  1. Dr Joseph F. Greco04-08-2009, 08:13 PM Can the infusion of platelet rich plasma (PRP) help to reverse or slow the process of Androgenic Alopecia? I just posted the study we did on my blog so you can read the results of the study. It demonstrated that the treatment group increased hair diameter when compared to controls at 4 months and 8 months. All we can do is slow it down. I will be posting a video of the patient demonstrated in the study soon and he is now at 13 months. We plan to do another session. Does this therapy actually reverse hair follicle miniaturization and ???wake up??? dormant follicular stem cells? It does in Alopecia Areata where this occurs. This is also demonstrated in the study. Hello Dr. Greco I think we all have many questions for you. Does this treatment work as well or better then Propecia? We are not saying that this therapy should replace any FDA approved therapy and it was never intended to replace any therapy. We have been utilizing PRP cellular therapy the past two years in surgery and noticed increased density sooner which prompted the study on non transplanted hair. That being said, it is an adjunctive option for hair surgeons to utilize in surgery, especially in the vertex or crown of young patients both male and females who are thinning. Non-surgically, we recommend it 1- in conjunction with FDA therapies, 2-in those patients that may have noticed a reversal after being on approved therapy or 3- for those patients who had side effects to Propecia or Rogaine. Do you have to be on any medication for this to work? No When will it it be available? It is available now in surgical procedures and I know Dr Cooley is using it. I have a number of hair surgeons scheduled to visit and we will share our experience with them. Any hair surgeon who understands PRP processing is able to use it. What is very important to understand is that ???not all PRP is the same???. It can be processed differently with higher concentrations of platelets and growth factors. If the PRP is not at a therapeutic level it will not work as well, so someone not skilled in processing may use it and not get the same results. Additionally, if it is used in a non surgical application and used on someone who has more extensive hair miniaturization or someone who is older it will not work as well, similar to Propecia. We are only beginning to understand how it works in hair applications and caution must be taken and realistic expectations must be given to patients. The most important thing about this therapy is that it is safe because it is your own cells, there is no downtime and it can be used in combination with other therapy. How many people have you treated and what were the results. Over 300 the past two years. My partner Bob Brandt is the real guru in PRP. In the past 12 years his company has done over 28,000 PRP procedures in almost every application other than hair. It is with this background that we are studying the effects of growth factors in hair restoration. Does it work on everyone who still has some hair? No. Unfortunately Paul, there is no silver bullet yet. --

  4. I take .5 two days in a row and one day off. I repeat this cycle with good results. Blood testing is also a good idea regardless if you are on Propecia or not. Read Sommers book ageless. The key is in the Hormones. Bioidentical of course.

  11. Clinical Summary Saw palmetto is a small, slow-growing palm native to southeastern United States. The fruits are a rich source of fatty acids and phytosterols and have been used to promote urination, reduce inflammation, and for treatment of prostatic conditions such as benign prostatic hyperplasia (BPH). Saw palmetto has been studied in many controlled clinical trials. Results indicate that it improves urinary flow rate and reduces nocturia in patients with benign prostatic hyperplasia (2) (3) (4) (5). However, data from a randomized study showed no benefit of saw palmetto over placebo for improving BPH-associated symptoms (6). Studies with a liposterolic extract of saw palmetto berries indicate that it reduced the uptake by tissue specimens of both testosterone and dihydrotestosterone (DHT) by more than 40% suggesting antiandrogenic activity (7). Further, the extract inhibited binding of DHT to its receptor (8) and blocked the conversion of testosterone to DHT by inhibiting the activity of 5-alpha-reductase (9). The berries also demonstrate anti-inflammatory and antiedematous properties by inhibiting cyclooxygenase and 5-lipoxygenase pathways, thereby preventing the biosynthesis of inflammation-producing prostaglandins and leukotrienes (10). A laboratory evaluation of six commercial brands of saw palmetto revealed the actual fatty acid content to vary considerably from the labeled dosage, from 3% to +240%. In addition, three of the brands tested were found to contain less than 20% of the stated amount. Patients should use caution before using over-the-counter saw palmetto products.

Remarkable Effects of Beta-Sitosterol In a randomized, double-blind, placebo-controlled, multicenter study of 200 men with benign prostate enlargement, half the group received 180 mg of beta-sitosterol daily, while the other half received placebo. After six months, the beta-sitosterol group saw improvement in the International Prostate Symptom Score, the measurement of urine flow (Qmax), and the amount of residual urine remaining in the bladder (PVR).1 The beta-sitosterol group showed a 7.4-point decrease in the International Prostate Symptom Score, compared to a decrease of only 2.1 points in the placebo group. This was a significant 3.5-fold improvement in the men taking beta-sitosterol (Figure 1).1 The measurement of urinary flow increased to an average of 15.2 milliliters (ml) per second from 9.9 ml/second in the men receiving beta- sitosterol. The placebo group only increased to 11.4 ml/second from 10.2 ml/second at baseline. Urinary flow thus improved almost 35% in the group taking beta-sitosterol, compared to only 11% in the placebo group.1 Most remarkably, residual urine in the bladder decreased to 30.4 ml from 65.8 ml in the men using beta-sitosterol ... a reduction of almost 54%! In the placebo group, residual bladder urine declined from 64.8 ml to 54.3 ml ... a reduction of only around 16%.1 In a follow-up study that evaluated durability of response to beta-sitosterol, the beneficial effects for beta-sitosterol were found to be maintained during an additional 18 months of observation.2 Figure 1 shows the significant difference in the International Prostate Symptom Score in men receiving beta-sitosterol compared to placebo. Benefits of Beta-Sitosterol Confirmed To confirm these remarkable effects of beta-sitosterol, another study was performed and the results were published in the British Journal of Urology. The study involved 177 patients with benign prostate enlargement. Patients received 130 mg of beta-sitosterol each day and were monitored for more than six months. Measurements of the International Prostate Symptom Score, urinary flow, and residual urine in the bladder after voiding were recorded.3 On average, urinary flow values increased by 4.5 ml/second while residual urine volumes decreased by a substantial 33.5 ml. The International Prostate Symptom Scores showed a statistically significant improvement. These results with beta-sitosterol are comparable to those seen with the commonly prescribed drug Proscar?®, used to treat benign prostate enlargement.3
  16. Do not tell anyone and here is why. I go to my friends house who I haven't seen in a while and he says "you need a hair cut...LOL. After dinner my friends wife says her brother called and said he was getting a transplant. So word spreads to other people. I held my tongue and didn't say anything, my friend says he has a little spot thats thinning and his wife has some thinning as well. People talk, so if you want it to be a secret to most, tell none.

  20. I a gree with Dr. Feller. If you do not have enough donor hair, then cut your hair short and live with it. Having your body hair on top looks worse. Sometimes the truth is not what someone wants to hear, but must be said in a respectful way. Thanks Dr. Feller for speaking out.

  22. I think Dr Feller is correct, but since "Hair Transplantation" can not be a cure for everyone due to donar supply, it shouldn't be used in a blanket statement. It needs a asterisk like the steriod users, Bonds...etc......
  23. I would wait months before I would start the Foam. Let nature take its course and build a srong foundation. If you speed things up too early, I feel you weaken the base.
