This is a 58 yrs old male, Norwood Class VI, black mixed with grey hair with good donor density of 96% and average scalp laxity. He underwent hair transplantation, dense packing over the frontal and midscalp region, a total of 3767 Follicular unit grafts, bearing 8014 hairs were transplanted. The detail of follicular unit grafts are as follow: 1 hair graft = 305, 2 hair graft = 2677 and 3 hair graft = 785. The patient followed up at 1 year 4 months with a significant change in cosmetic frame of face and a very natural hairline. Donor closure with trichophytic closure healed with a minimal scar. He also planned to have hair transplantation over the crown area in the near future. Note: Patient is not taking any topical or oral medication for hair growth. 23 year old caucasian male, Norwood III with markedly receding fronto temporal hairline, wanted to have flat and lower hair line. He understood and responsible for lowering his hair line quite low against my advice.The total bald area measured 72 cm2. The ultra refined follicular unit grafting was done with strip technique and trichophytic closure. The details of the hair transplantation are as follows: 1 hair: 300 FU grafts, 2 hair: 1760 FU grafts (3520 hairs), 3 hair: 611 FU grafts (1833 hairs) and 4-5 hair: 136 grafts (544hairs). A total of 2807 follicular unit grafts, bearing 6265 hairs were transplanted. Both temples were restored with 125 (2 hair) FU grafts on left side and 125 (2 hair) FU grafts on right side. The patient was followed at 8 months post operative with good growth and significant cosmetic improvement. His donor area has minimal scar as shown in the photographs. 67 year old, Caucasian male, Norwood V with salt and pepper wavy hair, good density and scalp laxity. He has receding frontotemporal hairline with extensive thinning in the midscalp and crown region, the total balding area measured 142 cm2. The patient also has a curvilinear forehead lift scar and desires to reconstruct both temples, measured 14 cm2. The Ultra refined follicular unit transplantation with 3,530 Follicular unit grafts bearing 6,755 hairs was performed. The grafts were placed in the frontal to posterior midscalp region: 1 hair graft – 635 grafts (635 hair), 2 hair graft – 2,565 grafts (5130 hair), 3 hair graft – 330 grafts (990 hair). The Temples were also reconstructed: 1 hair graft – 150 in left and 150 in right side, 2 hair graft – 120 in left and 120 in right side. He started taking finastride 1 mg daily 5 months after hair transplantation. The patient followed up at 1 year with significant cosmetic improvement as below snaps. 59 year old, Caucasian male, Norwood V with light brown, curly hair, good density but poor scalp laxity. He has a receding fronto- temporal hairline, with miniaturized hair from the front to the crown, the total balding area measured 161 cm2. The ultra refined follicular unit transplantation with 3,586 Follicular unit grafts bearing 6,815 hairs was performed. The grafts were placed from the front to midscalp region: 1 hair graft – 430 grafts (430 hair), 2 hair graft – 2,433 grafts (4,866 hair), 3 hair graft – 273 grafts (819 hair). The Temples were also reconstructed: 1 hair graft – 105 in left and 95 in right side, 2 hair graft – 125 in left and 125 in right side. The patient did not take finastride tablet or applied minoxidil lotion for fear of side effect. He was followed at 1 year 8 month with good results as shown below.
