  1. This patient had been undergoing various forms of hair restoration surgery since the late 70s. These older procedures left him with poor cosmetic results that caused him to pursue additional operations. Any growth or improvements that he experienced ended up being transient. This journey lasted about 35 years until he finally found Dr. Umar’s videos on You Tube. Since he no longer had adequate supplies of head donor follicles, achieving the type of coverage that was needed for his repair required the use of body follicles. Dr. Umar extracted a supply of 7300 grafts from the beard regions, chest, head and nape. These were inserted to fill in areas of sparseness by adding greater density to these areas, cover scars from previous procedures and to advance the hairline. The follicles that Dr. Umar harvested survived and produced excellent yield. The extractions were performed using uGraft, the Advanced Follicular Unit Extraction technology that Dr. Umar designed himself. These punches include special features that enable the removal of unpredictably angled body hair to be safely harvested.   2. This patient underwent multiple strip harvesting procedures and a body hair transplant at another clinic. However, the results of all these surgeries failed. The patient’s level of hair loss remained at Norwood 6 since most of the inserted follicles did not survive. Also, he had a number of highly visible linear scars. Due to the depletion of head donor grafts, he was not able to have his situation repaired through traditional forms of hair transplantation. Dr. Umar was able to harvest a donor supply of 7000 grafts to produce the coverage seen in the patient’s final results. This consisted of: (1) 1800 grafts from the head (2) 4500 grafts from the beard (3) 700 grafts from the chest The removal of these grafts was achieved through an Advanced Follicular Unit Extraction technology which Dr. Umar developed called uGraft. UGraft has been designed to include special features which overcome the challenges associated with harvesting donor hairs from the body: (1) A component which gently pulls the hair follicles to help remove them from surrounding tissue attachments (2) Customizable punch units which create specially configured wound shapes which minimize extraneous tissue around the lip. Aside from the fact that appearances are highly linked to career opportunities and success in the entertainment industry, Bernardo felt that his progressive baldness was affecting his confidence as well. Secondly, he also wanted to cover a large strip scar on the back of his head. Like most men who are opting for corrective repair procedures, this patient had diminished supplies of head donor follicles on his head. A third challenge inherent with this case was the patient’s bald crown. Crown hair transplants usually require a significant number of grafts for coverage. In order to achieve the patient’s ideal outcome, 6,000 grafts were needed. Dr. Umar was able to acquire the required donor resources mostly from the patient’s beard area. This was done using his uGraft system of tools and techniques. With uGraft, advanced forms of Follicular Unit Extraction can be performed. General FUE punches are more suited for head hair extractions. However, when it comes to follicles from other parts of the body, they may incur damage to these tiny organs, due to their sharp growth angles. Also, since areas like the face are highly visible, the demand for pristine healing results is particularly high.   6. This patient of Derm Hair Clinic had a prior hair transplant. But he wanted to improve upon the overall coverage, particularly in the hairline and temple areas. From the before images, it is quite clear. Dr. Umar was able to achieve the patient’s goals by using only 1500 grafts. Advanced Follicular Unit Extraction was used to take follicles from the head as well as the nape. A main challenge of this patient’s case was for the additional grafts to be inserted in a way that looks seamless with the existing hairs. Natural looking hairline and temples are characterized by smooth transitions. This includes the contouring from the temples to the hairline. And this also includes the gradual transition of finer thinner hairs to ones that are thicker in caliber. Because of this, it is important to be able to harvest both thin and thick diameter grafts. Hairs from the head may look quite harsh along the hairline and temples. Thinner hairs may be derived from the nape or the patient’s legs. However, a specialized approach is needed. In other words, nape hair is not necessarily lost in all cases of MPB. What more? The safe donor area (SDA) is functionally the area bordering the fringe of hair that would be left in a terminally bald person. (NW7+). yet we know that the majority of individuals do not experience this degree of baldness. The role of nape hair has hitherto been defined by 1. The notion that it is generally DHT susceptible 2. It is not typically an area subject to harvest given the limitations of the strip harvesting methods available. The second factor has been cancelled by the advent of FUE. Since then nape and temple hair (donor areas outside the SDA) have been widely used in successful repairs of strip-donor- depleted patients. These would be patients whose capacity for further strip surgeries has been exhausted. It is the extension of these experiences with repairs that opened the prospect of using nape hair as the vanguard hair of transplanted hairlines. Obviously, patient selection is key. Using criteria such as the degree of baldness relative to age, visual evidence etc. nape hair can play an important role in hair transplantation. * For the purpose of this discussion, nape hair use in hairline and temple creation refers to hair in the area below the SDA including areas around the ears.
  21. Thank you for posting your feedback everyone! Here are some additional details about this patient's procedure to help answer some of your questions. The 10,000 grafts were constituted of head and nape hair. About 8000 grafts were derived from the head and nape while about 2000 came from the beard. The patient still has ample donor supply left in the event he is interested in future surgeries. The procedure was done over 2 sessions. first session of 9000 grafts (head nape and beard) and another session of about 1000 grafts (head). In the first session 9000 grafts (head nape and beard) were transplanted. And another session of about 1000 grafts of head hair completed the procedure.   22. It's amazing how much younger someone can look with a well done hairline. Take a look at this patient.
