Repair case

 

This is a 39 year old patient who had two previous surgeries with a less than optimum outcome.

This was a difficult repair case because from his older treatment he developed multiple cyst’s and his recipient area was in very poor condition.

This could lead to poor graft survival but the patient knew this before proceeding.

 

In the first procedure we inserted 3303 grafts evenly in all areas (7143 hairs/2.16 hair per graft; 0.8 mm punch; 50 a 60 microns of hair; 567FU1/ 1681FU2/ 1004FU3/ 45FU4).

After one year we carried out another procedure with 3051 grafts (7331 hairs; 2.4 hair per graft; 0.9 mm punch; 312FU1/ 1371FU2/ 1203FU3/ 157FU4/ 8FU5).

A year later we did a 3rd procedure, with the extraction of 2404 grafts (5700 hairs; 2.37 hair per graft; 0.9 mm punch; 298FU1/ 1089FU2/ 855FU3/ 151FU4/ 11FU5).

That is probably the least yield I have seen from a Lorenzo patient but it states in the text the reasons. The Dr's standard is very high.

Based on vast research and meeting patients, I travelled to see Dr Feller in New York to get repaired.

Do the cysts cause poor yield throughout the entire scalp or just on the cysts or was this from the scarring from the previous procedures? I should still say that the final result is a nice transformation and he looks much better than before. Had he had no issues whatsoever, this would be a difficult case, as he was very advanced.

It looks like about 3000-3500 grafts from here.

The cysts make graft survival difficult? And that explains the poor harvest?

 

If the 'suspect' grafts are isolated and deemed worthy of extraction, and only then, under microscope, deemed worthy of planting, then it should read X000 grafts extracted which resulted in X000 healthy grafts and X000 deemed compromised and binned and a further X000 planted regardless in the hope they might add some volume. Otherwise it looks like a shroud, meh..the cysts, so you know...

 

Anyway, it is a poor result for 9000 grafts, but he definitely looks better front on. And another one for all the years we've had to listen to "FUE is for small jobs!"
