Hair transplant Advice

Hi there, I have an upcoming hair transplant booked with Dr das in January 2024. Ive been a little nervous just because my hair is diffusely thinning. Ive been thinning for 10 years, I'm 26 now. The top is really losing hair while the sides are slowly losing density and the back even slower. Just curious to get some advise on what to do. Ive been quoted for 4400 grafts with 1200 coming from beard. I'm on finasteride 1mg and topical minoxidil with dut 5%

Lol yeah unfortunately, 10 years of balding my friend. Ive been on medication for 10 months

In that case, give up on minoxidil and save your money. It’s not doing anything. 
 

You have aggressive hairloss and your focus should be on anti-DHT meds. Stay on finasteride daily. The crown will go rapidly if you stop fin. Ketaconazole 2% shampoo. 
 

4400 is a lot of grafts for someone so young with such aggressive hairloss. Maybe it would be better to wait until you’re 30 to see where your crown is heading and if medication stabilises you, use hair systems for now. 

But if you’re doing a HT now, use less grafts in just the hairline+midscalp without touching the crown for now and save your beard grafts for future. Your beard isn’t that great either tbh. 

Your beard will probably get better with age but fin usage is going to hurt it. At the same time you can’t stop Finasteride.

Thank you for the reply, I've got about 2500 beard grafts and a ton of chest grafts for later. I'm sticking with the finasteride.

Having lost so much already can actually be an advantage - you haven’t got much more to lose! So what you need is a very thorough assessment of your donor so that you know how many grafts you’ll have left after this surgery in case your crown goes and/or the lateral humps dip. As long as there’s enough to address future loss you’re golden. 
 

Id suggest your hairline needs to be as conservative as possible: maybe set slightly back from where yours is still clinging on so that if those miniaturised grafts do eventually go, you’ll have your new hairline just behind it to pick up where the original left off.

You are not a great candidate I'm afraid and that's actually a bit of an understatement =/

That doesn't mean you can't get a major aesthetic improvement from HT's though. Just be ready for the fact that future loss may leave you with an unnatural look that requires additional surgeries. So you need enough time, money, and donor supply to be able to do so. A lot of guys continue losing even on fin/dut it seems so just be prepared for the possibility if you're set on going through with it.

You could consider adding oral minoxidil. I was Norwood 7 and I found switching to oral minoxidil has strengthened my scalp and beard hair.

I really appreciate everyone's responses. I'm not looking for a full head of hair anymore, I wouldn't mind a more uniform look. Just don't want to be completely bald for the rest of my life.

Very aggressive hairloss for 26 years old, what is the family history of hairloss?

Seems your now NW5 with some thinning patch on the NW6, I don't see right now signs of NW7, but need to check if there's possibility in 4-8 years.

Theoretically if you'll stay about till NW6 with stable donor, you can have 4000+ session with your beard, then after 9 months to do more focused crown procedure.

Meds can help a lot in your case, just be aware of side effects.

I wouldnt go with Eugenix. Not that Dr Das is bad but thelong term planning isnt that great.

Zarev or Pitella is our best options both can get so many grafts.

I would wait for either of those 2 & in the meantime save.

I appreciate the response. Family hair loss is just my father with diffuse nw6/nw7. Not completely bald just like my situation just quite a bit thinner.

Hi, you need a total of 2 to 3 procedures to cover your baldness. First session you can go for 2400 to 2800 grafts. Save the donor area for future procedures a few years later. For the second procedure you probably need a scalp plus beard donor area. This second procedure will cover the remaining baldness which you will have a few years later. First procedure will change your look and personality. Topical Fin and minoxidil should be routine.
