Dr. Arocha, 8 months post-TP, 4464 Grafts in NW 6

28 year old male with persistent anterior fringe progressing to NW 6 wanting to start hair restoration to re-establish the framing of his face. He also desired to restore the rest of the top of his head. The post-transplant results showing robust growth at only 8 months. The crown is filling in more slowly. He now has a very natural and undetectable result which maximizes his appearance, so that he looks his stated age.

Very nice result, especially on the hairline and facial "framing." I'm curious, is the patient on any medications? He looks like he suffered from borderline diffuse unpatterned loss before the surgery?

Future_HT_Doc: Thanks for your feedback. To answer your question, this patient is on Finasteride 1 mg daily and Rogaine twice a day.

 

The patient did have patterned diffuse thinning becoming a NW 6. Dr. Arocha worked in the crown area and it is growing in nicely. I'm looking forward to seeing his results in a few months.

Correct me if I'm wrong, but aren't Diffused Pattern Baldness Candidates, BAD candidates?

 

Maybe it's based on what you've said Christine, "becoming a NW6"

 

I guess as long as he isn't losing his hair in his donor area, it'll be good

 

ps: I am simply asking for curiosity and my own education, I am 110% sure Dr. Arocha knows what he's doing

it is difficult to ascertain how much growth this patient. His preop pics are of his hair short. He has now grown out his hair and styled it to cover the areas of weakness. To determine whether he achieved significant growth would be to present the same type of haircut post op

If you are thinking of DUPA, diffuse unpatterned alopecia, then you are correct they are not candidates, because they will miniaturized their donor hairs. However, this patient has patterned hair loss, going from a persistent anterior fringe to a NW 6, hence he was an excellent candidade.

Dr. What I'm saying is that the patient sported a shorter hairstyle before the surgery and its difficult to appreciate how good this result is because patient grew out his hair and combed it from left to right. The front shot looks good and his sides are strong but the birds eye view reveals weakness. It seems that with this amount a graft work the coverage could have been better.

anouar, This patient is only at 8 months. he still has growth left. also, i guarantee you that if he tried his new hairstyle pre-op there's no way he could pull it off as it would look too thin.

hairthere, your guarantee is purely subjective. if you take that combed hair from left to right bring the part back to lleft side you will expose major weakness in the center. I guarantee that ;-). This is where the majority of his hairloss is focused. He doesn't have a typical your MPH. Im not saying he didn't get anygrowth. I just question for the amount of grafts placed that he should have more density in the vertex. It would be nice to actually see the graft placement

It is common for balding men to trim their hair short, to decrease the density disparity between their thick fringes and the thinning area. This patient had no part in the before photos, simply there is little to part. The hair in the frontal and mid-scalp was miniaturized. The transplant plan drawing clearly shows the reconstruction of the hairline, frontal and mid-scalp areas, these were the main areas restored. Some transplantation was done to the left side crown. The placement photos have been in the gallery. While this is an excellent growth for only 8 months, there should be much further improvements in the coming months. For the size area restored this is not too large a session!
