The African is known to have super curly hair roots which is very difficult to dissect and harvest. Moreover the skin is more susceptible to hypertrophic scar and keloid. However, we have many cases of African race coming for the hair transplantation and the results are acceptable. This patient is one of those. The patient is 30 years old and he wishes to transform his frontal hairline to female pattern. He has never encounter in hair loss/receding and has no family history of hairloss in the family. He had the surgery done last year [2010] and come back for the follow up in 2011. A period of one year gives him big changes. The hairline we created was drawn as figures, the feminine type hairline. The number of graft was 2,325 FUG occupied in the estimated are of 50 square centimeters. The ratio of the graft was: 1 hair= 544 FUG, 2 hair = 1,708 FUG, 3 hair= 73 FUG. The scar is acceptable, no hypertrophic change or keloid formation. He is very satisfied with the result.

A 52-year-old Caucasian male, Norwood IV with hair loss from the front and thining hair at crown and miniaturized hairs at the forelock and top. He wanted to transplant at front and crown. He didn’t want to take either finasteride nor minoxidil solution pre and post-op. The total bald area measured 67cm2 at front and 31 cm2 at crown. The ultra refined follicular unit grafting was done with strip technique and trichophytic closure. The detail of the hair transplantation is as follows: Frontal area: 1 hair: 329 FU grafts, 2 hair: 2030 FU grafts, 3 hair: 664 FU grafts and 4-5 hairs: 91 FU grafts. Crown area: 2 hair: 450 FU grafts, 3 hair: 150 FU grafts. A total of 600 follicular unit grafts. A total of 3,514 follicular unit grafts consisting of 7,590 hairs. The patient was followed at 1year post operative with good growth and significant cosmetic improvement. His donor area has minimal scar as shown in the photographs.

A 59 year old Asian male, Norwood V with balding area over the front and crown, miniaturized hairs over the top. He wanted to improve only the front part due to his limited budget . The total balding area measured 199 cm2 at frontal, top and crown. The ultra refined follicular unit grafting was done with strip technique and trichophytic closure. The details of the hair transplantation are as follows: Frontal & Top area: 1 hair: 204 FU grafts, 2 hair: 1613 FU grafts (3226hairs), 3 hair: 394 FU grafts (1182 hairs) and 4-5 hairs:237 FU grafts(1066 hairs). A total of 2448 follicular unit grafts, bearing 5678 hairs were transplanted. Patient has been on Finasteride 1mg 1 tab. daily and Minoxidil 5% lotion, 1cc twice daily. The patient was followed at 1year post operative with good growth and significant cosmetic improvement.

A 27 year old Caucasian male, Norwood III with receding frontotemporal hairline and thinning over the front wanted to have flat and lower hair line and temple restoration. The total bald area measured 51 cm2 at the frontal and temple. The ultra refined follicular unit grafting was done with strip technique and trichophytic closure. The details of the hair transplantation are as follows: Frontal area: 1 hair: 281 FU grafts, 2 hair: 1318 FU grafts (2636hairs) and 3 hair: 695 FU grafts (2085 hairs). A total of 2294 follicular unit grafts, bearing 5002 hairs were transplanted. Temple area: 1 hair: 50 FU grafts and 2 hair: 180FU grafts(360 hairs) on left side and 1 hair: 50 FU grafts and 2 hair:180 FU grafts ( 360 hairs) on right side were transplanted. We hope he will have markedly improvement in 1year after surgery.

A 58 year old Asian male, Norwood VI with balding area over the front and crown, miniaturized hairs over the top, wanted to have flat and lower hair line and temple restoration. The total bald area measured 183 cm2 at frontal, temple and crown. He has limited donor area. The ultra refined follicular unit grafting was done with strip technique and trichophytic closure. The details of the hair transplantation are as follows: Frontal & Top area: 1 hair: 529 FU grafts, 2 hair: 2445 FU grafts (4890hairs) and 3 hair: 453 FU grafts (1359 hairs). A total of 3427 follicular unit grafts, bearing 6778 hairs were transplanted. Temple area: 1 hair: 60 FU grafts and 2 hair: 90FU grafts(180 hairs) on left side and 1 hair: 60 FU grafts and 2 hair:100 FU grafts ( 200 hairs) on right side were transplanted. Total of 3737 FU grafts bearing 7278 hairs were transplanted. Patient has been on Finasteride 1mg 1 tab. daily and Minoxidil 5% lotion, 1cc twice daily. The patient was followed at 8 months post operative with good growth and significant cosmetic improvement. His donor area has minimal scar as shown in the photographs. He will return for crown transplantation in the near future.

