PATIENT'S DETAILS ✓ 36 years old (in 2017), losing hair since the age of 25, with a hair loss history in his family (uncle, father, brother). ✓ No medications. ✓ Thin caliber hair. ✓ Normal follicular units. ✓ High % of hair in telogen phase. RECOMMENDED TREATMENT Here we present yet another interesting case of a complete hair restoration by means of FUE technique. Also in this case, the patient wished to restore all of his balding areas in one single session. However, considering the important surface that needed to be treated, I recommended him to take a two-steps approach in order to ensure the best management of his donor area. The patient has eventually undergone two FUE surgeries : the 1st session (November 2017), consisting of 3654 FUE grafts, aiming at restoring the patient’s hairline, frontal and midscalp sections; the 2nd session (March 2020), 2132 FUE grafts, with the objective of covering the vertex area. Below are shown the results of the first surgery and the immediate pre and post-operative photos of the second session. As usual, we will share the progress and outcome of this second procedure as soon as new material will be available. PATIENT'S DETAILS ✓ 37 years old, losing hair since the age of 18. ✓ Taking Minoxidil 5% (intermittently) since the age of 27. ✓ Medium caliber hair. ✓ Normal follicular units. ✓ High % of hair in telogen phase. RECOMMENDED TREATMENT This 37-year-old gentleman wished to restore the whole of his balding areas in a single session. Given the extended nature of his hair loss, which affected the anterior as well as the posterior area, I suggested him to follow a two-steps approach : a first procedure aiming at harvesting as much grafts as possible in order to restore the anterior and midscalp areas, and a second session focusing on the vertex area. In April 2018, we performed a first FUE procedure consisting of 4261 grafts, including few hundred beard grafts used to increase the midscalp density. The pictures taken at the clinic 6 and 8 months after the surgery already show a drastic change in his look. In January 2019, we carried out the second FUE session - 2065 scalp grafts, in order to cover the vertex. The improvement is apparent also in this case; a total restoration has therefore been performed, as it can be appreciated in the pictures sent afterwards by the patient. Regarding the donor area management, it is interesting to underline the extraction pattern followed during the second FUE session. As it can be seen in the relevant photos, I ended the extractions in a progressive way, getting closer to the recipient area as much as possible. I tend to adopt this approach when patients are undergoing a second (or further) FUE surgery; in fact, this method allows me to avoid the so-called "fringe", a section of scalp between the donor and recipient areas which otherwise would maintain an original density, resembling a halo all around the skull and being often very ungraceful, especially when the patient wears his hair very short.   3. Thanks for your comments. Unfortunately we currently do not have any detailed pictures of the FUT scar. We will provide them when the patient will come in for his second round, as we will have the opportunity to shave his head in view of the procedure. Concerning this patient's hair color : as you can see in the "before" photos, his donor hair is salt and pepper and the transplanted hair maintained this characteristic once regrown. It is also to be noted that the patient was dyeing his recipient hair, before the procedure.

The patient is currently considering a second procedure in order to cover his vertex, next year (most likely an FUE consisting of approximately 2500 grafts).   5. PATIENT'S DETAILS ✓ 37-year-old female, suffering from Androgenic Alopecia since the age of 27. Hair loss stabilized. Her mother and aunt suffer from the same condition. ✓ No medications (side effects from Minoxidil). ✓ Medium hair. ✓ Normal follicular units. RECOMMENDED TREATMENT This 37-year-old female patient came to visit me to discuss about a possible solution for her hairline and temples thinning, caused by Androgenic Alopecia. She had suffered from that condition since the age of 27. She had tried to use Minoxidil in order to improve the situation, but unfortunately the medication caused side effects (hair growing on undesired areas). The loss had nevertheless stabilized since few years already. During the consultation, we therefore agreed upon performing a FUT procedure complemented by a small FUE in order to cherry-pick one hundred softer hairs from the nape area with the aim of restoring the very first hairline and temples. Eventually, the patient received a total of 2136 grafts. The post-operative pictures were sent in by the patient. One year after the surgery, she is very satisfied with her new look : the situation has actually remarkably improved, the yield seems excellent and the results are very natural. RECOMMENDED TREATMENT Here is a nice example showing a frontal area and vertex restoration, done in a single session, consisting of 3545 FUE grafts. While I am usually averse to the idea of grafting the anterior and vertex areas at the same time, while leaving a section of the midscalp untouched, in this specific case I agreed (following a fairly conservative approach). I have given my consent based on several factors : the patient's family hair loss history (just his maternal uncle reached the NW IV stage), the fact that he was under medications to maintain his native hair and that his hair loss was stable since a couple of years already. Moreover, his donor area is very rich and can still provide an important amount of grafts (approximately 3000-3500, not taking into account the potential BHT resources) so that it will be possible to cover the new balding areas in the future, if his hair loss progresses.   8. Thank you all for your kind comments. The patient started taking Finasteride two months before the procedure. He stopped the treatment some months after, then resumed again. I think that the patient's donor can still provide approximately 2500-3000 grafts. One day, with this amount, we will be able to achieve an almost total coverage thanks to the grafts. The graft placement visible in the photos the day after the first surgery just confirms one of my strong opinions. The placement is crucial. This patient's hair quality - very good, wavy - certainly improves the perceived density. However, this is exactly the kind of result that my team and I always aim to obtain.
