  1. This 34 year old gentleman had a hair loss pattern similar to a class 4v on the Hamilton Norwood hair loss scale. His goal was simple; rebuild his hairline and add density to the frontal 1/2. That said, he also wanted to be able to keep his hair short without anyone detecting that he had a procedure done. I explained to him that he would have a dramatic result and his overall goals could be accomplished after one procedure. I also explained that if he wanted more density in the back ? he would need another procedure. After establishing his medical history and understanding the progression of his hair loss, I diagnosed his condition as Androgenetic Alopecia (male pattern baldness), ruling out any other possible dermatological or medical conditions which can cause hair loss. During the discussion with the patient regarding his surgery I conveyed to him that my intent was to reconstruct a hairline that would be suitable for him now, but also something that will look good as he gets older. This is a critical issue which needs to be discussed when planning any work on the hairline. A total of 3,981 grafts were used. The break down was as follows: 1,307 single hair grafts, 2,652 two hair grafts and 22 three hair grafts. I used significantly more single hair follicular units which allowed me to create a very natural hairline. A natural hairline is essential in making a patient happy; it gives them confidence. When a patient has undergone a surgery they need to feel comfortable combing their hair/styling their hair in front of others as well as undertaking water sports or swimming around other people. A natural hairline cements this feeling of comfort and allows patients to experience a more carefree lifestyle. These are the patient’s results 10 months post-op. There will continue to be more improvement over the next few months. As I’m sure you’ve gathered, the patient is extremely happy with his results.

I explained to him that his goals could be met with one procedure. When planning the surgery I discussed with the patient the need to reconstruct a hairline that would be suitable for him now, but also considered how he would look as he gets older. A total of 2,527 grafts were used. The break down was as follows: 1,327 single hair grafts, 1,008 two hair grafts and 192 three hair grafts. As I was rebuilding a hairline I used significantly more single hair follicular units to ensure that the final result looked natural. These are his results 10 months after his procedure and there will continue to be more improvement over the next few months. The patient is extremely happy with his results.

His goal was to build up his hairline and add density to the frontal ? of the scalp. I explained to him that his expectations could not be met after one procedure. Rather, he will have a dramatic change after one procedure, but he still might want a 2nd procedure to add more density. After establishing his medical history and understanding the progression of his hair loss, I diagnosed his condition as Androgenetic Alopecia (male pattern baldness), ruling out any other possible dermatological or medical conditions which can cause hair loss. The patient’s goal was well suited to his needs and therefore I planned the surgery accordingly. I wanted to reconstruct a hairline that would be suitable for him now, but also something that will look good as he gets older. A total of 3,175 grafts were used. The break down was as follows: 1,209 single hair grafts and 1,966 two hair grafts. I used a significant amount of single hair follicular units which allowed me to create a very natural hairline. By doing this it will allow him to comb his hair without anyone detecting that he has had anything done. These are his results 9 months after his first procedure and there will continue to be more improvement over the next few months. The patient is extremely happy with his results.

The patient’s goal was to get as much coverage as possible while maintaining a natural hairline. I explained to him that he was going to need two sessions to reach his goal. However, after one procedure he would have a dramatic change; a natural hairline and good density into the frontal ? of his scalp. When reconstructing the hairline I kept in mind what would be suitable for him now as well as something that will look good as he gets older; a key part of making the hairline look natural. A total of 4,428 grafts were used. The break down was as follows: 1,398 single hair grafts and 3,030 two hair grafts. I used significantly more single hair follicular units which also allowed me to create a very natural hairline; an action that will allow him to comb his hair back without anyone detecting that he has had anything done. These are the results of this FIRST procedure 9 months post-op. There will continue to be more improvement over the next few months. The patient is extremely happy with the results of his initial procedure and is planning on having a second procedure completed sometime next year.

