I never get tired of seeing your result!

The last time I saw it I believe was before your last 660 graft procedure with Dr. Hasson. For those wondering here, it looked impressive even before the last one. I can imagine it’s even better now and the pics give us a great idea. Hope you have been well and always nice to see you sharing here.

Seattle area patient in his late 30s with fine textured hair who had been wearing a hair system. The first 4482 graft FUT procedure accomplished the mission of rebuilding the hairline, frontal zone and mid scalp. 1232 grafts were single hair grafts, 2872 were doubles and 378 were 3-4 hair grafts. The second 2900 graft surgery concentrated on some of the mid scalp and mainly the crown. 709 grafts were singles, 2061 were doubles and 131 were 3-4 hair grafts. Shortly after these photos were taken, he had a 3rd procedure consisting of 2341 grafts to increase density in the crown with some hairline refinement. A total of 7382 grafts were needed to achieve the result seen here. We hope to be able to update again once the latest procedure is sufficiently grown in to reflect the additional 2341 grafts.

I would say in this case it is correct 8,000 FUT grafts would equal the 6,000 FUE grafts associated with your case. If this patent’s hpg were closer to the average, then around 7,000 grafts via FUT would have equaled your 6,000 grafts via FUE. So you can see how things can vary from case to case. But we can conclude that FUE would likely yield a higher hpg than FUT in most cases. I generally figure on about a 20% higher hpg for FUE versus FUT give or take.

Good question, I’m glad you asked:-)

The short answer: FUT versus FUE.

The long answer: Actually according to my calculations, this patient had around 9750 hairs compared to your around 9300 (from one of your posts) but there is a difference in the hairs per graft or hair/graft ratio as you observed. You had noted in that post that your hairs per graft (hpg) was 2.33. This patient’s hpg is about 1.75. For a FUT procedure, I would say his hpg is on the lower side, he had a higher number of single hair grafts. I would estimate most FUT cases would fall around 2.0 hpg give or take. With FUT, we are taking out the strip and are presented with a distribution of grafts, we do not have the luxury of selecting only the ones we want. With FUE, we can “cherry pick” only the grafts we want. This allows the doctor to determine exactly how many of which type of graft he wants. So for FUE, he would likely harvest a higher number of 2 or more hair grafts. After rebuilding the hairline, he will want/need very few if any single hair grafts. As I mentioned in another thread, if we have a higher number of single hair grafts in a FUT procedure such as this case and we are done rebuilding the hairline, we will often place 2 single hair grafts in one incision to create more volume and density in a given area outside of the hairline.

Dr. Hasson and team rebuild a new hairline, restored the frontal zone, mid-scalp and crown. 5551 grafts were transplanted of which 1735 were single hair grafts, 3557 were doubles and 259 were 3-4 hair grafts. These photos were emailed to me by the patient 7 months post-op. As you can see, he is using gel in his hair. The nice thing about pics with gel is you can see into the scalp and get a good view of the uniform coverage. You can't hide any imperfections or lack of density when you can see the hair exit the skin. It takes a relatively high density to be able to pull off this look.

Some updated pics sent in by the patient. Around 1 year post-op with hair product. We'll try and get a video and more shots when he comes in for another follow-up.

Thank you for weighing in. Yes, we do have the tools and staff to do most anything when it comes to hair restoration and matters of the scalp. You do bring up an interesting point regarding fewer incisions but I don’t think this relatively small amount would make a material difference in the footprint of the donor. As far as the other side of the equation, for every one less incision during extraction, we would then have to dissect the graft so there would be no real labor advantage. We’re always trying to create the very best possible cosmetic impact. If that meant splitting grafts then I think it would be considered. I believe if possible, keeping the grafts in their true follicular units is ideal. So if we can just take the exact type of unit we need, then taking it out as it naturally occurs is preferred. If a particular patient in some rare case had almost exclusively 2 or more hair bundles, then we would, of course, need to consider splitting 2’s into1’s to get the necessary singles for the hairline. We naturally would not want to use 2’s in the hairline. The converse holds true for FUT procedures. If we have an unusually high number of singles, we will double up 2 single hair grafts into one incision where necessary. This will create more density than simply putting in single hairs and essentially doubles the volume of hair in the same given space versus all single hair grafts.

There is, however, an instance when splitting grafts may be warranted. If a patient has very coarse black hair and especially with a lighter colored scalp, we will consider splitting a 4 hair graft into two. If you transplant a 4 hair graft with a very large hair shaft diameter, it can appear to look “pluggy”. If it is dissected into 2 double hair units placed side by side, it will look more natural than the one 4 hair unit. And, with the assist of the lateral slit technique and it’s principles, 2 double hair grafts will cover more bald scalp than one 4 hair graft.

Thank you for the comments guys. Blonde or lighter colored hair has less contrast to the scalp and therefore will require fewer singles to create a natural looking hairline. Greater contrast can require 400 grafts or more. There is no need to split grafts as with FUE we can harvest only the selected grafts that we want. Under ideal conditions similar to this case, as few as 200 to 250 singles can create a nice hairline, the rest used in the temples if had to guess.

This Vancouver area patient in his early 30's came to see us about restoring his hairline and temple points. Dr. Hasson and team transplanted 2640 grafts via FUE. 290 grafts were singles, 1949 were doubles and 401 were 3-4 hair grafts. A .8 mm motorized punch was used. Donor hair and texture slightly above average. A burn scar was also covered. These photos were taken 11 months post-op.

Congrats on the wedding! Not only would it be back to normal (you'll look normal much sooner), you’d have some nice growth by November if you had a procedure by the end of April/May. This patient was “face down for around 1.5 to 2 hours. A total of around 5 hours from start to finish to complete the procedure. Some alternatives to the face down are being explored but nothing implemented yet.

He used Proscar off and on over the years and restarted it orally about 4 months before his procedure.

While the crown was the only area restored, this Vancouver area patient was gracious enough to let us use his photos so I have included a shot from the front for added perspective. In his 30’s he was particularly bothered by the crown. He liked to wear his hair short and this factored in the decision to have FUE as his preferred method of extraction. Average texture hair with a higher than average donor density. A total of 846 grafts were used of which 48 were singles, 359 were doubles and 439 were 3 hair grafts. A 0.9 mm motorized punch was used. These photos were taken just short of 2 years post-op.

mikeyhwk, I agree on the fade. However, when I discussed this with the patient, he said he doesn't mind the scar and telling people it is from his hair transplant.

HairlossPA, I talked to the patient and he has an adjustable trimmer. I would guess the setting he uses is equivalent to somewhere between a #1 and #2 guard.
