RECOMMENDED TREATMENT Here we present another nice example of an FUE restoration for a 39-year-old patient. He was an ideal candidate for a hair restoration procedure : his hair loss was stable since several years (despite he never used hair loss medications) and his hair characteristics were excellent (wavy nature, salt and pepper colour). In June 2023 this gentleman underwent his FUE procedure to restore his hairline, temple corners and frontal area. The new hairline was carefully outlined following an age-appropriate design, so that the results will be natural both in the short and long terms. A total of 2932 FUE grafts have been used during this procedure. The patient recently sent in a couple of photos showing his evolution at the 7th month mark; he is understandably thrilled about the results achieved so far and we expect the situation to further improve during the upcoming 5 months, especially regarding the hair density and texture. PATIENT'S DETAILS ✓ 46 years old in 2016 (at the time of the first surgery), NW V, losing his hair for 10 years, with a hair loss history in his family (maternal grandfather). ✓ No treatments. ✓ Thin caliber hair. ✓ Fragile follicular units. ✓ High % of hair in telogen phase. RECOMMENDED TREATMENT This 46-year-old gentleman was losing his hair for 10 years when he came to visit me for the first time, in 2016. Since his hair loss was already quite advanced (grade V on the Norwood scale) and since the patient did not intend wearing his hair very short after the procedure, we decided to perform an important FUT surgery aiming at restoring as much thinning areas as possible. In June 2016, the patient received 4512 grafts - which allowed to rebuild his hairline and cover the front-mid sections of his scalp. In February 2023, almost seven years after his first surgery, he felt that it was time to complete his hair restoration journey by covering the vertex area. Dr Montesanti and Dr Jenard performed this procedure, this time with the FUE technique, during which 2503 grafts were transferred – for a grand total of 7015 grafts in two sessions. This amount allowed to cover the vertex area and slightly reinforce the mid-section as well. RECOMMENDED TREATMENT This 35-year-old fellow physician wished to restore his hairline, which was receding since many years. During the consultation, we recommended the patient to reinforce also the frontal area, in order to anticipate a potential loss on this area - which would have obliged the patient to undergo another procedure to cover this area for aesthetic reasons. A FUE procedure of approximately 2000 grafts was recommended to reach his objective. In September 2022, the patient received 2018 FUE grafts. RECOMMENDED TREATMENT This 51-year-old gentleman came to visit us to find a solution for his receding hairline and temple corners. During the consultation, which took place 4 months before the surgery, he was advised to start taking oral Minoxidil to halt his hair loss and improve the overall hair quality. We agreed upon performing a procedure of approximately 2000 grafts and, given the fairly low number of grafts needed, the FUE technique was recommended. RECOMMENDED TREATMENT This 56-year-old gentleman had already undergone two FUT surgeries elsewhere when he came to see us in consultation. He was bothered by the loss on his vertex area. During the consultation we agreed upon performing a FUE procedure, +/- 1500-2000 grafts, in order to cover this area with a good density. Given the fairly limited amount of grafts needed, we did not need to shave the whole of his donor area but only a small strip, from where we harvested the FUE grafts – since the patient wears his hair quite long, this area was barely visible after the procedure. RECOMMENDED TREATMENT This 24-year-old patient wished to restore his temple corners, which were thinning since he was 17. Fortunately, no other areas of his scalp are affected by the loss; moreover, his family hair loss history shows limited loss as well, grade III-IV of the Norwood-Hamilton scale at the most. During our consultation we agreed upon performing a FUE procedure, consisting of approximately 2000 grafts in order to cover these areas with a high density. He therefore wished to recreate a natural-appearing and fairly dense beard shape. Because of budget reasons, we decided to split his beard restoration in two separated sittings, the second being performed one year after the first one. In this patient’s case I have used a mix of scalp and beard hair during the first sitting, while the second procedure was performed employing scalp hair exclusively (due to the patient’s weak beard donor area).   10. Thank you all for your comments, much appreciated. I think that it will be possible to harvest at least 1000-1500 FUE grafts for his third procedure. These grafts will normally be placed in the central section of the vertex to reinforce and improve the situation. The "wispy" look should improve during the next 6 months, as the transplanted hair matures and therefore becomes more similar in texture to the patient's native hair. This maturation process can actually last 18 months, sometimes even 24 months.

