Also, I'm worried about the amount of grafts needed - I get it, my balding pathern is headed towards Norwood 6, but at what point it is gonna show in the donor zones ? I can't believe they'll pull out 6000 grafts in the back of my head without it showing badly... any thoughts ?

I did 7070 grafts for my first surgery as a Norwood 6 in January this year and donor looked almost untouched, did another session almost 4 weeks ago with 3613 grafts...

WOW, this is really insane! I can't believe there were 7k graft harvested from the donor area 😳 It defies the laws of physic.

7070 grafts to be exact from first session January this year and 3613 grafts almost 3 weeks ago 🙂

When @Melvin- Moderator have the time maybe can change topic to:

    Dr Zarev 7070 grafts 1 session + 3613 grafts 2nd session?

  3. Wasn't PRP originally for healing purposes, I know in sports it's been used for a while for healing purposes, help a muscle recover or after surgery.

Doing post op PRP or during surgery PRP probably have its benefits but as stand alone treatment for male pattern hair loss, I think not. Women I think have better results with PRP as hair loss usually are for other issues than genetics and PRP can give a boost for recovery.

Definetely dr. Zarev's donor extraction pattern and few other doctors with similar style, is something what makes his work different than other doctors who use FUE, as their patients post-op is very clean, and about using such big amount of grafts is probably depend by donor area quality, and probably he choose such patients where can achieve such results, think there is no some "special" patents  , but mostly it's about what @BoldnotBald explained ... But time will say did dr. Zarev say true or not about "patent" ...

And think he will not be soon here on "recommended" list, as he doesn't have consultants as mots of clinics, this @HugoX  explained, and seems as HugoX wrote his patients mostly Bulgarian, and they will not post here probably, as he charges 4e per graft, and by 2x surgeries per week for Bulgarian standard he get profit, same as some doctor in Turkey performs 4x surgeries per week, but even it's like that, still strange work without consultant, no much info in public for international patients, as even top recommended doctors from Belgium, Spain, still have consultants, social networks that international patients easier find them, and available at forums too...

Interesting will be to hear where @HugoX got recommendation or found info about dr. Zarev :) ?

Doing all as 1-man show think can't lead to much success in future, else if he is only local oriented, as if you not get email reply in some normal time frame like 1-7 days, but need wait 1-2 months, rather will choose any other doctor with same or similar results, like Eugenix, Conior, or any other... And bytime it makes too much tired person who does all - email response, extraction-incision-implanting, consultations, etc. , strange view of doing business in this industry by Dr. Zarev...

I found Dr Zarev in videos and another recommend list of top surgeons, after that started to try and find info about him and found his website that for me was one of the better ones in showing before and after pictures in terms of lightning, angles and the amount of pictures, and there were many YouTube clips and I saw that there is a consistency in all the results. Also that he was basically one of few that had and showed many Norwood 5 and 6 with great results.

Comparing for instance to Dr Bisanga who also do excellent work, Dr Bisangas results for the most part are between 2000-3000 grafts on Norwood 1,2 and 3s and always for the most part on persons with great donor and existing remaining hair.

This is also interesting I think, the discussion about different surgeons and their work, why do you we not see many totally bald, Norwood 6 patients results with great donor capacity from as an example Dr Bisanga? Doesn't people with great donors capacity and Norwood 6 go to Dr Bisanga? Is he only choosing Norwood 2/3 with great donor and great existing hair?

How many surgeons can do 8000-9000 grafts over 2 days on a patient with great donor capacity?

Another thin I don't understand is why there is so much focus on patent talk? Dr Zarev have developed a tool that suits his way of work for extraction, but hes not exactly going around and bragging about it or saying it's revolutionary online, on forums or his website, for now he is the the only one that works with this method and working well by the looks of it. I don't think Dr Zarev is focused on these things now as there is no need.

I also think that because Dr Zarev isn't active in this or other forums or pumps out YouTube videos or interviews doesn't mean his not "successful", is that how you measure successful?

