  2. Hi, We would like to present this case of a young lady who suffered an area of hair loss in her hairline due to trauma as a child. This always affected her confidence and she was very conscious of it. She attempted to cover the area up but because it was in the central frontal hairline it proved difficult. Sometimes it can be difficult to predict the growth of the grafts when there is a traumatised area but she got a very good yield from the 399 grafts implanted and she is very happy with the result.   5. Thank you for all the feedback, it is appreciated. I have had a chance to discuss this with Dr Arshad. @Gatsby totally agree that over time the scarring can get less. Obviously this is dependent on the individuals biology and ability to heal. At the moment the patient is just relieved he can go out in public again and move on with his life. Continuing to apply bio-oil may improve the appearance further and he can always apply some concealer/foundation on the scarred skin. In terms of scalp advancement flap/forehead reduction surgery, Dr Arshad stated that this would only be an option if his male pattern hair loss was stable over a number of years and as a last resort if the scarring was an issue that persisted over time. In terms of the details of forehead reduction surgery, essentially the zone of scarring (shown in black) from the extracted grafts would be excised as per the picture below. This would involve making a zig-zag incision in the hairline, raising the scalp flap, advancing the scalp and anchoring to were the forehead skin has less scarring (lower line of the zone coloured in black). The closer of this linear wound would be using a trichophytic technique so hair follicles can grow through the scar. Essentially, the larger black area zone of punch scars would be exchanged for a finer linear scar hidden within the hairline. As a result of this, of course, the hairline would move around 1.5cm lower (depending on scalp laxity). Prior to doing the punch extractions across the full region of the botched previous hairline (in order to salvage as many grafts as possible) these future options were discussed by Dr Arshad in depth with the patient. In any case the patient is now enjoying his new hairline and further intervention can be considered only if and when required. I hope this information is useful, thanks again. Regards Adam
  6. Hi, I would like to share this case with you. This 26 year old male had an FUE hair transplant (not in UK) several years ago. Since then he had further male pattern hair loss and developed a large recessed area between the hairline that had been implanted and his native hairline (which had moved further back, particularly in the frontal temporal areas). He was quite distressed with the unnatural appearance, he struggled to cover the receding areas and found the new hairline to be unnatural (with poorly directed hairs). After multiple consults he spoke to Dr Arshad on several occasions and options to repair his hairline were discussed in depth. He then underwent punch extraction of the previous hairline and FUE from scalp donor to reconstruct a more natural hairline. Some points to note about this case are: the hairline that was previously implanted was too low and into the region of the frontalis muscle, these can be removed through various means, punching out these grafts has certain risks for example scarring, and regrowth of hairs. The patient was keen to punch out all of the previous hairline so these same grafts could be used further back. the punch extractions was difficult as the hairs were very misdirected the patient has some regrowth of the punched out grafts which he plucks and trims, he has some scarring and induration in this area because of the regrowth of some of the previous grafts in the hairline, the options of further punch out extractions was discussed with the patient. But he is worried about further scarring in this area. Also the option of excising this area and raising/advancing a forehead flap was also discussed with the patient, as he is now happy with the density at the front of his new hairline and is on finasteride, therefore after a period of observation (to ensure stability of his male pattern hair loss) such a forehead reduction procedure with Dr Arshad is an option for him in the future. Graft breakdown: - Punch outs from previous hairline 395 - Singles - 110, doubles 40, triples 1,139 Total number of grafts implanted = 1684 I hope you enjoy reviewing this case.   9. Thanks for pointing that out, I agree and I noticed this too. We have recently moved our hair transplant clinic into Leeds Private Hospital and so the lighting in the room has changed from the before picture (which was taken in our old clinic). I am going to move the studio into a room with no natural daylight and just LED white flash lights as that is more consistent in terms of reproducing similar lighting conditions to the before pictures. This patient is attending for follow up again in a few months so i'll post more pictures on this thread. Regards Adam

  12. Hi, Fine, light, blonde hair can lead to a requirement of multiple surgeries in order to achieve a denser result. However, depending on the graft quality/number of hairs per follicle a denser result can be achieved in some cases with one pass. This patient has coefficient of 2.77 hairs per follicle in the extracted grafts, which meant he has got a good coverage already at 6 months. Many factors can affect the outcome of hair transplant surgery, the hairs per follicle coefficient is one such factor. Hair graft growth after a hair transplant is unique to each individual and no two journeys are the same. This patient is very happy with his progress so far.   15. Hi everyone, This is a recent patient of ours. He is late 30's and is tall (6'7). He has large head circumference and advanced hair loss. But his donor area is very dense and thick hair calibre. We performed 5122 grafts over 2 sittings. Surgical details: Singles - 738 Doubles - 1351 Triples - 2439 Fours - 594 He is very happy with progress and we are planning a third surgery to add more to the crown. We have a comb through of his hair also which I will upload onto youtube and link here. Dr Arshad wanted to relay some important information via this case: - the surface area/head circumference has an impact on the amount of grafts required to create density - the type of hair (thickness/tone) has an impact on the perceived density - the amount of intra-operative bleeding/popping can influence how close the grafts are implanted - it is much safer to build up density over several sittings rather than try and overpack in one go - there are no strict hard and fast rules about the number of grafts that are required to create a density the patient is happy with. For this patient and many others we will likely end up doing 8000 grafts (his donor availability allows this). Most patients with advanced hair loss end up having multiple sittings depending on what their expectations are. Hope you enjoy!   16. Hi guys, I want to share with you some pictures of our patient, that he recently sent to us. He had his surgery in January of this year with Dr Arshad. He is in his early 40's. He is not taking oral finasteride but is using min/fin topical spray that we supply. Surgery details, FUE: soft singles = 125 1's = 275 2's = 1138 3's = 2505 Total = 4043 grafts He is coming back to our clinic for follow up at 12 months, so I'll get more pictures and a comb through for this thread to add later.   21. Dear forum members/viewers, This patient is from London and underwent FUE hair transplant by Dr Arshad in our West Yorkshire surgery centre. The results below are shown 9 months after surgery. Graft count: Soft single FU - 74 Medium single FU - 223 Large single FU - 43 Double hair FU - 1604 Triple/+ hair FU - 1000 Total = 2944 Meds - Finasteride 1mg / Minoxidil 5% PLEASE NOTE: Individual patient results and the onset/rate of growth from transplanted grafts may vary considerably between patients. The overall yield and appearance of density from a hair transplant is dependent on many factors including but not limited to: The extent of surface area that requires grafting (most patients with advanced hair loss will require 2 surgeries to reach reasonably good density). The blood supply to a particular area of the scalp (this varies from patient to patient). The extent and duration of the inflammatory response of the scalp post operative (this varies from patient to patient, in patients with persisting perifollicular erythema post operatively we recommend use of Ketoconazole 2% Shampoo). The texture, size and colour of the hair (large hair calibre provides better coverage, lighter skin to darker hair colour contrast requires more grafts for appearance of density). The innate ability of the hair follicles to withstand the trauma of surgery (this varies from patient to patient & also from different regions of the scalp within the same individual). The use of medications pre and post operatively (oral and topical DHT blockers, finasteride, Minoxidil and other anti-androgens).
