Many times, hair transplant clinics often do not have the updated technology or foresight to map out the patient’s treatment.   4. The video above illustrates the subject of hair loss progression after hair transplants poignantly. Since hair loss is progressive, often time additional surgery is required to catch up with the effects of time at a later date. Importantly, it highlights what credible hair transplant clinics and doctors should discuss with all hair transplant patients at the time of first consultation. The patient should be given the full picture so they are able to make a rational decision with their long term interests and goals in mind. The younger the patient is at the time of surgery, the more time should be spent on this aspect of the consultation due to their unpredictable hair loss progression course. After reconciling with the aforementioned variables discussed with Dr U for younger individuals, the patient in the video received 3200 grafts 8 years ago at the age of 26 from the nape area to restore receded temples and reconstruct his hairline. This surgery left him with harsh and pluggy looking hairline. He was also left with a bald crown which he wanted to cover up by combing over hair from his hairline, but he could not do so because this would expose its plugginess. Thus, his priority was to soften his hairline for a more natural look so he could grow the hair long enough to comb over and cover the bald crown.   7. This patient had previously underwent surgery at a different facility before seeing Dr. U. His hair was still thin and he was left with a surgery scar on the back of his head.   11. This gentleman initially tried to conquer his male pattern baldness through a strip procedure performed at another clinic. However, his results were very disappointing. The operation exhausted his head donor supply. Yet, many of the inserted grafts did not grow. Thus, his crown continued to be bald. Some growth did occur on top of his head. But this created an unusual wall of hair in front of the crown. Furthermore, the strip scar was an additional source of worry as the patient did not want other people to notice it. In order to conceal the scar and cover the areas of missing hair, more donor grafts were needed beyond the quantity that remained on his head. Therefore, the patient felt that a body hair to head transplant was a very logical choice for achieving his objectives. 4213 Grafts of Head and Beard Hair Since Dr. U had developed technology specifically for body hair extractions (i.e. UGraft ™ Revolution) and possesses an extensive track record of tangible patient success stories, the patient decided to chose our clinic for his procedure. RE: Anticipating a Norwood 6-7 level of hair loss, and considering BHT early on Most patients destined for NW6 - 7 would show evidence of early signs of significant thinning that is not congruent with what their young age. Often this is evident as early as in the 20s. Apart from this consideration, and family history, Dr U uses a test he calls a "Shave Test" before performing FUE. In this test shaving the donor area reveals distinctly contrasting densities between the so called safe donor areas and the non safe donor areas. In patients destined for NW 7 especially, a line of demarcation is often visible by the mid 20s. Dr U's study on this subject is about to be published in a major plastic surgery journal. In such patients, Dr U recommends the appropriate integration of BHT in HT from the outset. Dr U is not a proponent of first exhausting the head donor before the use of BHT in such patients. RE: Question: "Would the pictured patient have been looking normal if he had gone for FUT to begin with" No. As a matter of fact, of the 3 patients shown in this video, only the first one had FUE surgeries to begin with.   13. There are many types of patient cases which require more grafts than what can be supplied by the donor follicles on the scalp. And this can be problematic when conventional hair transplant surgical methods are used, where the scope of expertise is limited to only performing extractions on the scalp. At our clinic, many of our repair patients seek help for typical issues such as: 1. areas where grafts from previous procedures failed to grow 2. misplaced or awkward looking insertions that have a detrimental effect on the patient’s appearance. However, another issue that we often see is the over harvesting of grafts on the back and/or sides of the head in an attempt to cover severe areas of baldness with the use of only head hair donor. This results in linear areas of emptiness, or wider looking areas of missing hair in the head donor regions. Excessive Donor Harvesting and the Need for More Grafts In these cases, previous surgeons attempted to meet the pressing demand for improved coverage by extracting an excessive number of donor hair grafts from the scalp, creating the additional problem of barren areas in the back or sides of the head. However, it is important to maintain the awareness that except in some extraordinary cases where donors are above normal; the scalp can typically only offer a maximum range of 5000-7000 grafts. For most patients with NW 6-7 hair loss level, this is inadequate to offer credible global coverage. The best that can be hoped for with head hair only surgeries in these cases would be NW 3ish hairlines with very sparse to empty crowns. Patients requiring youthful hairlines with global coverage and density should be looking to supplementing their donor with body hair transplants.   