  9. The patient came to my office for a check-up consultation in March 2019, one year and two months after the surgery. At that time, the patient expressed his wish to undergo a second procedure in order to perfect his hairline and increase his frontal and midscalp density. At the end of May 2019, a FUE consisting of 2138 grafts was performed.   10. Thank you for your comment and query. So far, I have used pubic grafts mainly to restore eyebrows. Sometimes, to restore beard or sideburns. Given the weaker nature of pubic grafts, it's not always easy to extract them; however, if transection is avoided, they provide a good yield. I frequently use beard grafts to fill in the scalp. Pubic hair could help, if necessary.
  11. Indeed, I do use the hybrid punch to remove grafts, from frontal region included. Its transection rate is particularly lower compared to a sharp punch - it is a huge advantage, especially if you plan to reuse the grafts. Thanks for your query. Sean, JohnCasper, thank you for your kind words.
  12. PATIENT'S DETAILS ✓ 31 years old (in 2016, at the time of his first procedure), with a hair loss history in his family (father and brothers). ✓ Taking Finasteride and Minoxidil. RECOMMENDED TREATMENT This 31-year-old patient wished to restore his beard - which presented a lack of density on the cheeks area - and his hairline, showing a slight recession on the temple corners. In February 2016, we performed a first FUE procedure (2011 scalp and beard grafts) in order to restore his temple corners and frame the outer and lower part of the cheeks. Following the excellent results, the patient decided to further improve his beard by completely covering the cheeks area. Therefore, in December 2017, we performed a second FUE surgery (1232 beard grafts). The patient was again enthusiastic about the results obtained. In April 2019, he decided to undergo a third FUE procedure - consisting of 2889 scalp and pubis grafts, with the aim of further enhancing his hairline/temple corners, restoring the temple points and perfecting the beard density. RECOMMENDED TREATMENT Here we present a nice example of an FUE restoration for a 34-year-old patient with a quite extended hair loss, classifiable as grade V on the Norwood scale. The patient wished to cover the whole of the thinning areas in one single pass, however I suggested to focus on rebuilding the frontal and midscalp areas as a first step, in order to achieve an optimal density on these areas and ensure the best donor zone management in view of future procedures. We therefore proceeded with an FUE surgery (since he didn’t like the idea of having a linear scar), over two consecutive days, during which 3878 grafts have been extracted and implanted. The patient has recently reached the 14th post-operative month and is extremely happy about his results. He’s planning to come back at the end of the current year to complete the restoration by covering the crown area. RECOMMENDED TREATMENT This 54-year-old gentleman wished to cover all the thinning areas of his scalp with a homogeneous density, in a single session. Given his excellent scalp elasticity, I proposed him to undergo a FUT surgery first, aiming at harvesting the maximum number of grafts, with a potential complementary FUE in order to reach approximately 5000 follicular units. The FUT strip, as expected, provided an important amount of grafts (4797), so that the FUE extractions could be limited to just 300 grafts. He recently sent in his post-operative pictures, showing the results 6 months after the surgery. He’s ecstatic about the results obtained so far : the amelioration is actually already remarkable. Nevertheless, we expect the situation to further improve during the upcoming months, in particular regarding hair density. RECOMMENDED TREATMENT This 55-year-old gentleman came to my office to correct his poor, previous FUT procedures, undergone in 1996 (at the age of 33). Since then, he was forced to live with pluggy, sparse and unnatural results, as can be seen in the preoperative pictures. Since he wished to cover all the thinning areas - other than correct his hairline and anterior area as a priority - also for this patient I felt that a combo FUT + FUE was the best way to proceed in order to maximize his donor resources. In fact, thanks to the combination of the two techniques, I would have been able to correct the existing FUT scars - by including them in the new strip - and enhance the amount of available grafts via the complementary FUE. The patient has undergone the surgery in March 2018, when he received 5527 grafts in two consecutive days. The patient has recently come back for a check-up consultation and is enthusiastic about the results achieved so far (7 months and a half after the surgery). His hairline is now very natural and other areas show a remarkable increase in density. During the upcoming months we expect the results to further improve, especially regarding hair density and texture, as the hair matures. RECOMMENDED TREATMENT This 27-year-old patient presented a moderate hair loss and wanted to address his receding temples and hairline. Although his family showed no hair loss history, I suggested him to reinforce his forelock and the whole anterior zone as well, so that this area will be stable even if he will continue to lose hair in the future (he wasn't willing to take any medications to prevent further hair loss). In June 2015, an FUE consisting of 2671 grafts was performed. The patient recently came back to the clinic for a check-up and we had the opportunity to take the pictures and videos below, three years and three months after the procedure.   18. As anticipated in the previous post, the patient came back in April to undergo his second surgery with the aim of covering the vertex and reinforce the density on the adjacent areas. The procedure consisted in a Combo FUT + FUE, during two consecutive days, thanks to which we have been able to harvest a total of 3602 grafts (2550 FUT + 1052 FUE). The patient has recently reached his 6th post-operative month and has sent in a set of pictures showing his current results. He is enthusiastic about the regrowth rate and the cosmetic improvements achieved so far. On a side note, it is interesting to notice how the lighting conditions can significantly alter the perception of density (cf. the last two pictures at 6 months post-op, taken with flash, with the previous ones, taken under natural light). It consists of a very precise pedal that enables to control the movement of the punch, that I also designed myself.
