Wonder if anybody in India offers it. I looked up a few on google but the results were misleading (upon contacting, I found out those that've put SMP on their site don't even offer it!)

@thewalker10 -  did you find any good SMP surgeon/clinic in India, will be helpful if you share details.

Scalp micropigmentation or SMP (the process of tattooing the scalp to promote an appearance of naturally growing hair) is becoming an increasingly popular procedure for balding men, especially those who’d like to maintain a closely cropped head of hair with an appearance of stubble. This intriguing procedure is frequently used to camouflage widened scars from previous hair transplant strip procedures. However, some hair transplant surgeons have begun utilizing this procedure to aid in thickening the appearance of naturally thinning or transplanted hair. But is scalp micropigmentation for everyone? What are the benefits, limitations and practical usage of this procedure? Is scalp micropigmentation a viable alternative to hair transplant surgery?

Scalp micropigmentation isn't a new procedure, but due to its increase in popularity, some hair restoration physicians have begun to investigate and even incorporate it into their practice as an adjunct to today's state of the art hair transplant surgery. akc sorry I don't. But when researching for an SMP artist, approach it the same way you would with a hair transplant. Really do your due diligence and ask to see 'lots of photos of their work' and ask to see some of their customers. More importantly ask to look at photos of the work they have done in 'people who have had a transplant and wish to thicken the density visually.' If they do not have a great volume of work that you can see then move on. Most importantly too. ask them how long they have been performing SMP for. Many people have taken up SMP just to make money (not unlike many hair transplant surgeons - think Turkey, etc). With your black hair it should look very good! All the best!

@Gathsby - Thank you so much for these useful suggestions. I truly appreciate your help.
I'll check in this group about few suggestion for SMP surgeons in India.

Is the hair that you are stating is thinning native hair or transplanted hair? One option if the hair is stabilized with minoxidil and finasteride is SMP. I have had it done to help camouflage my donor scars and a large lateral scar along my hairline.

@Gatsby - Thanks for your suggestion. In the crown & mid-scalp areas believe Native hair are thinning. With short hairs it's less visible, but when I have long hairs, scalp b/w thinned hair is visible. giving patchy/bald look at the back.

Sure, I will check for SMP options. Do you know any good SMP clinic/surgeon in India.

Look into microneedling. The best one you can buy is the derminator from owndoc.com.

check out @CosmoKramer‘a thread for info. There’s another guy here who basically got hair transplant like results from a combination of the big 3 and microneedling. Not guaranteed to work for you but worth a shot.

@Sam818 - Thank you for the suggestion. Will surely give a try.

Tough to give advice with such limited info. Need before and after pictures, meds, age and surgeon.

Hi @bismarck Thanks for your reply.  @Melvin-Moderator also asked for these details which I recently shared with him. I'm forwarding those details to you as well.

    -------------------------------------------------------------------------------------------------------------------------------------------------------------

For your detailed understanding, I'm enclosing surgery details with pre/post operation pictures:

    > Total Surgeries - 2 (Same Surgeon)
    > First Surgery Details:
       Type - Strip (FUT)
       Grafts - 4,200
       Distribution: a) Front & temples - 2,700 Grafts
                     b) Crown - 1,500 Grafts
    > Second Surgery (Was more of a revision due to Frontal Visible Patch & shallow temples (especially right one), had uneven/patchy hairline, patches were making the transplanted grafts/work visible/noticable)
       Type - Strip (FUT)
       Grafts - 1,200 t0 1,500 (Front & temples)

    Concerns:

    a) Visible Frontal Patch - Understand it's difficult to get high density with transplanted hair, but if you closely check the front circular patch, you'll see that right in the middle, hair are missing, making the patch visible. Post first surgery surgeon mentioned that patch could be b/c of some press that restricted hair to come out, asked to go-for second surgery (chargeable) for rework of front and side temple patch. After 1+ year of my second surgery today results have deteriorated. Pls have a look at the picturs.

    > Shallow side temples - Right & left temples that were reconstructed by lowering down the hair line are shallow. Again understand, difficult to achieve density, but if you closely look at side temple picture, you'll see another patch (without hair) at the end of right temple.

Surgeon recommended second surgery to fill both of these patches, but post surgery, instead of noticing any improvement, visiblility of these patches have increased.

    > Visible Scar - Understand & respect that despite surgeon's efforts sometimes scars are stretched, however, in my case:

    a) the scar is stretched both in thickness & length, making the strip area visible even after dense/long hairs at back
    b) the scar still has redness & itchiness even after 1+ year of surgery
    c) could feel sensation/slight pain at the strip area at times

I mentioned all these concerns to the surgeon, initially he suggested a cream to reduce scar visibility, later when post few months it remained same, he suggested for SMP, which he did, but that lasted for few days & faded with wash. Pictures were clicked post SMP, do you see traces of SMP anywhere on thi

It'll be helpful if you could look into these concerns, I've been following up with my surgeon on these concerns for months now, but he's unresponsive, & suggesting another surgery.

The scar does look like it stretched, you can either transplant beard hair in to the scar or have SMP. I don't think it's anything out of the norm, stretched scars can happen, despite the surgeons best efforts. As for the circular patch, can you share some pictures before you had surgery and post-op? It's hard to say without having all of the information, for example how many grafts did you have, what areas were restored etc.

