Hairline repair with Dr. Bisanga (BHR Clinic)

Hello I want to share my repair procedure done by Dr. Bisanga in Brussels Waterloo.

Before the actual writeup I want to clarify some things.

Some of you will probably quickly notice that I’ve also shared my previous hair transplant on this forum under different name. I hope @Melvin- Moderatoris not going to kill me for creating second account for this thread 😆 but I don't want to associate this two procedures and I feel like the new user name is much more appropriate.

I was debating whether to do this writeup at all because I don’t want to focus to much on this period of my life as it has cost me a lot mentally already but I feel like there are some people in similar situation and it may give them some hope.

This is separate procedure and I don't want to talk the previous doctor in any shape or form for both legal and mental reasons. All I can say is that the doctor who did the first procedure doesn't agree with the opinion of other surgeons that I have consulted and that's okay. Everybody has a right to their opinion. Personally I obviously align more with the opinion of other doctors that I've consulted and considered. If you somehow happen to know the doctors name please don't mention it in this thread and also please don’t ask questions like “was it dr. X”. As a rule of thumb please don't mention any doctor not mentioned by me in this post.

So this time I’ve consulted many surgeons before making the final decision. There wasn’t a single doctor who told me that another procedure is unnecessary, there have been some differences how to repair my situation but all suggestions were similar when it comes to the general idea.

My 3 main surgeons that I’ve consider and shortlisted were Dr. Feriduni, Dr. Przybylski and obviously Dr. Bisanga. All three presented me a similar idea for the repair procedure. A session of graft removal then another surgery to recreate the natural hairline. I feel like all of them are good and compassionate doctors so the final decision was made based on the doctor's experience in repair procedures and the schedule. Unfortunately Dr. Feriduni has about 1 year waiting time and Dr. Bisanga was able to book me much faster for short removal session. Another reason for choosing Dr. Bisanga over Dr. Feriduni is availability of Polish patient advisor Jan (not to confuse with Ian who is also patient advisor for BHR 😊) who is always very fast to respond. He answered all of my questions and helped me to book the in person consultation and then the procedure. Also i had all of the important informations, documents and procedures tansalted and given in polish which is always helpful. Obviously the team doesn't speak polish so a bit of English is still necessary.

 

So why need of repair? I felt that my new hairline looks bad even worse than before the first procedure. To me it looked unnatural and sparse.

Dr. Bisanga during the consultation gave me the following reasons for that

  •           Hair direction of transplanted hair is in conflict with my preexisting hair
  •           The density is low
  •           The grafts for the hairline were poorly selected as thick grafts from central donor were used instead of softer grafts from the “ear area”
  •       The angles are also not good
  •       The temporal recession angles look almost like "antennas" sticking out
  •       The hairline was lowered too much. Especially considering my age. The overall shape of the hairline is also not good

 

I’ve agreed with all of those points.

The plan? Remove everything. Let it heal and recreate natural higher and age appropriate male hairline with micro irregularities. Reuse the harvested grafts behind the hairline to reinforce the area.

The day started with signing all the consents documents. dr. Kostis and dr. Bisanga had a short consultation with me. Then dr. K took some pictures and prepared me for the surgery. Then he gave me antibiotics and left me with dr. B for the procedure. Here I want to point out that opposite to my first surgery there was no need for any sedatives or tranquilizers drugs. Also I was informed what every is drug before taking it.

Being given strong tranquilizers on my first surgery made me feel uneasy. Especially given that  HT is a procedure that can be performed under local anesthesia and I was expecting that.

Then there were anesthesia shots and dr. B started punching out the grafts. It took about 1 hour to remove almost all of the grafts (as you can notice there are few grafts left here and there probably to minimize the scarring risk). The doctor told me that he took out 300 something grafts. So the difference between “expected” and “done” 2000 by the first clinic and 300 extracted was quite huge as  you can tell. Then dr. B did the incisions – I’ve counted about 300 though I’ve maybe skipped a few as I’ve done it to only keep my mind busy 😊. During next 2 hours 2 ladies implanted all of the grafts. Then I had eaten a good lunch and was explained post operative instructions and left for the hotel room. The whole procedure took 4 hours with the surgical part taking about 3 hours.

Before the procedure i was feeling very nervous but in retrospect very unnecessary as team did everything to make me feel comfortable.

 

Overall my experience is very positive. Now I hope that the extraction area is going to heal well and we can do a 2nd procedure of about 1000-1400 grafts to recreate the hairline.

Day 0

The left side, reimplanted grafts marked with red - keep in mind it is not alot about 300 grafts total. On the removal area a healing cream was applied by the team. That’s why it looks white.

Day 1 after Surgery and after the wash in the clinic. I have to apply a healing cream 2 times a day so you can see some white “left overs”. On the pictures.

It seems to me that all is healing well but we will see soon. I'm going to post an update after 2 or 3 weeks.

2,5 months after the procedure. Finally i can style my hair again. I think left side looks now more or less the same as it was before the first procedure. The right side unfortunately took a hit with shock loose from the first procedure. All in all i think it looks much better than after the first procedure. Now I'm waiting for the procedure to restore the hairline. You can see that there are some grafts (especially on the right side) still growing. I trimmed them for now but those grafts will be removed as well.

Great progress and no visible extraction scarring even though you're only a couple of months in. Just goes to show what can be achieved when the hairline extractions are doing carefully, bit by bit.

When will you be having your second surgery?

Thanks! Yes everything is going well. I had my surgery 2 months ago and i'm going to do a writeup soon. Just haven't had much time to do it. I have better density 2 months in than i had after 12 months before 😉
