6½ months in, I think we can call this a failure. But I've never seen something so strange.

Update:

Bad news, sadly. The last 'update' I posted showed real improvement over the span of a week. Around the same time, I began to take some fin/oral min to speed up the results. If anything, I have regressed since then. All in all, there hasn't been much progress since month 4-5 other than the initial hairline.

I went to a consultation with a specialist. He said he could see some single hairs, some twos in the region that is lacking. He could offer no specific explanation as to why that entire area has not grown. He recommends that I have an FUT to get more effective treatment due to my hair texture.

I went to Wimpole, too, and they could not offer an explanation. They did, however, offer a second treatment to 'fix' it. They recommended I wait a few months first, however.

I'm devastated, not so much so for needing a second procedure, but rather because there seems to be no explanation for this appearance. Was it the procedure? Was it a surgeon/technician error? Is it genetics? Was it the after care? Did the meds cause hair loss? Without those answers, it's hard to know where to turn.

Context: this pattern of hair growth (focused only on the outer hairline) was noticeable as early as month 2. Here is a timeline.

Future?

Will I need grafts removed to make room for another transplant? Or is there enough 'space' to add density? Will that transplant 'fix' it, or only slightly improve it? Do I need an FUT? Do I need another clinic? Or can this be easily remedied?

If you started Finasteride after getting those transplant results, it could just be an initial shed which is completely normal when starting meds. I’m surprised the clinic couldn’t tell you this. 
 

Stay on the medication and wait another 6 months then judge the situation.

This is probably the initial shed. Do you have any pictures of what your hair looked like before getting on meds?

Identical to what it is now. The OP is an accurate reflection of what it was. I began to take the meds around that time. Initial shed lasting 3-4 months into the medicine would be surprising, no?

That’s not true, his last update looked way better than it does now.

Really strange that fin or minox was associated with a shed of the transplanted hairs, as they normally shouldn't be DHT sensitive.

The patchy back and forth and sudden growth over a week makes me think of lichen planus or a nutritional deficiency (vitamin D is a huge issue for darker skinned people in dreary UK type climates, and supplements need K as well). I'm generally in favor of getting lab markers and biopsies with atypical growth patterns like yours.

Definitely keep us updated.