(He's btw fully booked last time I was there until end of November 2022 (of course there will be cancellations in between))

For me I would rather see a surgeon that is consistent in his results and has high involvement with the patient before, during and after surgery and this Dr Zarev deliveres without a doubt.

And its not Dr Zarevs fault that his patients doesn't go to forums and write about their experience. He even didn't know I was writing in forums until a couple of months after my first surgery when he was getting inquiries saying they saw my post on a forum.

But he knows it's not that good that people who send inquires (specially non Bulgariens) have to wait a long time for a reply as the staff working now does not speak English and Dr Zarev who speaks English is busy with surgery and patients.

But he hopes this will become better when opening the new clinic and hiring a patient coordinator that speaks English.

  5. As I was tagged and only active patient of Dr Zarev at this forum I can say a couple of things.

The amount of results that Dr Zarev have published is nothing compared to his portfolio on his hard drive/cloud storage. Each and one of them as amazing as the ones published and different Norwood scales, not just Norwood 5/6/7s with excellent donors.

Someone commented that Dr zarev extracts "all over the place", how is he's extraction pattern all over the place but not Zoomsters as an example from Dr pradeep, if you compare he's and mines post op after second transplant, how is not Zoomsters all over the place? What am I missing?

    (Couldn't find exact angles) SmartSelect_20211101-191842_Chrome.thumb.jpg.ce857cb66e21eb5a2b92a27cb799f2d0.jpg

    SmartSelect_20211101-192639_Gallery.thumb.jpg.53aee5a52df95f46c0f84a1af8967de9.jpg

And obviously Dr Zarev wouldn't extract the same way if I went to him when I was around 23 years old, still a lot of hair left but crown and hairline/temples thinned out.

And his "technique" that also was mentioned I am guessing is about his vacuum assisted extraction method, I have asked about this, obviously he would not tell me exactly all the details about this which is understandable, from my understanding the machine is his own developed and there is no other kind like the one he has, I belive it was based on another similar vaccum assisted machine but he has changed and developed so much that there is no other like it and he is the only one today that is capable of handling this machine, in other surgeons hand without proper training would not work well from what I understood.

I have been basically totalt bald, Norwood 6 since I was 30 years old, from that time to January this year I have looked the same, and I will turn 40 in a month, my safe zone is obviously different than someone who is still Norwood 2/3/4 and younger or around my age. 10 years from now if I had done nothing I would 95% have the same donor as I had before any surgery (as long as I am healthy).

Dr Zarev have taken in calculation that I will loose a bit more on sides and back (from the top) and that is why he has saved around 1500 grafts for the future.

And let's not even mentioned my beard that would easily give 4-5k grafts but we haven't even touched that subject.

I have asked Dr Zarev why he isn't publishing more results on his Web site or social media, simple answer is time, Monday to Friday basically 09 clock to 23/00 in surgery and consultations and he is also busy overseeing the new clinic that's being built.

As Dr Zarev does most(all) of his work himself, even the little things that other surgeons just don't do it simply leaves him no time to be active on social media or interact on forums which is a shame as there is alot of curiosity about him.

I will end with saying that Dr Zarev does his work mathematically, hi calculates and measures to see what is achievable. Age, current and future hair loss are all within his calculations.

Hugo, are you on a medical regime, and are you privy to your donor stats pre surgery in terms of density per cm2 ?

    Thanks

Just minoxidil with 0.1% Finasteride and some vitamins and shampoos.

Donor stats I can't say I have (or remember) but I know we have taken approximately 60% of my donor.

And recipient has an average of approximately 50 grafts/cm2 I believe , higher around hairline (around 55 grafts/cm2) and lesser cm2 other places.

Honestly cannot believe how awesome this transplant is! Do you find your back and sides have thinned out in your donor area when it’s grown through?

Well obviously it's going to be thinner than before any of the transplants 😅, after the first 7000 grafts I barely noticed any difference from before (as you can see from before pictures before 2nd session).

I am guessing it will be a bit more noticeable after this round compared to before first and before second sessions.