His goal was to be able to comb his hair back without any signs of thinning in the hairline. After establishing his medical history and understanding the progression of his hair loss (which is important, as you’ll see below), I diagnosed his condition as Androgenetic Alopecia (male pattern baldness), ruling out any other possible dermatological or medical conditions which can cause hair loss. The patient’s goal was well suited to his needs. That said, I understood his progression of hair loss and it allowed me to better anticipate where he would thin as he aged. In educating and discussing this with the patient, we were able to agree on an approach to the surgery that would reconstruct a hairline that would be suitable now, allowed the patient to attain his goals, and would also look good as the patient aged as it took into account where the patient would likely thin in the future. A total of 3,241 grafts were used. The break down was as follows: 1,620 single hair grafts, 1,122 two-hair grafts and 499 three-hair grafts. I used significantly more single hair follicular units which allowed me to create a very natural hairline. This action will allow him to style his hair as he would like without anyone detecting that he has had anything done. These are his results 10 months post-op and there will continue to be more improvement over the next few months. The patient is extremely happy with his results.

He wanted to be able to comb his hair back but his thinning hair kept him from being comfortable doing so. As with every patient, I had to determine his medical history and understand the progression of his hair loss. I diagnosed his condition as Androgenetic Alopecia (male pattern baldness), ruling out any other possible dermatological or medical conditions which can cause hair loss. It is always important to understand the rate of a patient’s hair loss and also the pattern that their loss has followed as it allows me to educate the patient on the options available to them during surgery. In speaking with this patient about the levels of density that were achievable during surgery, I determined that he preferred a more natural result that would create more density and rebuild his hairline but was conservative enough to keep others from detecting that he had a procedure done. As with every surgery I perform, I planned the surgery according to the priorities of the patient; what would make the patient happy. I used a total of 2,375 grafts during this procedure. The break down was as follows: 1,321 single hair grafts and 1,054 two hair grafts. You’ll note that I didn’t use any three hair grafts. Although the patient wanted more density, it was evident after educating him about all of his options that a good result for him would be a result that was extremely comfortable. To achieve this goal I had to give him a natural result that had enough density to ease his concerns regarding his thinning in the frontal region but didn’t create a new concern by focusing so much on density that I compromised the natural presentation that would make the patient comfortable. Concentrating on the patient’s goal I only used one and two hair grafts. I also used significantly more single hair follicular units as this allowed me to give the patient the fuller density that he desired but also created a very natural hairline that will keep anyone from noticing that he has had a hair transplant procedure. These are his results 10 months post-op and he will continue to see more improvement over the next several months. The patient is very happy with his results.

After establishing his medical history and understanding the progression of his hair loss, I diagnosed his condition as Androgenetic Alopecia (male pattern baldness), ruling out any other possible dermatological or medical conditions which can cause hair loss. The patient’s goal was well suited to his needs and therefore I planned the surgery accordingly. A total of 2,952 grafts were used. The break down was as follows: 1,098 single hair grafts, 1,502 two hair grafts, and 352 three hair grafts. I used significantly more single hair follicular units which allowed me to create a very natural look. More grafts were packed into the crown and less packed farther back into the mid-zone. These are his results 8 months post-op and he will continue to see more improvement over the next few months. As I suggested, he is now taking Propecia to control further hair loss. As you may have anticipated, the patient is extremely happy with his results.

  13. Hi, hairlineGuy! This is an area where a small amount of dry blood is being held in place by sutures and will come off after suture removal without leaving any trace. Please apply some antibacterial ointment overnight to soften the scab, do not try to pick it off, let water run gently over the scab when you take a shower - it will be completely gone and will not leave a trace in a couple of weeks. It may be gone by the time we take sutures out.
  14. Hi, efflux! Swelling sometimes happens and is usually resolved by end of the week after the procedure. We recommend to keep your head above your heart when you sleep, finish taking Medrol doze pack, warm compresses ( warm teabags work well ). Swelling is somewhat dependent on how your body mobilizes fluid and can be more or less pronounced but it is transient and will be gone and forgotten by the time we take sutures out.

  18. This is a patient that we previously posted to the Network but he recently came in and we decided to video tape his results 3 years after his procedures. To view Stuart's previous post click here. The video you are about to see is of a patient who had his first procedure completed by us three years ago, at the age of 30. When the patient was 23 he had undergone a procedure, performed by another doctor, which left him with an undesirable hairline. I was confident that we could correct the patient's hairline and give him significant density. Two procedures were performed and these are his video results three years later. Note: some still photos are presented to show the patient's progress over the three year period. The video portion was taken three years after his procedures.
  19. This 52 year old gentleman had 3 procedures done back in the late 90’s by another Physician in Chicago. During his previous procedures they had used larger grafts, which left him with a “pluggy” look as his pre-existing hair thinned over the last 12 years. He was not comfortable exposing his hairline and therefore he was somewhat restricted with his hair styles. After establishing his medical history and understanding the progression of his hair loss I was confident that I could repair his hairline. I dense packed 2,910 follicular units into the frontal 1/3 of his scalp. There were 1,130 single grafts placed just into his hairline followed by 1,261 two hair follicular units. 519 three hair follicular units were placed just behind the hairline. These are his results 10 months post-op.

