He received 2600 grafts, mostly 2s and 3s, to address a thinning crown. We shaved a little into his existing hair so that future thinning won’t leave an island of hair. Nevertheless I think he’s taking propecia to help preserve the surrounding and front to middle regions despite a pretty decent family history for hair. Note the slit placement and graft placement recreates his swirl. At the 3 month pics almost nothing is showing but he looks pretty nice now. Dr. Lindsey McLean VA William H. Regular Member Dr. William Lindsey Posted August 16, 2011 Author Regular Member Share Posted August 16, 2011 I always prefer to do the front as opposed to the crown as 1. the customer gets to see it everyday, vs seeing the crown in a mirror or pictures 2. its easier to get a nicer result in the front due to overlapping hair shafts. Remember, we are not curing baldness, but rather covering bald scalp. So in the front, most hairstyles allow for overlapping hair shafts, so all the doctor has to do is get a decent hairline and pretty good density and nature takes care of the rest. In the crown, by virtue of hair splaying out in all directions...there is no overlapping at the swirl. So even in people without hairloss, their crown looks thinner than the rest of the head. So when you do have hairloss, and you're getting some density with good placement, nature and gravity are fighting your results simply by the hair going radially in all directions. I try to beat this into all crown consultations and I use baseball analogies. In the front, I expect (not guarantee mind you) to hit a triple or a homerun. Once in a while you get a double but the front is pretty consistent. In the crown....unless the patient has pretty thick and wirey hair shafts and a good color match, I expect a double, and I hope for a triple...but I think its critical that patients know this preop. Not all remember it, but we go over this every time. But yes, this fellow certainly got a triple. Dr. Lindsey McLean VA William H.