We shall se after 4 months, but skin fades is not an option any more as it would be more noticeable.

Utterly outstanding! Even your first round with him looks amazing buddy! Did he put more singles in your hairline? Can’t tell by the photo as his works super clean! Is he still operating out of the Galatea clinic? Awesome results buddy!

I think a couple of singles were added, but most was behind hairline for definition and to build density all over.

Yes, still at the Galatea clinic, but I got a sneek pre view of the new clinic that should be finished in January (if no more delays).

Looked very nice and exclusive, going to be a nice clinic with views towards Sofia Mountains and also accommodation for 1-2 nights for the surgery days.

Hugo thanks for sharing those pictures, looking good.

What made you want to go for another surgery after your last results? It looked like you had very good coverage and could almost pass as a full head of hair.

I guess a little bit of hair greed, I would have been happy with what I achieved from first surgery if nothing else could be done, but as I had the opportunity to gain even a better result... Why not 🙂

  10. Hi guys,

Back home now and after some rest, here is some update.

Summary of first surgery that was done beginning of January 2021, 7070 grafts ( I thought it was 7000) and now 10 months after 3613 grafts.

First some before pictures before all of this started as e reminder, followed by 10 months result after first procedure and post op pictures.

After this procedure I should have approximately 1500 grafts left for the future according to Dr.Zarev.

Long on this forum but just as a silent watcher. :)
Done the surgery with amazing dr Zarev 3 weeks ago. Probably will keep you updated with my results also (5600 FUE).

Oh, sounds great, another Dr Zarev client that will share his story, I think the forum need more.

Great, keep us updated with your own thread.

To implant a hair follicle, there has to be an opening of some kind.

I think his 'USP' as such, is a vacuum assisted punch he uses for extractions - @HugoX has had surgery with him, so he will be in a better place to help advise.

Correct, extraction method is as mentioned vacuum assisted punch which there is some info about here, https://www.hairtransplantation.bg/index.php/en/hair-transplant/transplant-methods/the-ava-fue-technique-we-apply

The implanting is done as also mentioned with implanter pen, (no premade slits),. And he does all parts of the surgery himself, exept loading the implanter pens (obviously).

He did mention that he will in the future explain a bit more about his extraction method.

    HugoX: About financing at Dr.Zarev : If the price of 1 graft = 4 euro; then 7000 x 4 = 28.000 euro for 7000 grafts right?

Did you had to pay all in one time? Or is there any financing options available at Dr.Zarev?

    Thanks in advance :)

The plan was actually 6000 grafts, but ended up with 7000 grafts, but Dr Zarev did not charge me for the additional 1000. From what I have experienced for Dr Zarev it's not about the amount of grafts, it's about the result and making sure to get the best result/capacity out of your donor.

And yes you pay everything in one go, when booking a date you transfer 10% of the total amount agreed on to commit yourself.

  14. the only way I see this as possible, and I have a friend who did this, and its difficult to say what the results would have been if he had not done this, is that where the system is attached (front/sides and back, basically all around), is not attached to anywhere the transplanted follicles are, so you would need to make your system bigger so that you would go over that line, so attachment is only on where the donor is.

I hope you understand what I mean, so that would mean adding a 1.5-2cm, (making the system bigger) to where you normally would attach and only use tape, no bond/glue and change tape every 3-5 days, and as much as you can when home and not working go without.

Of course, the first 10 days until its healed and scabs fallen of you cant wear anything, if you can go 2-3 weeks without anything even better, maybe when you have vacation or maybe its possible working from home….

Last step? This is not the last step (LOL). This is incredible and doesn't look as if you have lost a strand. Helps for sure as it looks as if you have been genetically blessed with good quality and plentiful donor.

    Enjoy!

atleast hoping last step for a long time 😅. Thanks

Do you know when the doctor is switching to his new clinic ?

No, not exactly, he did mention October, but it has been delayed, so when I go back for my last session end of October it will at the current clinic. He is hoping for end of November I think. I was hoping to be first one there, oh well :)
