It was only by an accident that I happened to see the thread myself. I think most consent forms are pretty standard and straight forward similar to Shapiro Medical. I do have a busy work schedule and don't read majority of the threads that are posted. Sometimes I get little alerts or made aware of some threads via emails so that's what may need to be done to get more forms from other clinics. I'm sure most clinics would be happy to post it if they knew about this. Spanker, Our consent form has been in existence for over 20 years. I believe we took a standard form and modified it to fit SMG. Your understanding and mine are the same on telogen hairs. Shock loss hairs from ht procedures generally come back after 2-3 months but the weak or miniaturized hairs may not come back if they were on it's way out anyway even if that person didn't have ht surgery. Permanent shock loss probably did occur 15-20 years ago (or less) at clinics that practiced unrefined techniques. Now a days I would think it's very rare but it's a liability issue that clinics want to inform patients of all possible risks before undergoing surgery. Regular Member fritz68 Posted August 1, 2011 Regular Member Share Posted August 1, 2011 Based on my research with various clinics before my last HT, I've a lot of respect for the Shapiro group and how professional and up front they are with the pre-op info, and just as importantly, how flexible they are with the deposits, it talks mountains about their business ethics. I would think all of the top clinics would be similar in that regard. Ian, Like Janna said, most hair transplant physicians and clinics are extremely busy and it's rare that these individuals have the time to review and comment in discussion threads on a daily basis. Personally, I believe the lack of replies from clinic representatives has to do with time management and not seeing the thread in the first place. Frankly, I think it's presumptive to assume other clinics are avoiding the issue or provide complicated, disingenuous consent forms simply because they haven't replied in the thread. I hope this clarifies the situation! I don't want to jump to any bold conclusions simply based upon a lack of response in a singular thread. It's important to understand that informed consent for surgery is not just a piece of paper - it certainly is a critical tool in the process but there also has to be a very thorough disclosure of information in dialog between doctor and patient. This is a standard part of our consultation and pre-op preparation - we also provide a 16 page information booklet for our prospective patients.
