  2. Hi Everyone, we would like to present below a case of a patient with a Norwood VI stage. You can find below the details of the surgery. Patient Details Patient´s age: 35 years old when the first surgery took place. Second surgery took place 1 year after the first surgery. The patient started to use finasteride 1 month before the 1st surgery and he continued afterwards Hair Thickness: 52 microns in the parietal areas and 54 microns in the occipital area. 0.8mm micromotor and 0.85mm manual punch were used for the extraction in both surgeries. Surgical Plan for the 1st surgery: Hairline reconstruction, density increase in the frontal, fronto-temporal area and mid scalp.. The surgery was performed in two consecutive days with 5 sessions of extraction and implantation. Overall, 4677 grafts were implanted, 3318 in the frontal area and 1359 in the mid scalp. Surgical Plan for the 2nd surgery: density increase in the frontal and mid scalp and crown area. The surgery was performed in 2 days with 4 sessions of extraction and implantation. Overall, 3226 grafts were implanted, 338 in the hairline, 1489 in the frontal and mid scalp area and 1399 in the crown.   3. Hello @Rawkerboi, the patient mentioned some concerns about thinning in that area. During the consultation once the head was shaved and assessed with the trichoscope we identified some areas that could be improved hence the implantation.

Patient Details: 28 years old patient using dutasteride and minoxidil prior to the surgery. Surgical Plan: Hairline reconstruction, density increase in the frontal, fronto-temporal area and crown. The surgery was performed in 1 day. 1959 grafts were implanted in total, 250 single grafts were implanted in the hairline, 1500 in the frontal area and the rest in the crown. 0.9mm motorised punch was used for the extraction. Patient Details Surgical Plan: Hairline reconstruction, density increase in the frontal, fronto-temporal area, mid scalp and crown. The surgery was performed in two days with 5 sessions of extraction and implantation Patient´s age: 50 years old and he did not use any medication pre or post surgery Hair Thickness: 58 microns in the parietal areas and 61 microns in the occipital area. Hair Type: caucasian, grey and average thickness hair Number of Grafts Implanted: 4847. From them, 2667 were implanted in the hairline and frontal area, 1317 in the mid scalp and 868 in the upper vertex and crown area. 0.8mm micromotor punch was used for the extraction.   6. Hello @AhmadChaudhry, Please find the replies below: 1- What size of blade or needle is used for grafts placing? Dr Turan uses the implanter pen for graft placement, that means that there is no pre-incisions made. He uses implanter pens that range from 0.7 to 1mm width. In patients with very thick hair, he uses 1.2mm implanter pen for multiple grafts (4 and 5 hairs) 2- What is the density per sq. centimeter? If it is convenient you may reply otherwise no issue. Density per cm2 in a hair transplant depends on many factors such as: hair caliber, hair/graft ratio, hair colour, skin laxity conditions, native hair or no native hair in the areas to be implanted, areas of implantation (hairline requires greater density than other areas), etc. So, they vary for every patient. In this case, the implanted density ranged between 50 to 40 grafts per cm2.

The patient was using finasteride prior to the surgery and he continued using it afterwards. Hair Thickness: 45 microns in the parietal areas and 49 microns in the occipital area. Hair Type: caucasian, dark and thin hair Number of Grafts Implanted: 1968, all grafts were implanted in the hairline, frontal and fronto-temporal area except 300 grafts that were implanted in the mid scalp and upper vertex. 0.8mm micromotor and manual punch were used for the extraction. Both Dr Turan and Dr Gur are training other Doctors to increase capacity in our clinic, but neither Dr Turan or Dr Gur will retire/transition or change their involvement in their surgeries. The patient was only using minoxidil sporadically before the first surgery. Afterwards, he started using Finasteride and minoxidil daily. Surgical Plan for the 1st surgery: Hairline reconstruction, density increase in the frontal, fronto-temporal area and mid scalp.. The surgery was performed in two consecutive days with 4 sessions of extraction and implantation. Overall, 3597 grafts were implanted, 2408 in the frontal area and 1189 in the mid scalp and crown. Hair Type: caucasian, light brown and thin hair Number of Grafts Implanted: 2888, all grafts were implanted in the hairline, frontal and fronto-temporal area except 200 grafts that were implanted in the mid scalp 0.8mm micromotor and manual punch were used for the extraction.   12. Normally, a natural hairline won´t have such sharp design in the fronto-temporal area like the one you marked. As you can see in the implantation, Dr Gur only had to close the fronto-temporal area slightly to link it with the temporal peaks of the patient. Due to the fact that the patient did not receed at all in the temporal peaks, there was no need to implant in that area.

  15. Hello Duckling, the patient had thinning in the mid scalp area, hence Dr Gur spoke to the patient during the consultation to work on that.
