Senior Member Drnigam Posted April 11, 2013 Senior Member Share Posted April 11, 2013 A case of NW7 for in-vivo Doubling / HM / DP - Day 1 @ Dr. Nigam's This is a special case with poor donor approx Bald area 36 cm x 7 cm = 252 sq cm Donor Area 22 cm x 19 cm = 418 Session 1 (Today - 11th April 2013) – 2268 bisected grafts from Donor to 2268 G to recipient Session 2 (tomorrow - 12th April 2013) – 2000 bisected grafts from Donor to 2000 G to recipient Session 3 (to be done) – 2500 bisected grafts from Donor to 2500 G to recipient In 3 session he will receive 6768 G This patient has received 1st shot of activated stem cells, DP cell & growth factor and will received 2nd shot after 6 weeks. We hope to convert this patient from NW7 to NW2 by 31st Dec 2013 with such a poor donor and unfortunately he is in India only for 15 days. Just to clarify, you are removing the graft with a 0.6 mm FUE punch (in vivo) and bisecting (in vitro) under magnification? Have you noted any unnecessary transection with the 0.6 mm punch? This is quite narrow, and I'm wondering if you're able to remove 3-4 hair follicular unit grafts, and whether or not you're seeing transection with any graft larger than 1 hair? We are the 1st clinic which uses Ultra Sound guided Partial Follicular Unit extraction. So we can see where we are bisecting it’s not blind technique. It’s a beginning of giving eyes to the Hair Transplant industry.Even fut/fue should be special usg guided. This is In-Vivo doubling plucked hair transplant with stem cells and DP cells. In this case I can go only upto 1mm in the epidermis unlike traditional procedure where they reach epidermis junction, then I tweeze and pluck the graft. As this plucked graft will not have dermal papilla and root so I inject DP cell and after 6 weeks, DP culture + Growth Factor + Stem Cells to repair the bisected follicles. On Day 2 69 donor extraction already showing regrowth, we expect complete donor regrowth to be reuse after 2-3 months after patient return from Malaysia. At present we have done donor doubling of 4417 G. In next 3 days we will be doing more 3000 G, 1000 G each day, which brings the total of 7417 G and the same procedure will be repeated after 6 weeks when the patient will return from Malaysia. You can see the quick healing & fading away of the marks. He has a very poor donor area, mostly single and thin grafts. The patient is from Malaysia and sent this pictures from there as he had purchase a videomicroscope for sending us videomicroscopic pictures. The patient will be coming after 1 month for his multiplied stem cell injection and further doubling of 6000 plus grafts. Till now he has not seen any shedding and the bisected implanted grafts are growing slowly and donor regeneration is also progressing smoothly. Donor Regeneration is up to 80% to 90% by 16 days as he has not send us all the picture of the donor so cant assess it properly. As you can see from the donor regeneration pictures there are no white dots visible and hardly any marks left there. Sorry I don't have much to comment on as I'm not really 'big' on the technical side of things.........but I'm glad for your research and you posting here! 2,000 grafts FUT Dr. Feller, July 27th 2012. 23 years old at the time. Excellent result. Need crown sorted eventually but concealer works well for now. Propecia and minoxidil since 2010. Fine for 8 years - bad sides after switching to Aindeem in 2018. Switched to topical fin/minox combo from Minoxidil Max in October 2020, along with dermarolling 1x a week. Senior Member Drnigam Posted May 15, 2013 Author Senior Member Share Posted May 15, 2013 (edited) With our doubling technique we expect more than 100% regeneration at the donor becuase of stem cells & DP cell injection effect on the bisected follicles and 80% to 90% growth at the recipient as on today. Also note, because bisected follicle needs to be repaired at the first stage and then will go in the shedding phase. Please find below the Day 31 picture The below videoscopic images (before-after) are not of the exact location (general before & after images of donor & recipient are provided) as these images has been sent by the patient from the Malaysia. These images of regeneration at Donor show excellent re-growth with hardly any marks (scarless procedure). ou can compare generally the density of donor & recipient. Once we receive the macro pictures within 2 to 3 days from the patient, we will be able to compare the present picture with before picture. We will definitely convert this NW7 to NW2 in maximum next 2 or 3 sittings once this patient comes back for the further procedure. Kindly see the increase in the density at the recipient although before pictures are of random area and not of the entire scalp. I don't see the tattoo/birthmark (I'm not sure which it is) in the day 31 images that is visible in the day 0 images.
