This 33 years-old patient comes to consultation with an inestable alopecia, so we suggested to start finasteride and carried out a surgery 6 month after with conservative frontal design and a slight reinforce of the temporal picks. We extracted 2801 grafts (6614 hairs; 2.36 hair per graft; 43 a 49 microns of hair; 0.8 mm punch; 266 FU1/1348FU2/1104FU3/85FU4).

Clever approach, planning and execution. Best documentation in the business(videos, wet hair, comb through, parting, intra-operative, post-operative, mid-year updates etc).

I'm not sure any other doctor could have accomplished this result with only 2800 grafts. I think most would have gone through 4000+ trying to mimic this same result, and probably failed. The yield is superb, and usage of grafts incredibly efficient.

That is Dr Lorenzo's philosophy and ethics. Dr Bisanga also follows a similar philosophy. They prefer to use as few grafts as possible but because of their skill, the yield is usually excellent. I am seeing too many other surgeons using a superfluous amount of grafts on guys who are norwood 3 or 4. What if the surgery does not work out? What if the balding progresses?

Yes, if I had one criticism for Dr Feriduni (who I am booked in with), it would be that he appears to use a lot more grafts for comparable cases than Bisanga and Lorenzo seem to. Don't know if this comes down to commercial reasons or skill differential, but it is something I have noticed....

Every Dr has their own approach. Some are more aggressive with hairline closure (young guys tend to like but not always best for them) e.g Rahal, Feriduni so you will need more grafts, some are really conservative like Bernstein and then there are others that fall between the extremes like Lorenzo and Bisanga you mentioned. Some doctors I believe are more efficient users of grafts than others which is important as you ideally you want to get the best result with the least amount of grafts but this is not always possible when you bring in all the different variables.

Based on vast research and meeting patients, I travelled to see Dr Feller in New York to get repaired.

I'm not quite sure how efficient Feriduni is compared to the other top doctors, but I do know he is willing to go more aggressive than most. That might account for higher graft estimates on his part. For example, I was quoted 1800 by Lorenzo, 1500 by Bisanga, and 1800-2400 by Feridnuni. However the quote for Lorenzo and Bisanga was for a more conservative hairline (they both resisted doing the one I drew), while Feriduni was quoting what I had drawn. Ended up doing 2550 with Feriduni. Would have been interesting to know how many grafts Lorenzo would have used over the same area. But I do know if I had gone with Lorenzo I probably would not have gotten the hairline I wanted.

That is quite possible. I think a surgeon like Lorenzo and Bisanga would be more comfortable doing a more juvenile hairline if the patient is older, has stable hairloss, above average donor area, minimal loss etc where other surgeons have a bit more leeway on certain factors.

 

Quite interesting that there turned out to be a 1,000 grafts difference between Bisanga and Feriduni though. You must have received a killer hairline!
