Hi Guys,

Hope you find well this message,

Just want to know your advice regarding Finasteride.

It's been a while I'm using it, almost an year. Since August. I started with Finasteride 1MG/daily for a month and I noticed immediately side effects. But I had huge side effects regarding sexual performance, the were nonexistent...

I stopped for a month and started with 3MG/weekly and it was better. But recently, a few days ago I was with a girl and I had some difficulty being with here. Now with that in mind, I'm thinking to reduce even more, and may be start with 0.5, every other day.

My problem is that my hair transplant is in a month or two...

What should I do? Try to postpone the surgery? And try this dose before the hair transplant? I waited almost an year to do...

But I'm afraid to lose more hair, because I don't no if is stable.

I would appreciate some advices

Kind regards

Alexandre

You have to ask yourself what your goals are with hair restoration. Will you be satisfied with a mature hairline and a thin crown? Would you be okay with the results of transplantation alone, unassisted by meds? If you want it to be full and thick and luxurious, you really need to be on a DHT inhibitor, otherwise you’re just adding water to a leaking cup.

Yeah, I mean I would love to use finasteride daily. But unfortunately is not possible for me. I may be try 0,5 every other day and see how I react.

To be honest I don't mind with the crow. Only the hair line and the middle.

@Alex94gasp I have found myself in the exact same scenario. I had my second surgery two months ago, twelve months before that I had my first surgery. I also have had problems with oral finasteride with sexual side effects in the past too. My third attempt with the drug was 0.25mg a day after the recommendation of my dermatologist. This was probably about seven months ago. A month before my recent surgery I realized that my sexual side effects were back but with surgery around the corner I stayed on the drug. After surgery the surgery I have stayed on the drug and recently dropped to 0.25mg's every second day. Now that my scalp has healed I am going to try topical finasteride. One thing you could also consider is to speak with your dermatologist and look into oral minoxidil. I'm not a doctor. But I have been on this drug for about 7 months and I have found it a real game changer for myself. My hair and beard feel thicker and certainly improved the results of the first hair transplant. Wishing you all the best which ever way you go. 👍

I was even thinking of stopping finasteride and hope it stabilize somehow. I don't believe I will be a higher Norwood.

But I no is a bad decision and I want to stay at least with some finasteride in my system

I'm not sure why you don't think you will end up a high Norwood. It looks like you are thinning throughout the entire top. See my red line below.

Unfortunately you are among those unlucky guys who experience side effects of Finasteride.

Try to reduce the dosage and check.

Why are you not trying Minoxidil?

I used topical for 6/7 years, daily. But I stopped 2 years ago.

Maybe I will start oral Minoxidil!

I started again with 2 times per week. Monday and thursday. (1MG)

I dicide to postpone my hair transplant for next year. And see how I react with this medication.

To be honest I'm more than happy if I stay like that.

How many grafts should it be needed?

@Alex94gasp,

Personally, I think you should speak with your surgeon and get their opinion regarding proceeding with your hair transplant. In my opinion, there are many individuals who are still good candidates for hair transplantation, even if finasteride isn’t working well for them for one reason or another. Finasteride is pretty good at slowing down or even stopping the progression of male pattern baldness.  But some people simply can’t use it because he doesn’t work for them or it produces serious side effects that just aren’t worth it.

Now, based on your pictures and the information you’ve shared, you have pretty aggressive looking hair loss and you are only 28 years old. As a result, I suspect you will likely lose a lot more hair without nonsurgical treatments.  That said, depending on your donor area and how much you have available over the course of your lifetime, you still should be able to proceed with surgery if you go to the conservative route.  In other words, if your surgeon re-creates a conservative looking hairline with just enough density to create cosmetic improvement and then goes back and blend into your existing hair, that would be a good start.

You will likely have to go back for additional work as you lose more natural hair so it’s important that you and your surgeon leaves plenty of donor hair in reserve for future and subsequent procedures.

Now, given your circumstances, I do think it would be worth it to perhaps try some other non-surgical solutions, such as topical finasteride.  You can then I’ve that in conjunction with minoxidil.  Of course, you could also consider continuing oral finasteride, but just reduce your dose and see if that ends up working for you without side effects.

So I don’t think hair transplant surgery should be ruled out for you, but I do think you need to work with your surgeon and proceed with caution.   This includes keeping plenty of donor hair in reserve for subsequent procedures as necessary.

I hope this helps.

Well I have a thick hair in the back, I believe that's good. I'm thinking to do a experience and try 0.5MG every day. May be I don't tolerate 1MG.... Meanwhile in the sides I don't think I have a higher donor, only in the back. Let's see what the doctor says