After understanding the patient’s concerns, I discussed several options with him on how we could reconstruct his hairline and fill in his crown. Since the patient was 45, and he had responded well to Propecia over the last few years, we agreed on a slightly more aggressive approach with his hairline. We placed 3,616 grafts, which were a combination of 1, 2, and 3 hair follicular units. These are his results 11 months after surgery. He is extremely happy with his results.

He had been wearing bangs for the last few years to hide his receding hairline. After establishing his medical history and understanding the progression of his hair loss, I diagnosed his condition as Androgenetic Alopecia (male pattern baldness), ruling out any other possible dermatological or medical conditions which can cause hair loss. After understanding the patient’s concerns, I discussed several options with him on how we could reconstruct his hairline. We decided to take a more conservative approach and leave some recession in the temples, which would give him a good look after the surgery, but also as he continues to age. We placed 2,937 grafts, which were a combination of 1, 2, and 3 hair follicular units. These are his results 8 months after surgery. He will see more improvement over the next 6 months.
  23. This patient is a 32 year old gentleman that came into our office concerned about his thinning hair. After establishing his medical history and understanding the progression of his hair loss, I diagnosed his condition as Androgenetic Alopecia (male pattern baldness), ruling out any other possible dermatological or medical conditions which can cause hair loss. He had been prescribed Propecia two years prior to our meeting and was still taking the medication. The patient felt that his hair loss has been under control since starting the Propecia, but the medication had not reversed the damage that occurred prior to his taking it. Therefore, he wanted to correct the hair loss that had occurred prior to taking Propecia. He was uncomfortable with the thinning of his hairline along with the thinning within the crown area. After understanding the patient’s concerns, I decided that we could address both areas of concern in one surgery. My plan was to build up the hairline and frontal 1/3 of the scalp with a majority of the grafts. The remaining grafts would be placed in the crown area to increase his density in that region. The graft breakdown for the surgery is as follows: 1,401 single hairs, which were all place in the hairline 1,674 2-hair grafts, which were placed in the frontal 1/3 530 3-hair grafts, which were placed in the crown area A total of 3,605 grafts were placed during this patient’s surgery. These are his results 10 months post-op.
  24. Bigonefive – The cleaner appearance of my immediate post-op photos is directly correlated to three factors that I work very hard to control during surgery. The size of the grafts or follicular units and the size of the recipient sites have to closely match in order to minimize the “wobble” effect generated when a graft is too small for the recipient site in which it is placed. The size differential, when too large allows for extravasation of blood. When you see this you are witnessing the aftermath of the “wobble” of the graft in the recipient site. Reducing or eliminating the mismatch between the size of a graft and the site in which it is placed requires a high degree of skill, patience and experience. If any one of these items is missing the result will be sub-prime and the patient will not get the best results possible. The second item that minimizes the appearance of blood in the immediate post-op photos, is the fact that I control bleeding during surgery with the help of tumescent techniques that I’ve perfected over the 20 years that I have been performing hair restoration. Finally, larger amounts of bleeding can be caused by using dull instruments. I change blades frequently, as sharp instruments create less tissue trauma. I am very meticulous about the instruments that we use during surgery to ensure they are sharp. Dull instruments cause additional trauma, bleeding and subsequent crusting. Excessive bleeding can lead to excessive crusting, which has been shown to delay and stunt the growth of grafts. I have found that when you take the additional time to institute several quality control measures you can reduce the immediate post-op bleeding. The measures tend to ensure graft survival and also have the effect of leaving the recipient area very clean to the great satisfaction of patient and doctor alike. Therefore, the cleaner photos are really a product of meticulous care, planning and quality control that we regularly pursue.
