Dr. Lindsey 2500 grafts at 15 months McLean VA

This nice fellow came down and had a medium sized frontal case last year. He did really well and was easy to work with. At a 9 month visit, he was certainly behind schedule but caught up nicely by his recent 15 month checkup. Despite very fine hair and a significant disparity between skin color and hair color, he looks really good, is happy, and perhaps more important, his wife is happy. Shown are day of surgery, a week, 4 months, 9 months and 15 months. His scar turned out nicely as well. I put up both flash and no flash in the last few pics.

Looking at the immediate post op it looks like hairs were transplanted at a fairly even density throughout. What are your thoughts on using a higher density at the front of the hairline?

 

This was a large area for 2500 grafts but if I was this guy I would be planning a 2nd HT to increase the density at the front. It looks on the low side to be honest.

Rootz, I believe you are looking at the 4 month photos. I thought the same at first too, but if you look at the 15 month photos I think the density is spot on. With light completion and dark hair I think too much higher of a density would look irregular.

I think I'm looking at the 15 month ones, I'm just extremely critical... perhaps too critical. The ones towards the very end with the hair pulled up and back don't give the impression of low-ish density, but if you let the hairs stand upright naturally you can see it. For example in pictures 12/19 and 15/19.

 

The area is a bit large for 2500 grafts so I don't necessarily think there was a problem with the yield but it doesn't seem like the frontal hairline was transplanted any denser. The transplanted density seems pretty even throughout the entire area (looking at the immediate post op), which is odd to me.

I think the frontal region looks thin also. The result as a whole looks pretty good though for the grafts used.

So, at 9 months...I figured we may be way behind and need something or some more grafts to bolster that hairline.

 

At 15 months, with his fine hairs...I personally would not do anything. And his wife agreed. Like many people though, he does have a bit of hair greed and we did talk about adding some density.

 

My main concerns with that though are:

 

Perfection is the enemy of good, it would be possible to wind up looking un-natural in the persuit of perfection in a really good result.

 

And he's likely to lose more hair later..so I personally recommended doing nothing. He looks NATURAL and NORMAL now. IF we do another case in the future, sure, lets take some singles which may be useless in the crown...or middle third, and bolster the front.

 

But to go through some surgery to turn a really nice result into a little better one which isn't noticable from 2 feet away is asking for trouble.

 

But I appreciate your all's comments. Plus he may still thicken a little over the next 3 months...he was really slow to kick in.
