  3. Hi Everyone, we would like to present below a case of a patient with hair loss at stage Norwood III with diffuse thinning in the mid scalp and crown. You can find below the details of the surgery. Patient Details Level of Alopecia: NW III + diffuse thinning in the mid scalp and crown. Surgical Plan: Hairline reconstruction, density increase in the frontal and fronto-temporal area. Due to the age of the patient (30 years old), Finasteride and minoxidil treatment was recommended to stabilise his hair loss and improve miniaturised hair. Patient had to discontinue minoxidil after 4 months of usage. Finasteride treatment is still in use. Hair Thickness: 46-48 microns Hair Type: caucasian patient, brown hair, thin hair Number of Grafts Implanted: 3648 0.8mm motorised punch was used for the extraction. Graft Breakdown 1s: 554 2s: 1978 3s: 918 4s and 5s: 198 Overall hair/graft ratio: 2,21
  4. Hello Nikos, Please find my replies below: 1) Can i make this exam in an doctor in greece and then send the results of trichorizogramme fotofnder trichoscale here to check my donor area? By trichoscopical analysis, we mean that Dr. Turan assess your donor and recipient area with a trichoscope to identify if there is any miniaturisation or any other issues. 2) I read that with dose that they not have good donor area and have agressive hairloss (my case), FUT should be better, as more grafts can be extracted. So, if i do not cut my hair on back of head very short (so the scar is not noticable), that means that FUT is better long term option than FUE? The yield in a FUT extraction is greater than in FUE. Both techniques have their pros and cons, you need to assess them and choose what suit you better. 3.Ok, couto seems the best of the word. After couto, which iconsidered s the best independelty of the cost and region for the Natural hairline design? your personal opinion pls. Freitas? Zarev? Konior? Hasson and wong? It is not our position as a hair transplant clinic to assess the job of other hair transplant surgeons. Though, and this is my personal opinion, there is no best doctor in the world. Each doctor has his/her artistry and this will lead certain patients to like one and other patients to like others.   5. Hello Nikos, Thank you for your message. After reading your thread, there are several things Dr. Turan would like you to take into consideration here: 1) It is not possible to diagnose retrograde alopecia only from photos; a trichoscopical analysis will be needed to determine if the area is affected by it. Having said that, some of your photos seem to indicate retrograde alopecia. Once that is established, it is important to understand that signs of retrograde alopecia at an early age are an indication of a high norwood progression in most of the cases. Furthermore, you already mentioned that your family members are Norwood VII from an early age, thus it is likely that it would be your pattern of progression too - as your lower crown seems to be affected at this point in time and your safe donor area seems to be very small in size -. 2) In your case, the usage of medication seems paramount to maintain your native hair. You may consider dermarolling for your crown area. We have seen very good improvement in young patients who use this therapy. 3) From the cosmetic point of view, it is easy to understand the willingness of a patient of your age to try to fix the situation as soon as possible. But, in your particular case, donor area management will be key to achieve a long lasting solution. Best of luck on the journey.   7. Hi Everyone, we would like to present below a case of a 28 years old patient with diffuse thinning in the frontal, mid-scalp and crown areas. The patient was recommended to start finasteride and minoxidil, he started the treatment 3 months prior to the surgery and he continued afterwards. Patient Details Level of Alopecia: Diffuse thinning Surgical Plan: Without lowering the hairline, Dr. Turan worked across the entire scalp to increase density. Hair Thickness: thin hair 49-51 microns. Number of Grafts Implanted: 2430, from which 1157 were implanted in the frontal. And 1283 were used in the mid scalp and crown. Donor Area Graft Density: 60 grafts/cm2 Graft Breakdown 1s: 653 2s: 1465 3s: 311 Overall hairs implanted: 4516 Overall hair/graft ratio: 1.86 NOTE: The after photos were provided by the patient and the difference of hair colour is due to the different lighting conditions.   8. Hello Gramatik, The extraction is performed by our extraction specialist, Mehmet, who has 9 years of experience doing extractions. All our extractions are performed using manual and micromotor punch. Normally the manual punch is used for the parietal areas and the micromotor for the occipital. Also, please note that the grafts are extracted in stages. Each extraction consists of approximately 1000 grafts, once the grafts are extracted and classified, then Doctor Turan will do the implantation. Then, there is a lunch break and after it, another extraction and implantation takes place. If the patient needs more than 2000 grafts there will be a third extraction in the day. When the patients need more than 3000 grafts and the surgery is performed in two consecutive days, there are two extraction sessions per day.   10. Hello RandoBrando, As you mentioned most of the hair mills charge fix price. However, our clinic has nothing to do with a set up of a hair mill. Dr. Turan performs maximum two surgeries per day, and when the surgeries require more than 3000 grafts the surgery is performed in two consecutive days and in those two days, there will be two patients being treated. Also, during the surgery the grafts are extracting in series of around 1000 grafts and they are implanted immediately limiting to maximum 1.5 hours the time that the grafts are outside of the scalp. This means, that a surgery of 3000 grafts is performed in three sessions of 1000 grafts each. When the surgery is split in two days, there are normally two sessions per day depending on the number of grafts needed for the patient. These practices set us far apart from a hair mill.   11. Hello Randobrando, I believe we are not the only clinic whose price is based on graft brackets. Our logic is that we want to remove the uncertainty of the final surgery price from the mindset of the patient. By charging a closed price based on the number of grafts, our patients know since they leave their home country how much the hair transplant surgery will cost them. It might seem banal but let´s be honest the price of a surgery plays a key factor in the decision making process and not to know exactly how much the surgery cost until the surgery finishes brings additional stress to the patient. We are aware that many clinics around the world charge per graft but based on the feedback of our patients, they highly value the fact of knowing how much money they will pay for their surgery since they do their bookings.   12. Hello Gatsby, Yes, as stated in the original post, the patient was advised to use Finasteride and Minoxidil for the mid scalp and crown area which were showing some thinning. This was decide by Dr. Turan due to the young age of the patient.
