  1. We are replying to all the concerns in most truthful and honest manner

    At Eugenix, our approach is to first evaluate the donor area (both scalp and beard) and estimate conservatively what best we can harvest without damaging donor with excessive thinning and at the same time not relying too much on beard (as the beard hair growth turns out good to excellent in some patients and moderate to poor in others; probably dependent on one's growth rate of beard hair which is quite variable in most patients).

    So that's why in this particular case, we didn't over promise beard.

    We felt that 2000 grafts from safe donor zone can be extracted. We tend to avoid very thin hair or miniaturized hair to avoid poor results). The remaining grafts we planned to do from beard.

    So, there was no miscalculation of grafts as we had clearly demarcated the area that we are planning to cover. Also, our plan was to keep midscalp density lesser so that maximum area is covered.

    At the time of pre surgery counselling, we clarify completely that the transplanted hair provide full look in dry state. However, if the hair are made wet with water or oil is applied, then it will look sparse and skin will show. (However, we don't have documentation of the video of face to face consultation in the clinic 😔 but these points are mentioned in pre surgery consent form).

    At 8 months timeline, our patient was extremely unhappy with midscalp growth and wanted immediate implantation to fix the problem. He has mentioned that he is satisfied with frontal hairline growth.

    Regarding the document signing:
    In past some patients have been accusing us for poor results despite good growth and threatened to defame if refunds aren't done. Our approach is to document the results objectively and do our best to satisfy the patients. But refund in cases of dissatisfaction despite successful procedure is unwarranted and we have avoided that. To protect our interest, we had requested to sign a document that after this free top up surgery and producing visual change in appearance, we have completed our obligation and don't want to face further threats for refund or defamation. 

    But we went ahead to do our work and stick our commitment for happy experience by doing free top without signing of the document. If it was forced, then we wouldn't have done the surgery without the document. 

    We understand that satisfaction is inversely proportional to expectations and in current scenario; we have been unable to set the expectations leading to dissatisfaction. 

    In current situation, the pictures posted for the hairline have been clicked with flash of camera and giving see through appearance. 

    We are ready to do everything we can do to make the hairline more dense even now but we fear that Previous scarring may prevent the growth of all implanted grafts.

    Fine hair caliber might lead not achieving desired density.

    We will request the patient to guide us what he wants from us now to make sure that he feels that procedure was a success and not a failure.

    Topical finasteride gel is generally recommended to patients who do not wish to take the oral drug. Based upon the literature review, and our very limited personal experience, we have found that Finasteride gel is working well for many patients. Whether its efficacy is same, or lesser than oral Finasteride is not known and lack of studies on the topical version prevents us from taking a position statement. You may use finasteride gel alternatively i.e. on the days when you're not consuming oral finasteride. You can use 0.5 teaspoon once at bedtime, to be rubbed on to the roots of the thinning area.

    I would appreciate your response.

     I am on NW2 and scheduled for HT at Eugenix. I was quoted some 1700 Grafts for hairline and temple points. 

     I would like to know how doctor would match the number of grafts already in center vs number of grafts planning to  plant in receded areas as you can see in picture.  I just want make sure the center blends in with rest of the frontal area and does not look different from sides( may be because of low density)

    With the estimated 1700 grafts, we shall be able to provide you an optimum density on your receded areas, while keeping the pre-existing hair at the centre untouched. Also, when you take a picture of your hairline from the front it would appear to be in a straight line, compared to when you take a picture from the sides which makes it look M-shaped.

    Hello @StarTouch, Numbness or mild hypoesthesia after undergoing a hair transplant procedure is quite common and nothing to worry about. It usually rectifies itself on its own in about 30-45 days.

    One question, for you and all the experts is that I have noticed that Eugenix generally tends to shy away from operating on areas where there is already less dense native hair citing that it might damage them. I have seen other clinics take much more liberty to operate on places of existing hair to make them more dense. For example, i have a lot of sparseness in between my native hair but Eugenix suggested that its best not to operate on these small gaps given the risk of damaging existing hair follicle. Any idea what's the best policy in such cases?

    On my updates, I have completed 2.5 months now and starting to see really thin and small hairs popping out. To my naked eyes It looks like there's lot of new hairs that have germinated but in camera it still looks completely empty. I will post lastest pictures in sometime.

    Although implanting grafts in a thinning or non-bald zone is surgically doable and within the prowess of any experienced surgeon, it is absolutely necessary to take into consideration the disproportionately high probability of such implantation causing severe, and in some cases,  permanent shock loss of both the newly implanted as well as pre-existing hair in the transplanted zone. Such an outcome would clearly defeat the purpose of getting a hair restoration procedure done in the first place. 
    For a successful hair transplant outcome, when we're implanting between the pre-existing hair, the gap between the pre-existing hair should be 2 mm or more. Implanting grafts at a lesser distance, as might be the case in a sparse area, is therefore not advisable. A good hair restoration result, as you would agree, should be sustainable, which requires a good amount of foresight on part of the operating surgeon apart from the skill, artistry and experience.

    Here is another fantastic example of a hair restoration procedure done right! A total of 3510 grafts were harvested for this gentleman and although he has only completed 5 months since his procedure, his exquisite results are already beginning to show.     Below you can see a typical example of a partially transected single graft wherein you can see two hairs above the skin, giving the impression that it's a double (external appearance). However, down below it has a single root and only that particular single follicle would survive and continue to grow. Hope this clarifies.

    The redness has come down quite a lot. It's not completely gone, but greatly reduced. I've been applying E45 lotion to the recipient area to try and reduce redness. I can't compare how red it would have been had I not applied it - maybe the E45 hasn't done anything - but the redness has come down so I suspect it's helped.

    I've not experienced very much itchiness in the donor or recipient to be honest. The odd tingle or itch but nothing bad or bothersome. I know some people get a fair bit of irritation post-op but I've been largely fine.

    The recipient hair feels almost beard-hair like and indeed looks a bit different from the native hair. From a distance or in certain lighting it's not noticeable but closer up or in harder lights things start to look a little weird at this length.

    Some lingering doubts about the hairline design remain although I'm sure once it's all grow out long it'll be fine. The shortness of the hair right now exposes a lot. I've not succeeded in getting a close enough picture, but there definitely are some multi-graft hairs in the first/second row - especially the second row. How much of an issue this will be will reveal itself with time so. I feel as though transitioning out of the shedding phase and into the new hair will be an easier and more gradual transition than the pre versus post-op transition. I'm actually quite looking forward to the shed, as weird as that may be, as I've found the change in look to be quite drastic and harder to comprehend and accept than I ever imagined I would have. There are certain angles & lighting that I really like what I'm seeing and other angles where I'm less keen and doubtful.

    I've attached a few [slightly ropey] pics from today which marks the 2 week anniversary of the surgery.

The doubles that you’ve been noticing in the second row of your hairline zone, are in fact, partially transected single grafts. Partial transection happens as part of extraction in any hair transplant procedure due to the simultaneous axial & tangential forces acting on the graft while using a punch. Say for instance, a double graft can get partially transected or cut due to which only one of it’s roots will be extracted while the other will remain in the donor site of extraction. Hence, the unextracted root will continue to grow in the donor while the other root that we extracted is treated as a partially transected single graft. The external appearance of such partially transected single grafts can look like doubles or two hairs growing above the epidermis (skin), which is what you’re also noticing, but they are partially transected single grafts only.