14. Hi Ben, Dr. U uses hair from all parts of the body. Determinations on the different graft types are made on an individualized case by case basis. Some common factors include: available quantities, coverage goals, need to create fine detail (e.g. evenness of texture, gradual transition from thin to thick hairs). Body hair is certainly capable of lasting long term. Here is an article from our site with an actual patient example and additional detail which you may find useful. Here is an animated video which explains how it works. The features have been designed to safely remove sharply angled follicles, such as those found on many body surfaces. Also the punch creates wound cavities with a specific shape that promotes higher standards of healing on the skin. Any scarring that occurs is typically cosmetically insignificant. We have many examples of patients who have agreed to be featured via photo and video who have had thousands of grafts extracted from their face or other highly visible area, and yet are able to feel confident about their appearance due to the quality of healing that resulted from their surgery.   15. Hi Win200. Before seeing Dr U, The patient had from 2 other clinics, 5290 FUT grafts and 200 FUE for a NW 5. The result is the before pictures in this presentation. He was not happy with the coverage, the strip scar and hairline finishing. Of the 4500 UGrafts used by Dr U in this patient over 500 went into camouflaging his FUT scars. The NW5 area was addressed with about 4000 UGrafts by Dr U
  16. Hi Sean, Generally for temple areas; Dr U uses one of the following: 1. Head and nape hair (if the patient qualifies for using nape hair) or 2. Head and nonbeard body hair (for patients that are not good candidates for nape hair use). In this case the first approach was adopted. Generally anywhere from 300-700 grafts would be sufficient for temple-hairline refinement work
  17. Blake: Here are close up photos of beard area. The chin is fine. He has mostly grey hair in his beard and the areas with black hair stubble are mostly in the chin area. That is what you are seeing. They are not scars. Down his neck, there are black hair stubble, but they are fewer.

Achieving the type of coverage he wanted required about 6000 grafts. The majority of these follicles were extracted from the beard areas. Errors included: (1) linear scar (2) disappointing growth (3) pluggy strands Ideally he wanted to achieve abundant global coverage as well as a youthful looking hairline.   21. Thank you David for fostering an atmosphere of fairness in the community. Our office made an attempt to determine whether or not the user known as Gram was indeed treated by our clinic. However we were not able to find any support to verify his status as our patient. As a clinic, we take our patient follow-up very seriously and are always looking for their input on the work they had done. But we need to establish the authenticity of someone claiming to be our patient.
  22. Dr. U’s patient, also known as “Sofarsogood” (on the hair transplant forums) is the world’s first recorded case where leg hair was transplanted as grafts for hairline restoration. He had undergone strip surgery at another clinic. Due to the thickness of hair that was used, the edge of his hairline developed a very pluggy looking appearance. He attempted to keep this hidden by growing the front layers of his hair long and keeping it combed forward. Ultimately he decided to undergo a hair transplant repair surgery and chose Dr. U as his surgeon. Using his signature UGraft technology, Dr. U extracted 1025 grafts of leg hair. These were inserted into a 0.5-1.0 cm margin around the patient’s hairline. Why Leg Hair? Leg hair is much thinner compared to regular head hair. Patient After 9 Years “Sofarsogood” underwent his hairline restoration back in 2006. His procedure was even featured in JAMA Dermatology in an article published by Dr. U called, “The Transplanted Hairline: Leg Hair For Improvement.” At the four year point, the patient’s hairline continued to thrive. This year of 2015 is the 9th year following the patient’s surgery. He has recently contacted us and sent these photos showing how his hairline has held up in all these years.   