Considering 2nd FUE for 2000 grafts

I had an small 1000 graft FUE done in 2010 and I was happy with the results for the last 8 years. However, now my native hair around my temples and hairline are continuing to recede and my previous recipient area is looking sparse, and there's also thinning in the mid-scalp area.

 

I'm thinking of getting a second FUE done and I've been quoted around 2000 grafts from a reputable doctor in Turkey. I'm not looking to lower my hairline but rather fill in my temples and hairline and hopefully address the mid-scalp area as well.

 

I'm a little concerned with getting an FUE within my existing hair - is it safe to place within native hairs? I'm also a little concerned with shaving my whole head as the previous FUE area will be noticeably sparse compared to my forelock.

 

I've added some pics of my hairline, midscalp, and temples. Any thoughts on my plan? I'm almost 38 years old and I've been on minox for 16 years, fin (0.25mg EOD) for 11 years, and use the laser comb fairly regularly.

It's completely safe to transplant hairs into your native hair. The whole shaving your head thing sucks but you just have to deal with it. Get a hat. That's why I've had both of my fue transplants done in winter lol so i could wear a hat and nobody would think twice about it. At 38 you're looking really good hair wise. Get your 2nd fue done and you'll be looking great before you know it.

It is safe to plant hairs within native hair, but not when that hair is still very dense, as yours is at the midscalp area. I'm going by the second photo you posted, with your hand holding hair back. The hair in that area is still far too dense to justify adding grafts to.

 

It does look like you could benefit from adding some density to your temples and probably across your whole hairline, too. Would need to see more photos to be sure.

Your hair looks good. I’m just wondering if you feel like your recession has stabilized over the past few years. If so....go for it. Mine stabilized in my late 30’s. I had my first surgery at 50. I’m sure 2,000 grafts will be plenty for your temples and touch up the hair line. Good luck to you.

It’s native hair .. because it’s usually already weak n miniaturized,, that’s why u r Transplanting into that area; they r the last survivors of MPB in that area n soon to lose the battle .. so if u traumatize them they will lose the fight sooner.. a good doctor would be able to tranpsplant hair around them n not cause too much trauma.. but eventually they will die off because it’s their destiny .. so u will have gaps n need more surgery to replace those native hairs later on in life .

The donor area usually grows back if experiencing shock loss from the transplant because it’s not affected by dht so the hair is still strong n thick n can survive the trauma of surgery..well that’s my understanding of things from doing my research :)

Shock loss is difficult and really impossible to predict. My recession had stabilized at your age (38) and I noticed no shock loss. That’s not saying that none occurred...it just means that it was unnoticeable in my case in both surgeries. Don’t worry about possible shock loss. Trust your doctor. Get the surgery and you will be glad you took the plunge. Keep us posted with pictures.

First off, your hair looks great and I think fixing the temples/corners will do wonders to make it look even better. I would focus on that for the time being and leave the mid-scalp alone for now. Did the 2,000 grafts they quoted you include the mid-scalp? And did the surgeon do anything to ease your mind about putting grafts in that area?

You're fine on the temples but the midscalp is far too dense.