Significant cosmetic improvement 11 months after hair transplant surgery 43 year old Asian male, Norwood III Vertex with receding frontotemporal hairline, thinning of hairs at crown wanted to have flat and lower hair line, temple and vertex transplantation. He understood and responsible for lowering his hair line quite low against my advice. The total bald area measured 181 cm2 at frontal, temple and crown. The ultra refined follicular unit grafting was done with strip technique and trichophytic closure. The details of the hair transplantation are as follows: Frontal area: 1 hair: 388 FU grafts, 2 hair: 1286 FU grafts (2572hairs) and 3 hair: 657 FU grafts (1971 hairs) . A total of 2331 follicular unit grafts, bearing 4931 hairs were transplanted . The patient was followed at 11 months post operative with good growth and significant cosmetic improvement. His donor area has minimal scar as shown in the photographs.

He just starts on Finasteride 1 mg a day and 5%minoxidil lotion twice a day after taking these photos and I hope he will gain more hair.

Significant cosmetic improvement 1year 1 month after hair transplant surgery 31 year old Asian male, Norwood III with receding frontotemporal hairline , wanted to have flat and lower hair line, temple transplantation. He understood and responsible for lowering his hair line quite low against my advice. The total bald area measured 56 cm2 in both frontal and temple. The ultra refined follicular unit grafting was done with strip technique and trichophytic closure. The details of the hair transplantation are as follows: 1 hair: 339 FU grafts, 2 hair: 1447 FU grafts (2894hairs), 3 hair: 301 FU grafts (903 hairs) and 4 hair: 78 FU grafts(312 hairs). A total of 2165 follicular unit grafts, bearing 4448 hairs were transplanted over frontal area. Both temples were restored with 1 hair: 70 FU grafts and 2 hair:100FU grafts(200 hairs) on left side and 1 hair:70 FU grafts and 2 hair:100 FU grafts ( 200 hairs) on right side. The patient was followed at 1 year 1 month post operative with good growth and significant cosmetic improvement. His donor area has minimal scar as shown in the photographs.

46 year old Caucasian male, Norwood IV with receding frontotemporal hairline and thinning at vertex, wanted to have flat and lower hair line, temple and vertex transplantation. He understood and responsible for lowering his hair line quite low against my advice. The total bald area measured 61 cm2 in both frontal and temple, and 48 cm2 in the vertex. The ultra refined follicular unit grafting was done with strip technique and trichophytic closure. The details of the hair transplantation are as follows: 1 hair: 528 FU grafts, 2 hair: 1418 FU grafts (2836 hairs), 3 hair: 197 FU grafts (597 hairs). A total of 2143 follicular unit grafts, bearing 3961 hairs were transplanted over frontal area. Both temples were restored with 50 (1 hair) FU grafts on left side and 70 (1 hair) FU grafts on right side. 500 (2 hair) follicular unit grafts were transplanted over the crown. The patient was followed at 3 years post operative with good growth and significant cosmetic improvement. His donor area has minimal scar as shown in the photographs. I understand that the patient expect a lot of hair growth and disappointed when the hair did not grow to his satisfaction. Asian hair are different from the caucasian's with less density. The result of hair transplantation varies from person to person depending upon the donor hair and of course, the skill of the surgeon and the team. Most experienced surgeons expect the growth rate to apporach 90%. The total number of hair is better than the total number of grafts when counting in hair transplantation. His recipent balding area measured 83 square cm. The number of follicular unit grafts transplanted are as follows: 1 hair (862), 2 hair (2038) and 3 hair (251), a total of 3151 grafts and 5691hairs. His donor density is 76.6%. so, the average density transplanted was 37.96% presumed all the grafts are growing well. He has a lot of single hair grafts at the donor area and not much 3 hair grafts. He is improved at 8.5 month postoperative however it is still early to conclude. I believe that he should wait another 4-8 months to see the final result of the transplantation session. If the density at 38% is not appropriated he might need more session to add on the density.