    Hi Melvin,

First of all thank you for looking into my concern. I truly appreciate your time & suggestions.

Possibly a scar revision with grafts added in the needed areas? I’m no expert on repair work. Who was the surgeon?

@LonelyGraft - Thanks for the reply. Appreciate your support.

I discussed these concerns with my surgeon, pls find below the details:

    > For Frontal-Patch, he's sayin that sometimes due to press hair don't come out properly, firstly suggested to wait until full growth and then suggested to hide patch with different hair-style. Surprisingly after initial & then another (both chargeable)surgeries, he couldn't fix the patch
    > For Strip Scar, he mentioned that sometimes the strip area stretches, leaving visible scar, mentioned in future whenever I plan for nxt surgergy he'll see, did SMP at scar area that lasted until few hair washes
    > For Shallow side templates, he mentioned that purposely he added less graft on right temple b/c with side partition hair falls from left to right, difficult to understand why one temple would have shallow-lining, making baldness on temple visible.

@LonelyGraft - I've sent you a personal message, mentioning the details of surgery, it'll helpful if you could check & provide few suggestions. Surgeon is recommended on this forum, so I've requested Moderator Melvin, to have a closer look at my concerns.

@Melvin - Request to have a look into the details I've shared with you. Also, let me knw if you need any further details. Looking forward to hearing from you soon...

How long have you been taking finasteride? It takes some time to see it's results. You stopping taking minoxidil definitely sped up the hair loss, which should stabilize after you get back on minoxidil. After being on minoxidil and finasteride for 6 months see if there are any positive results. If the results are good, keep on applying the meds until you see the effects wearing off. After which you will need a 2nd hair transplant. In time however, you will definitely need to accept that you will need a 2nd hair transplant to add hair density to your crown area. Until then, you can buy some time with minoxidil and finasteride. So yes I suggest you to start using minoxidil 5% and in 6 months see the results. If you used liquid then try using foam maybe you will have less skin side effects.

I've been taking finestride for a while, but when my scap pimples accelerated, I stopped both minoxidil and finistride, but then later I resumed to finestride, I'm taking 1mg finestride daily, will continue along with minoxidil, thanks for your suggestions.

Fin and Min both have the possibility of shocking out hairs.  This would happen in the first 3 months and then between 6-12 months you should hopefully see improvement.

A good result is to slow down the rate of loss.

Just stick with the Fin for now and give it more time.

You crown is very similar to mine, though I just had a HT on it.  763 grafts (2000 hairs).

Thanks for the suggestions, I'll soon be resuming to minoxidil again as my scalp pimples have improved.

Just want to take your suggestion that considering the exisiting thinning, can it be managed, can the density be improved through medication and other non-surgical options, I am worried it's becoming more and more visible now and further thinning may lead to crown baldness, I had transplant an year back in the frontal area, but now crown area has started troubling, please suggest

Are you using anything right now for hair loss??

Thanks for your reply @Lohvisa. I had puss pimples in the crown area, so doctor suggested not to use any topical solution, it has been 6 months now that I've stopped Minoxidil & thinning has accelerated since then, I've been taking oral finestride medicine but that alone isn't helping. Now that pimples have improved, I'll resume to minoxidil.

@Lohvisa, request your suggestion to stop further hair thinning & to improve density in the crown area, earlier crown thinning wasn't visible, but from last few months it has started to become more visible, and I am worried further thinning may lead to crown baldness, please suggest.

There are two types of loss. The one you see and the one you don't.  The one you see, shedding, is normal.  Most believe 100 hairs a day is the norm, (the follicle gets tired of producing hair and it takes a vacation.  That follicle will resume doing its job in 3-4 months). Hair loss, however, is hair you don't see.

Go under a bright light and look at the hair in the temporal areas.  You'll notice strands that are nice and robust.  Others are finer and some are so fine you can hardly see them.  We refer to this as miniaturization.  The hair just dissipates and withers away.

Your hair is lengthy and it's getting in the way.  I am unable to see if there is loss elsewhere other than the crown.  If it's crown only...I do think you have a couple of options.

First, meds.  Propecia, Rogaine, Laser and PRP are the modalities we typically discuss when dealing with this debilitating issue.  The mechanism of action of each is totally different and thus, there is synergism when used simultaneously.  Fortunately, most tend to show benefit in the crown.  Be careful however.......

I find most people measure success by visualizing.  Obviously if there is more after 6 months, everything worked.  If you look the same, the meds did nothing.....WRONG!

Meds are intended for you to retain hair. So, if you look the same a year later, the meds did what they were intended to do.  (The unknown here is whether or not you would have lost anything in that time period). The meds are not intended to regrow hair.  Nothing regrows hair - except a transplant). If there is a visual improvement, it is because there was enhancement of the native hair.

Meds need to be continued to maintain the effect.  If you start taking them, stay on them.  Getting off them only results in waste of time and money. You'll resume losing as if you'd never started the medication.

2nd option is to do a transplant.  Go to the doctor and find out how much donor you have.  (To get decent density, it would take multiple procedures.  We call this area "the black hole." It can truly eat your lunch). The issue here is, if later you start losing elsewhere and you're out of donor, you may be stuck and not able to address the issue.

Give meds a good year....then decide.  This is the most logical way to proceed.

@LaserCap - Thanks for the detailed suggestions. I truly appreciate your time & knowledge. I'll definitely look for these options & share progress update in few months.
