Hi folks,

I was wondering whether to add on to my previous thread about my first HT with Dr Bisanga but decided to start a new thread instead.

You can can find details of my first procedure here

I had my 2nd on 10th January 2023.

This time I travelled to Brussels.

The goal of this Ht was to fill in the crown and add density to the midscalp.

Some things to note on this surgery, Dr Bisanga had estimated 1700 to 2100 grafts to do the crown. This consultation took place in Oct 2021 and I felt I'd lost a bit of ground since then.I asked the Dr to cover the crown as planned and then fill up as much of the midscalp as possible,I asked him to take as many grafts as he could,he ended up getting  2500.The grafts in the midscalp would be going in amongst native hair and transplanted hair.

My donor was weak before my first ht with 70cm2 pu fu at the back and 55cm2 per fu as the sides, hair calibre was thin.Baring all this in mind he did well to get 5500 grafts over two surgeries.

As you can see in the pics he went high with the extractions on the back, maybe outside what the conventional "safe zone " might be.He made this call based on my balding pattern over the last few years and my age.Such is the benefits of using the same skilled surgeon over a few years who is familiar with my head!

All in all couldn't be happier so far,any questions fire ahead!

so your last surgery with Bisanga and this current one were only a few months apart? (apologies if you explained this already - I'm at work and don't have time at the moment to read all your previous posts).

What was the reason? Too many grafts to do in one go? Better graft survival having them spaced out?

oh gotcha, nvm then. In this thread ,the previoud thread preview link says "Because they are back to back surgeries" so I just guessed that was the case .Nvm then lol.

No worries, that was another poster that said that.

Anyway, pretty much exactly 3 years apart!

Did Bisanga discuss using beard?

Yes he did but I didn't want to use beard (even though I've a great beard donor area ironically enough!)

I think just he was fairly confident of getting enough grafts from the scalp or he might have pressed the beard issue a bit further.Im pretty sure im all maxed out now and if I did ever want to go for another procedure it would be all beard

it looks like you may have lost some ground in the front/mid scalp as well since your first surgery, no?

either way, this work looks great. excited to follow

Yes i think I did.

I started finesteride in around 2010 but stopped around 2017,I had my first ht in 2020 and I guess some loss continued as I still wasn't taking finesteride. I started back on the finesteride the day of my 2nd ht 10th Jan 2023 (so a total of 6 years of a break from taking it)

Hopefully this will stabilise things.I had felt that due to my age and balding pattern than it had stabilised without medication but I'm afraid there probably was a bit of loss.

Also that's the reason I asked the doctor to put some grafts in the mid scalp this time around again.

Congratulations. I see the thread of your first surgery. Your hair looked really full in the front.  Cab you share a photo of your front the clinic took before shaving your head. The photos you shared from the back doesn't really show much from your first transplant. Best of luck

I think the way the hair is brushed forward in the pics at the top of the thread is pretty unflattering. I guess when they are taking before pics at clinics they want to show the hair at its worst.

Here are some of the front taken at clinics requested before 2nd ht earlier this month Screenshot_20230122-202614_WhatsApp.jpg.3d0b5835c03b7a0ecd43bc963bb44f93.jpg

That makes sense. They did the same to me. I went from I have a solid head of hair combed properly to wow that's horrible hairloss when pulled back In a certain correction with the headcomb

Had surgery 2 weeks ago today.

Seems to be going well so far.

Redness is almost all gone which is crazy at two weeks. The shed hasn't started yet and I hope it mightnt start at all! No shock loss so far which was a big worry of mine as after my first ht I had bad shockloss in the donor that lasted 8 months.

I think I can see a nice sworl pattern which wasn't as obvious in the days straight after surgery.

Pics taken with flash

The difference between 2 weeks and 3 weeks....

Shed starting now at the crown, the sooner it starts the sooner it grows back so happy enough with that.

Redness went after less than 2 weeks,no donor shockloss so far which is great as after my first HT I had serious shock loss in donor that lasted 8 months.

Dr Bisanga took note of this and changed up the anesthetic to tailor it a bit more specifically to my head,so far so good!

No issue going out and about without a hat, just looks like a tight hair cut.

Date of 2nd HT was 1 month ago from today.

A few pics and update.

The crown area has completely shed now, thankfully the midscalp where there was hair already hasn't shed  much at all,I really hope that continues.

The front and midscalp may look a bit thin but the reason is i think its mostly transplanted hairs from first ht there now,it's not going to look full at this length.

Redness as mentioned in previous posts was gone in less than 2 weeks and donor looked back to normal aswell.

I have been out a few times without a hat and feel comfortable with it.Thats great in my opinion as I was wearing a hat for the first 3-4 months after ht number 1.

I'm planning to have the sides shaved down to a 2 blade in the next week or so.

6 weeks ago yesterday so i thought I'd do a little update.

Native and some (certainly 1st ht hair and possibly 2nd ht transplanted hair) growing back nicely.Havent worn a hat in a few weeks.Had the sides tidied up a week ago

Donor seems back to normal aswell. This is big for me as I had very bad shock loss first time around.

I can't get over how easy it is the 2nd time around,of course there are many reasons for this.Im happy enough to look like this until the crown starts to grown in,I can feel lots of little spikey hairs there already.

Pics are taken right beside window in bright light.

8 weeks...

Not much happening as is to be expected, maybe some spickey hairs starting to sprout on the crown, possibly a bit of shockloss in the mid scalp.

Very happy with donor recovery, a really excellent job by Dr Bisanga on that.Bare in mind this is a weak donor area that had severe shockloss after first ht.Now it has had 5500 grafts taken out of it over 2 surgeries and looks pretty OK to me!

Difficult to recreate exact angles of pics (especially taking them yourself on an average camera!) But this gives an idea,I'm pretty happy for 8 weeks.Had a haircut today and barber couldn't believe the difference since last cut.I expected the crown to take a lot longer than my 1st ht to grow in but it has definitely started now.
