She underwent hair transplant 2 years ago and thought it was not successful and later had scalp advancement 9 months later. She has obvious white scar along the frontal hair line, the hair line is straight and quite low. It’s difficult for me the design the new hair line to partially camouflage some scar and frame her face and add on density further inside. She also has cowlick at front. After multiple surgeries the blood supply was compromised and also scars, one will not expect excellent growth as in normal scalp. The weir hair direction from the lift, scar from previous transplanted and cowlick was quite challenging how to place grafts. I have attached B/A for comparison and I believe there is improvement after surgery but not no growth. This is only one year after surgery , it would be best to wait longer at year and a half to two years and reassess while the circulation is continue to improve with time and what is best to do. The hair loss is continued to progress, medication for hair loss is quite helpful to stop the loss. The original hair transplant was about 72FU/cm transplanted, so it wouldn't have been 100% virgin scalp, but it didn't have any noticeable scar texture/discoloration over the original recipient. The growth issue is also all of the areas behind the scar, in the areas of my previous transplant, not so much in front of it.
