case, guess he was trying to perfect it to put his mind at ease

 

did Propecia usage merely fail to stop his hairloss, or was it a case of it getting even worse while on the drug?

is he still on DUT now?

He has better hair/density than Elvis! Love the hairline also. Was it hand drawn by Dr Lorenzo or was the laser template used?

I have never seen so many grafts put into such a small area :D

2 poor unsatisfactory hair transplants performed in the UK.

 

Based on vast research and meeting patients, I travelled to see Dr Feller in New York to get repaired.

Obviously a really nice result, but even though he had 4 touch ups I think Lorenzo has had better hairline results. Video is a little blurry, a bit hard to see clearly though.

so this is what "hair greed" is? I can see this guy jumping off the London bridge if all is other hair fell out and he had no more donor. Hopefully he will be fine and he can live out his life with a ridiculous amount of hair. It comes to show how many grafts can be spent for ultimate thickness which is not even possible for most of us.

What is this patient's age?

 

Instead of using (wasting?) 5k grafts on this..wouldn't this guy's donor..and cash..best be saved for further down the road for future loss?

 

Great result..obviously...but this guy really needs to go spend $15 for a haircut.

Going from memory of when I watched the video, it didn't seem like he gained a whole lot on the last 1 or 2 revisions. But who knows maybe he thought it was worth it.

Having said that, getting that afro is basically like getting the message : youre not going to die tomorrow as scheduled, you have 20 years left.

Eh...unless he's still in his 20's or MAYBE very early 30's, it just looks stupid and highly inappropriate. If he's any older, I'd venture to guess that his overly-juvenile hairline may spark questions as to whether he's 'had work done'.

I would have held back, and so should doctors do generally. You can easily get a decent result with half amount of the grafts. But if the pat demanded 5k grafts, what can you do?

Insane Density. This is going to look really very odd if/when the guy has more/progressive loss.

Hes basically used most of his complete donor on the front alone. Not a good strategy. Was he warned against using so many grafts? I guess some Surgeons would not have agreed to do any more work.

They draw and lower the line in the first surgery (2007), by hand obviously.

He change to dutas after the third procedure. With finas, new thinning areas (firewalls) appeared behind the transplanted area, reason why we did the second and third surgery. With dutas the alopecia was stabilized. This patient is very attached to his hair, so it remains with it, yes.

Logically, the donor area was conscientiously and thoroughly examined before the 4th procedure. He is one of the lucky patients with a very dense donor area that have many grouping or families of follicular units that multiply the yield. The team has not doubt that his donor area has a potential of more than 10 k, as you can see before the 4rd procedure:

Thanks for the explanation. His donor looks amazing considering he had over 5,000 grafts extracted. He could probably easily get away with a grade 1 buzz.

Being that he had 5k in a small area - i dont see how another 5k over the rest of his head would be enough for the future. The density would be nowhere near the same.

Just my thoughts anyway. In the here and now he must be happy. Its just future planning i wouldnt be comfortable with.