During this second sitting I have extracted and implanted 2486 grafts via FUE technique exclusively – which gives a total of 4724 grafts transferred so far in two separated sessions. This 52-year-old male model wished to restore his hairline and temple corners, which were thinning since his 30s. During our consultation we agreed upon performing a FUE procedure, consisting of approximately 2000 grafts, in order to rebuild his hairline and temple corners; since his frontal area was thinning as well, we decided to reinforce also his forelock : if this area was not treated at this stage, he might have needed another surgery in the future, if his hair loss had continued, because of aesthetic reasons. As mentioned in the original post, the patient was scheduled to undergo his third procedure to further increase the general density, shortly after his last check-up consultation in November 2021, During this third sitting I have extracted and implanted 3236 grafts via FUE and BHT techniques (3057 scalp grafts and 179 beard grafts) – which gives a total of 9730 grafts (8711 scalp grafts and 1019 beard grafts) transferred in three separated sessions. The general density is now much more important, and the slightly frizzy texture of the newly implanted hair will further soften over time. If we compare the photos taken before the first surgery to the ones the patient recently submitted, the improvement is quite remarkable.   14. Thanks to everyone for the comments. So far, the patient is not planning to address the crown area. His donor area can safely provide other 4000 grafts via FUE. His beard hair donor is also quite rich, with favourable characteristic (the beard hair colour matches his scalp hair colour), so that this hair could be used as well – mixed with scalp hair - during a potential procedure to cover the crown. This patient was losing his hair since a very young age and this situation affected acutely his self-esteem during the years. The fact that he is not taking any medications to halt further loss was taken into consideration and discussed with the patient before the surgery : his front-mid areas have been restored in a way that even if he loses his existing hair in the future, the result will most probably still be satisfactory or, at the worst, will need a very minor touch-up. If the patient will reach the NW VI stage, he will need approximately 3000-3500 grafts to cover the vertex with a good density : this amount can easily be reached using exclusively the remaining scalp donor hair, plus beard hair could also be used to optimise the number of available resources.

Given that he wished to continue wearing his hair short, we agreed upon performing a FUE procedure, consisting of approximately 3000 grafts, in order to restore his hairline and rebuild his anterior and midscalp sections. In August 2021, he underwent a FUE surgery during which 3054 grafts were eventually extracted and implanted. The patient has experienced a very fast growth : at the 4th month mark, the change in his look was already remarkable. RECOMMENDED TREATMENT This gentleman came to my office to find a solution for his receding hairline and temple corners. He was an excellent candidate to undergo a hair transplant procedure : he had favourable hair characteristics; his donor area was in excellent conditions; his hair loss was stable since several years (despite he stopped taking Finasteride 2 years before the surgery because of side effects) and the area to be covered was relatively small. Taking into account these factors, we agreed upon performing a FUE procedure following a quite aggressive approach. 2052 FUE grafts were eventually extracted and implanted. I have used an important number of single follicular units to restore the very first row of his hairline and temple corners, in order to give a natural but at the same time dense look. Double, triple and quadruple grafts have been gradually placed behind the hairline to achieve fullness and coverage. You can find below his evolution until the 12th post-operative month. RECOMMENDED TREATMENT This gentleman was losing his hair since several years when he came to see me for the first time in consultation. Taking into account his family hair loss history, I suggested him to start taking Finasteride in order to halt and prevent further hair loss. We subsequently agreed upon performing an FUE procedure in order to rebuild his hairline and frontal area. In March 2021, a 2007 grafts FUE procedure was performed. PATIENT'S DETAILS ✓ 52 years old, NW V (diffuse thinning), losing his hair since his 30s, with a hair loss history in his family. ✓ No medications. ✓ Medium caliber hair. ✓ Normal follicular units. ✓ High % of hair in telogen phase. RECOMMENDED TREATMENT This patient underwent 3 FUE procedures in order to cover the whole of the thinning areas of his scalp. The first procedure, which was originally scheduled in two consecutive days, was split in two separated sessions (two weeks apart) due to organisational reasons. These two sessions (2008 + 2018 FUE grafts) gave a complete coverage to the patient's scalp, following a fairly conservative approach. Six months after these sessions, the patient was already so satisfied about his results that he wished to perform a third, smaller session in order to further lower his hairline. During this third procedure, I employed single and double grafts exclusively (1254 FUE grafts). I recently had the opportunity to welcome the patient at the clinic for a follow-up consultation, during which we took the pictures I gladly share below (1 year and 9 months after his third procedure). Regarding the donor area management, it is interesting to underline the extraction pattern followed partially during the second and, especially, third FUE session. As it can be seen in the relevant photos, I ended the extractions in a progressive way, getting closer to the recipient area as much as possible. I tend to adopt this approach when patients are undergoing a second (or further) FUE surgery; in fact, this method allows me to avoid the so-called "fringe", a section of scalp between the donor and recipient areas which would otherwise maintain an original density, resembling a halo all around the skull and being often very ungraceful, especially when the patient wears his hair very short.
