Regular Member Dr. William Lindsey Posted November 10, 2009 Regular Member Share Posted November 10, 2009 This nice fellow was in a few weeks ago and had had 1500 minigrafts elsewhere a few years previously. He had thinned since his first case and had a fairly wide scar. Although he had returned to his first doctor who reportedly recommended 2500 to "fix the entire head", we chose to concentrate and densely pack the anterior 40% of his scalp. He was an easy tension free 2 layer closure, bled very little, and looked pretty good at day 7 when sutures were taken out. He left the country for a vacation but should be back around Thanksgiving for a scar check, and if pics are reasonable at that visit, I'll put them up. Dr. Lindsey McLean VA William H. I was suprised to see the extracted donor strip, visual:graft count ratio. It looked like a relatively thin strip but harvested 3200 grafts. Then, I was again suprised to see the following image of the patients head immediately after strip removal, where the area looks much larger than the strip itself. Obviously this is down to scalp elasticity and the pressure being exerted on the area. These images and this case I feel have educated me a little more, and I am grateful for that. The work looks very clean, and if only the day 12 pics could continue "shedless". Good luck to all involved. Dr Lindsey, thanks again! First, his (and almost all) scalps really contract away from the strip site and it gives a nice picture to have a fully expanded strip defect, and then a tension free 2 layer tricho closure. Regarding the strip itself....it contracts down like all excisions do. I offer men an "excision of turkey jowl deformity" that I posted on this site in the "other plastic surgery area", where we remove the floppy neck skin and close the muscles in about 25 minutes. I have done this on my own father in law and around 260 other men including teaching a course on this last year. Anyhow, Wendy, my tech, used to take a gauze and outline the skin incision, then show the patient how much the excised skin contracted in just 20 minutes. It was usually around 20%. Same deal with strips, although not to the same degree, due to the thickness and presence of significant subcutaneous tissues which limit contraction. Second, I figured we get 2700 and was surprised to get 3200; but like sex and money, often times with hair, more is better. Dr. Lindsey McLean VA William H.
