It was definitely make-up and is consistent with how Hollywood has portrayed the procedure for decades. I remember specifically however at the end of the scene, the agent asks Hellboy if it looks good and Hellboy responds, "Absolutely, I've been thinking about getting it done myself sometime" but the response was an obvious joke.
