Hard to say how it will look once grown out. Given that you have thick bushy hair it should look good. But that is of course, only if the growth is good. Only doctor I know in Virginia is Dr. Lindsey.

Unfortunately, I’m almost certain you will be a Norwood 7. I can see the signs now. Getting a hair transplant at this stage could be disastrous, especially without medication. Try to stabilize on some form of medication. Doing nothing is never a bad choice. You could opt for smp. I don’t think any ethical doctor would choose to perform surgery on a 20 year old with that level of hair loss and Norwood 7s in the family.

You don’t have to be rich or famous to not be a bald baby daddy.

I'm gonna be brutally honest. There is absolutely no way in hell you were born with that hairline. I would say you're hair loss is aggressive for your age. Without finasteride and minoxidil you could end up a Norwood 6 by 30. 1mg of fin 3x a day will still be effective enough to slow your hair loss. Because the half life is long, you don't have to use it every day to get results. You could also use topical dutasteride which is said to be just as or even more effective. If you respond well to medication. You may not need another hair transplant for a very long time. Now, you should definitely bank on having at least one or two more hair transplants through out your life even with meds. Now, i'm not sure if your expectations match the reality. You're a young aggressively balding guy. A dense Norwood 2 may not be possible. It all depends on how well you respond to medicine. I suggest getting on medicine for at least a year before deciding. In the end, doing nothing is never a bad choice.

Looks like some scarring. You can try fraxel laser surgery. That's why its important to understand hair transplants are a life long commitment. Thinking you can just shave your head after is not true.
