Greetings of the day @demezz2, I would advise you to start Finasteride 1mg daily or on alternative days, MPB is more aggressive during 20-30s. You seems to be NW2, If you can share more pictures and other needed information like age, pics of occipital then I can provide more efficient information. However looking at the picture I think around 2000-2200 grafts will be required. The exact graft count may vary during the time of surgery and It can be done in a single sitting itself. You seems to be NW3 and considering your age I think you should continue on medication. Finasteride will continue working, it will stop the conversion of DHT and prevent your existing hairs from baldness but it will not help you to grow new hairs. Finasteride side effects topic is always a topic of debate, but generally it has been seen in many studies that only 2-3% people face side effects, they do suffer short term side effects which improve without treatment. There may become men who very rarely suffer longer term or unexpected side effects. Most reported cases of sexual dysfunction occurred soon after starting the medication. Not to worry at all, as the sexual side effects can be reversed if you discontinued taking finasteride. Looking at your case, its good that you don't have any testicular pain and decrease in libido but considering the fact that erections are weak at night and semen is watery, the best suitable advice will be consult a good dermatologist/doctor. Taking low dose of Finasteride (on alternative days or twice a week) or topical Finasteride preparation. However, the efficacy of both these is yet to be proven. You must commit to take Finasteride even after the HT for longer time to increase the longevity of the transplanted area. Age seems to be a good factor in your case making you a decent candidate for hair transplant. You can opt for a fronto temporal transplant where yoy would require 1200 to 1500 grafts for your transplantation procedure. You wouldn't have to go for transplant on your temple points. It will make the hairline and the temples look natural and will frame the face.

If it is not improving with medications then 800 to 1000 grafts approximately shall suffice for the crown.

You would require 150 grafts each approximatly for the temples that you have roughly designed.

You should not transplant on the crown right now. You already have a lot of pre existing hair on the crown. You should continue with finasteride. 1500 to 1700 grafts are only for the frontal zone. For the temples, you will require 150 to 160 grafts each.
