  1. This video shows how we address hairline feminization. This patient has had 4 browlifts, with LOTS of scarring, in an effort to lower and feminize the hairline. BUT instead, all she wound up with was a distinct hairline and lots of scarring. We did 2000 grafts across the front, softening and feminizing the hairline, and then went into lateral scars. HOPEFULLY we'll get periodic followup. AND hopefully, she'll give this the year to year and a half required to get results...(in the past, impatience has led to additional facial surgery probably before full healing occurred...)   2. To my knowledge...i.e. phone call.... no concealers. SUPPOSEDLY, he's rolling through our area next week and agreed to stop by for a video. Dr. Lindsey ALSO, he's half asian...and has quite thin hairs and you can see from his preop, and pretty big color mismatch. Not to split hairs as they say...but I'm looking at my hairline more than progression of mpb.
  3. So we get TONS of calls of guys, and a few girls, saying they had multiple surgeries and although they initially looked better...the hair they thought they got is "all gone". My best guess, and what we almost always see if they come to the office is the same thing. A person goes to a hair clinic, often gets sold a bunch of laser treatments and vitamins as they "weren't ready for surgery". After a year or so, when they have spend 2000.00 and are still losing hair...they now proceed with HT at that office. After all, they are now really comfortable with that clinic...they've been getting light treatments and spending money with them for more than a year. So they get surgery, sold as "filling in all of the weak areas". Now those clinics are great sales people...they convince the customer that they can fill in a couple of thousand grafts, with existing long hair in place, and in a day or so...the patient can go back to work. I hear these ads on TV and the radio! Probably would sell me too on this if it weren't for forums like this! And so they get some hair placed, my best guess is that its hard for the doctor to see where to make the holes, harder for the techs to see the holes, and impossible for surrounding hair not to stick to and pull out some of the new hairs... And when a year is up or maybe longer, the clinic says...."well our surgery worked, its just you lost more hair!" Whereas in reality they have lost DONOR hair and MONEY, often with little to show for it. Here's a typical call: Please do your research! Dr. Lindsey
  4. This guy had 1500 or so to his hairline way back, 2 office locations ago! He is on meds and sent this pic in today. A photoshop novice...I put blue on his face, but his hairline looks fantastic. He was 27 at the time of surgery so I'm guessing he's almost 35 now. The middle pics are at 8 months, the last time I saw him in person. Holding up well. Dr. Lindsey McLean VA
  5. Here's a fellow that came in with 2 divergent donor scars, separated by a band of good hair. WHY? Who knows, the same guy did both cases and its beyond me why people don't just cut out or sew up to an existing scar....heck I've done a video on it! Anyhow, we cut out his scar, got 350 grafts, 6 years ago and he stopped by. Now in all fairness, he's had body hair transplantation elsewhere in the meantime, but still wants a bit more hair. So we're talking about me excising this and getting 800-1200 grafts. The trade off is...I took him from 2 wild scars and got him a good one...do we dare play with fire. I can't guarantee him his scar won't be worse....so he's thinking about it and we'll see. But look at this before and 6 year result...pretty good. Dr. Lindsey McLean VA
  6. Spanker you are absolutely correct. As usual. MFUE has its place but is no more scarless surgery than strip and FUE...each have their issues...weighing root stress vs linear scarring. And that is coming from me, who openly admits to sewing better than most doctors do, having done 3900 facelifts...not bragging, just a fact. That said, MFUE evolved from one 10 day cutaneous stitch which left, on average, a small dash of hairless skin on most people at each site, to a 2 layer closure which cut that dash in size 50% (but not 100%). As with strip, I told and tell everyone that I get 4 crappy scars every year. I just don't know who is going to get them. Its seems pretty similar in terms of percentages, for poor scars from me whether its strip of mfue. I told, and tell everyone that they will likely need 3/8 to 1/2 inch long hair over their donor region FOREVER. Now many don't but as long as one can accept that, I feel the benefit of NON-FUE outweighs the thousands of little white scars, donor depletion, and frequently (particularly in thin hair) poor results. This guy and 2 other recent ones that I've repaired, either with strip or mfue..were fue'd first by well known guys that 70% of readers on this forum know. They just didn't grow enough. And the tradeoff for them, and this guy, of a little more scarring for improved density was worth it. But its not for everyone. You're on track as usual Spanker. Dr. Lindsey

  8. Some of you guys may recall this fellow. He is a muscular young man who had a frontal FUE by a very reputable FUE doctor but simply had lower density than he wanted. Now he has decent hair, but as I've discussed many times, FUE stresses the roots in an unpredictable way. AND he likes to have a very short hairline...so I completely GET why he did FUE. But he wanted more density and strip was out, due to his hairstyle requirements. So we did an MFUE 14 months ago and he walked in yesterday. His hairline looks great, although like a lot of perfectionists...he's sure he'll want more hair down the road. I advised him to sit tight and quit looking in the mirror so much.   9. Its that time of year again when an old man's thoughts turn to Utah snow. I'll be at Park City Feb 3-6 and have 3 slots per day to see new and previous patients. Despite my frequent warnings about the crown being a black hole that sucks up every available hair....you can get outstanding results in the crown in select individuals...in particularly, those with curly bushy hair, minimal color constrast, and willingness to have the hair 3/4 of an inch long so as to layer (not combover....LAYER). Here's this fellow before, with thin miniturized hair all over his crown, and at a year, and now. And a closeup of his trichophytic scar shows MOST of the scar has hair growing right through it.   11. Spex, Don't sweat it. I hope despite the poster's comment that anyone reading