This fellow received 3300 grafts in the fall of 2010. Shown are intraop and early postop pics on days 7 and 30; and pics at 5 months when he was starting to show some early growth. He changed jobs and stopped by a week or 2 ago at 17 months out and looks great.

Thanks

Yeah, I agree. This guy is a home run. One of you visitors from England was here the day he came in....and met him in person. If you read this, please go on and add your 2 cents.

Doc -Were any grafts put into the crown? His crown shows huge improvement post op and I am not sure if grafts were put there or its just the pics.

I put nothing in the crown. Only the areas that are shown in the pics. Now I vaguely recall he's on meds now which may have helped the crown.

I almost never do front and back on the same day. I would rather spend all the hair in one area than to do 2 areas with less density, except in select cases or redo's.

I unfortunately did a crown and then up the partline on a fellow a while back. We planned about 200 in a select part of the partline and then 2500 in the crown. As the day went on, and after we trimmed up the partline, we ended up packing the entire partline with 5 or 6 hundred and then only had 1900 for the crown....and it wasn't enough. I had to do a touchup this year. And its both his and my faults....we should have stuck with the plan initially and we'd have solved his crown complaints. As it is, he'll likely never have partline problems again though...

Plus, although we don't piddle around all day, by the time one area is done, the patient is ready to leave and the placers are tired too.

Thanks Dr. Lindsey. Helpful as usual. Another question if you don't mind. Do you hear many patients come in where the biggest issue bothering them is their side profile? Specifically, the back crown laterials? When looking at me form the left, you cannot see any hairloss but from the right, the crown creeps down the side a bit and is rather anonoying. The top center crown does not bother me all that much and the bottom is still up pretty high. Anyway, the question is how many grafts does it typically take to take care of these crown laterials and sure up a side profile? Again, I know it is case specific but I am only looking for a general idea only. I had 5229 done by Dr. Rahal which restored my entire front and mid as well as the left crown lateral. In fact, I am amazed at how much milage I got out of the 5229. He put a few on the right crown lateral but I would want to hit it again for sure. I will be going in to see Dr. Rahal in the near future for a follow up but was just wondering your opinion on it.

Can't decide...

You lost me on your description. But suffice it to say that perfection is the enemy of good. If you look un-remarkable (whether after a hair transplant, nose job, facelift etc) you likely had a great job and shouldn't try for that extra little bit of achievement.

The halls of medicine are full of cases where "just a little more" resulted in a bad outcome.

Dr. Lindsey - I am not sure how I lost you but fair enough :)

To be clear, I am thrilled with my results. I could not agree more that going back for somthing very small like a 300 grafts tweak is somthing I would never do if I had essentially an otherwise complete restoration and it looked good. My crown could take 2500 grafts so we are not talking about going back for "Just a little more".

Absolutely I told spex that. But 3 things: 1. spex knows more about the pros and cons of hair than most if not all potential patients so if he wants to add a little more....well I'm ok with it. 2. spex has so much hair in the areas in which we worked, that only by really looking for it, could you find much of a preop hair "vacancy", but it bothered him...probably wouldn't bother a lot of folks at this point. And 3....i've known spex for a while....its a bit like a cousin asking you to do something. i know he's not crazy, and that he understands all the issues, maybe more than me, and its not likely to do any harm, and its likely to make him a little more content with the situation, so lets do it. For patients other than having this kind of relationship with me, I'd be concerned they would be in it for the wrong reason or be unrealistic.