23. This post represents a two year update (since his last procedure) for a patient of Dr. U who received 6300 grafts of beard, head and nape hair. He continues to show abundant coverage as well as improvement in the repair results of his strip scar which gave him the confidence to wear his hair even shorter. Also he proudly wears a soft facial stubble since there are no obvious signs of surgery despite the 4300 grafts that were extracted from this region. His initial intent was to correct the mistakes made by a previous strip transplant procedure performed at another clinic. However, this operation resulted in poor growth along with a linear scar. His objective of restoring full global coverage was not feasible through conventional methods which can only use grafts taken from the scalp. Since Dr. U developed UGraft as a reliable means for acquiring additional donor follicles from body and beard regions, the patient decided to choose this type of surgery. A Renewed Hairline, Temples Crown and No More Strip Scar Worries With the 6300 grafts, Dr. U replenish the patient’s coverage and created a youthful looking hairline along with matching temples to best frame his face. Beard hair follicles were inserted into the strip scar tissue to obscure the empty line on the patient’s scalp. He had previously tried to keep this hidden by wearing his hair in long shaggy lengths. Amazing Healing of the Beard Area The donor areas of the patient’s face and healed in a most excellent and pristine manner despite experience the extraction of 4500 grafts. In his update video below, Dr. U asked him what his thoughts were upon seeing his chin in the mirror. He answered, “It looks normal. There is no scar.” Also Dr. U selected the beard hair grafts in a such a way to avoid creating obvious patches of missing hair.   24. @Heliboy- Thank you for the detailed sharing your experience. @BUSA and @Hairweare: On fees: The cost of surgery is not discussed here as these are private matters between patient and Dr. So your assumptions are speculative at best. BTW Dr U has been doing BHT for about 10 years now with consistent results. Back then several of the clinics you are praising lobbed stones at him for implying that BHT works! and now? The typical candidates have been butchered prior to seeing Dr U with no recourse. Some have been laughed out of several clinics. Dr U has done several pro bono and heavily discounted surgeries in several instances out of empathy. There are always cheaper clinics for every work/service, but some have established their reputation over the years and patients expect it would cost more to go to top notch clinic. Dr U is often the answer were the margin of error is zero. Many of his patients were in fact referred by some of the recommended Drs (or their patient reps) on several of these hair forums. Some of these patients had returned to Dr U after succumbing to the lure of cheaper surgery (elsewhere) with dismal results. Many of the patients in our videos have had failed BHT attempts elsewhere. @BUSA Please refer to my earlier post from 4.10.15 where I discuss the hair texture of BHT surgery patients. With the right styling techniques, more natural looking textures can be achieved. I even posted several examples of our patients who illustrate this. This particular patient featured in this post has been advised by our clinic on the use of products like gel. But he has chosen to wear his hair as it is featured in his video. We are not here to judge or criticize the grooming preferences of our patients. It is after all, their personal choice.   25. @Heliboy- Thank you for the detailed sharing your experience. @BUSA and @Hairweare: On fees: The cost of surgery is not discussed here as these are private matters between patient and Dr. So your assumptions are speculative at best. BTW Dr U has been doing BHT for about 10 years now with consistent results. Back then several of the clinics you are praising lobbed stones at him for implying that BHT works! and now? The typical candidates have been butchered prior to seeing Dr U with no recourse. Some have been laughed out of several clinics. Dr U has done several pro bono and heavily discounted surgeries in several instances out of empathy. There are always cheaper clinics for every work/service, but some have established their reputation over the years and patients expect it would cost more to go to top notch clinic. Dr U is often the answer were the margin of error is zero. Many of his patients were in fact referred by some of the recommended Drs (or their patient reps) on several of these hair forums. Some of these patients had returned to Dr U after succumbing to the lure of cheaper surgery (elsewhere) with dismal results. Many of the patients in our videos have had failed BHT attempts elsewhere. @BUSA Please refer to my earlier post from 4.10.15 where I discuss the hair texture of BHT surgery patients. With the right styling techniques, more natural looking textures can be achieved. I even posted several examples of our patients who illustrate this. This particular patient featured in this post has been advised by our clinic on the use of products like gel. But he has chosen to wear his hair as it is featured in his video. We are not here to judge or criticize the grooming preferences of our patients. It is after all, their personal choice.
