Doctor rahal does not do open technique to harvest strips?

My first HT with Dr Path, he does open technique to harvest donors. Less graft transection this way because the grafts are watched while the skin is cut. And my scar healed up very good, i could not feel the scar nor feel any bald line where the incision was.

 

For my second HT it was with Rahal and I remember a machine was used to slide across the donor area that cut the skin. Meaning there must have been grafts that were transected and not used in the transplantation. And my scar is lumpy and has a bald line. Maybe due to tension. Dont know.

 

But question, why didn't rahal use open technique?

I'm not sure what you mean by open technique. The machine you're talking about is not used for the incision; it is a vibrating machine that is run along the area to 'confuse' the nerves during the removal of the strip, helping to reduce any discomfort that might be felt.

Apologies for hijacking this thread but think its related. Matt, i'm seriously considering a FUT procedure with Dr Rahal (i've previously sent all my information to the clinic and this was his recommendation), i'd like to understand what Dr Rahal would be doing in the procedure vs a technician. Are you able to advise?

But while the machine was run along the donor area, sometimes I felt skin being cut and was given injection. Was the machine cutting the strip or was it done by hand?

Dr Rahal removes the strip and performs the closure. He also makes the incisions, which have to be made to the correct depth and angle. The dissecting of the grafts from the strip is done by the technicians under microscope and technicians place the grafts. Those tasks of course require multiple people.

That's because the vibration machine is used during the cutting, to help minimize how much you can feel it happening. Obviously the area has been numbed with injections, but the vibration is a little extra. I can understand how you might be confused as you can't see what is going on back there during the procedure.

I see. Thanks for the reply Mattj. So Rahal does do the strip cutting. Your right, it was hard to know whether the machine or rahal did the strip removal. But all good. Just needed clarification. :)

Just to clarify further.

The vibration nerve fibers are different from the pain nerve fibers. The vibration sensation travels faster to the brain than pain fibers and closes the gate to pain traveling to the central nervous system. This is called, the gate control theory of pain.

Dr Rahal uses the vibration, using a hand held small skin massager when applying the local xylocaine (which tends to have a sharp pain sensation and is often confused with cutting.

 

Dr Rahal, is the only person cutting and removing the strip, the assistants help in handing him the instruments and holding the head during the procedure. Plus, the skin is cut only after the anesthetic has taken.

The strip is then removed, trichophytic closure done, and then sutured with two layers to ensure proper scar healing.