the topic who is local and would like my opinion on their situation feel free to visit me for a consultation. And there are few cases harder than a black guy FUE case with scarring issues....likely I'd pass on that one and the interested party should think TWICE before jumping to something that sounds too good. To all: I certainly don't profess to making everyone happy...but we do make lots of people happy and very few cosmetic doctors are as blunt about expectation management as me.   12. So this is one we don't see everyday. This former bodybuilder lost his hearing in his 40s and got a cochlear implant. His dad had a hair transplant "back in the day" so he came in for a consultation. Frankly, I'd never seen a patient with a C I get an HT. As a former ENT doctor though, I knew we can't screw this thing up. So in the video I discuss what we did differently, how the implant shortens the strip and thus the available hair, and show him during the case and over the first week.   15. Here's ANOTHER lower yield FUE case, this time, performed by a VERY competent FUE doctor that we bolstered with a small strip case. The video goes into a bunch of issues, including FUE vs Strip, cleaning and smoking, Color contrast, expectation management, and a short video of me running a comb through the result at just under a year. I've put up a bunch of these this month....don't get me wrong FUE has its place. Many of you may remember I did and FUE on my son....but the false advertising of scarless surgery with the same results as strip, that I see and hear almost every day..is wrong. This guy had a fine FUE result...probably comparable to what I'd would have accomplished with fue on him....it just wasn't enough. Now maybe the original doctor told him what I'd tell him...that he'd want a second case or it wouldn't maybe be enough, but the stress to the roots simply doesn't allow FUE to equal strip. And most doctors are not as blunt about stuff like this as I am...hence so many repairs of this issue in the past 2 years (with a bunch being by well known FUE doctors). Bottom line...make sure you talk about the pros and cons of all surgical issues PREOP with your doctor and make the decision that is best for you. The video is: Dr. Lindsey McLean VA
  16. This fellow is now 13 months out from us doing an MFUE to repair his low density FUE, which was done by a perfectly competent FUE doctor. I explain why I think he got low yield from his FUE (skinny roots don't like the FUE stress), discuss his MFUE scarring (which went from good to bad back to pretty good), and show that even in a packed hairline...guys with fine hairs and significant color contrast still have fine hair!   17. Here's yet another scar that rolled in for revision. Now, frequent viewers will know that I think scars are Part Doctor, Part Patient physiology, and Part Luck....and post op care. So this middle aged Indian has a wide scar, too low on his head and we'll try to improve this. We did this yesterday, and he emailed me this morning from out of state, that all was fine and he's not having pain...For a small guy he's TOUGH!   19. Some of you may recall this guy's intro video where Wendy and I review likely reasons for his poor result. Mainly, unsuccessful extraction of roots with hair, lack of dense placement, and probably surrounding hair sticking to and pulling out some grafts. He came around and agreed with my assessment so we did a repair using strip and here's his video. It starts with the same intro but about 4 minutes in we get to his case. Now....I don't know that this will work either...but, statistically he will get a nice result in a year. HOPEFULLY he'll come for periodic checkups so I can update his progress.   20. Here's an update on our lady who was in a car wreck and lost the central portion of her right eyebrow. She's growing some already, will peak in another year, and may yet need extra hair. Like most females (recall I've done 3900 facelifts) its virtually impossible to get these ladies to come in without makeup. At a year, I'll make her wash it all off, but today she has "school and kid duties" and won't wash off her makeup.   21. Here's an update on a bushy haired 60 year old that we did a plug repair on 18 months ago. He looks good, better in person than either the videos or pictures AND we're almost certainly going to do a 2nd case to work toward his crown late spring. The video shows him now, and if you keep watching, shows him at 12 months and goes into my philosophy on plug repair. Also, as we're redoing our website...the guys told me to put things on VIMEO, which I'd never heard of...so this will be the first attempt to post from vimeo.   22. This nice lady from out of our area sent in some pics of good growth at 6 months. We did the same case a few years ago on her niece. She likely will spin by sometime this spring and we'll get a picture/video in person to add to this. At 6 months she looks fantastic and will only get better. In the video I talk a bit about my reservations about female HT...in that its critical to discuss expectation management BEFORE the patient signs up. Its pretty easy to make most men satisfied or really happy. BUT females need to know the limitation of their donor region and that there is no way to make MOST females with female MPB look like the ladies on the cover of the supermarket tabloids. Many of my competitors apparently fail to discuss that as we see an angry female patient from somewhere else about once every 3 weeks. The video is: Dr. Lindsey
  23. This fellow lives out of state. He came by last week to plan his second case. Now he is a challenge! First off, he had LOTS of scalp to cover. Second, he has limited donor hair. Third, his donor hair is FINE...meaning like skinny trees (see any of my numerous planting videos on youtube) it takes more skinny trees or skinny hairs to cover an inch of real estate than if its thick bushy trees or hair. This short video shows how I approach this problem and views of him preop, a short video at 6 months, and then now at 23 months. We'll do his second case in a few months and I'll update this. Although he's busy, and out of our area, hopefully he'll log on and add his 2 cents. Really, on guys with this problem, EXACTLY half of my potential patients say that if we can't cover everything, they want nothing. AND half say framing the face and having a bald crown is a great option. Those of you who've seen me in person, with class 5-7 hairloss, know that I START the discussion with that sentence.
