RECOMMENDED TREATMENT
This 53-year-old gentleman presented a very advanced hair loss, classifiable as a grade VII on the Norwood scale.
Since he wanted to be able to wear his donor hair short after the surgery, we agreed upon performing an FUE restoration consisting of two separated sessions.
The first surgery took place in October 2019 and aimed at rebuilding the patient’s hairline, anterior and midscalp areas. During this first session we transferred 3150 grafts (2811 coming from the traditional scalp donor, 339 extracted from the beard donor - the area under the chin). One week after the surgery his donor area was already quite immaculate, as you can appreciate in the photos published below.
One year later, in October 2020, the patient underwent his second procedure with the objective of covering the crown area. During this second sitting we extracted and implanted 3344 grafts (2843 scalp grafts, 501 beard grafts).
The results are truly remarkable if we consider his starting point and the patient is understandably ecstatic about his improvement, so much so that he has just booked his third procedure (to further improve the general density) for November 2021.
I am glad to share below a comprehensive photo and video documentation, which will be updated following the patient’s next procedure.

RECOMMENDED TREATMENT
I'm glad to present another nice hair restoration outcome by means of the FUE technique.
In this gentleman's case, we initially decided to adopt a three-steps approach to treat his advanced baldness, classifiable as grade VI in the Norwood scale.
The first surgery (subject of this post) took place in August 2019, when we implanted 2238 grafts FUE to restore the patient's hairline, frontal and midscalp areas. As you can observe, the surface treated would have usually required a bigger amount of grafts (around 3000); however, taking into account the patient's limited donor area, we decide to err on the side of caution by limiting the number of grafts to the minimum necessary during this first session.
The results the patient achieved are excellent, as you can appreciate in the photos published below - especially considering the amount of grafts used and the surface covered. This case shows exceptionally well the fundamental importance of a well-planned and strategic grafts placement.
As usual, we will not hesitate to update this thread when the patient will undergo his second session.

RECOMMENDED TREATMENT
This 32-year-old patient wished to restore his beard, which visibly lacked in density particularly on his cheeks.
Given the good amount of resources available in his beard donor area (the area under the chin and the neck), we agreed upon performing a FUE procedure employing beard grafts exclusively - the most reliable body hair. When available, I always recommend to use beard hair to perform a beard restoration, since it has proven to be the best resource to achieve natural results. We could also have considered the use of other body hair as well (i.e. coming from the chest, or the pubis); however this hair is usually weaker, hence less reliable in terms of transection rate and actual regrowth.
As you can appreciate in the photographic documentation below, the patient experienced almost no shedding of the transplanted grafts and a very early regrowth. The beard donor as well experienced a very fast and clean healing, with extractions signs barely (if) visible just one month after surgery. At the 6th month mark, the improvement was already remarkable. The photos that the patient sent in one year after the surgery depict his final results.

Here we present yet another nice example of an FUE restoration.
This 33-year-old gentleman was losing his hair from the age of 24. At the time of the consultation, his balding pattern was classifiable in between the grade III and IV of the Norwood scale; despite the examination revealed a high percentage of miniaturized hair in his recipient area, the patient was not willing to start any treatments to halt his hair loss.
Taking into account this fact and his family hair loss history, I recommended him to focus on the hairline and anterior area restoration at this stage. Since the patient wished to be able to wear his donor hair short, we agreed upon performing an FUE procedure using exclusively scalp hair grafts.
In June 2020, he received 2258 FUE grafts in one surgery day. The results were already remarkable at the 6th month mark, as it can be appreciated in the documentation published below. The patient has just sent in another photo, showing the final results, 1 year after the procedure.
We hope to meet him again at the clinic in the future so that we will be able to provide the community with better quality pictures showing his final results.

RECOMMENDED TREATMENT
This 29-year-old gentleman presented a quite advanced baldness, grade V on the Norwood scale.
Finasteride was prescribed 1 year before undergoing the surgery in order to stabilize his hair loss and improve his native hair quality.
Given the important surface to be treated, I recommended him to focus on the anterior and midscalp areas reconstruction during the first session. Since the patient wished to be able to wear his hair short afterwards, we agreed upon performing a FUE procedure using exclusively scalp hair grafts.
In February 2020, the patient received a total of 3523 FUE grafts during two consecutive surgery days. Below you can appreciate his evolution until the 14th post-operative month mark under different lighting conditions. He is understandably ecstatic about the results achieved so far.
He is currently considering undergoing a second session later on this year in order to complete his hair restoration journey by covering the vertex area. As usual we will be happy to share further photographic material when we will meet him again.

The first surgery final count was as follows : 2415 grafts FUT, 1521 grafts FUE.
Indeed, the patient can now wear his donor hair shorter following the donor zone repair, as it can be appreciated on the last set of pictures he sent one year after the procedure. In a scarred tissue such as these big round scars, implanted grafts have practically a standard regrowth rate.
I implanted approximately 400 grafts in the strip scar, while the remaining grafts (+/- 1050) were used to fill in the big round scars and restock the patient's donor area.

✓ 32 years old, had undergone two failed procedures via "3D grafts" technique.

RECOMMENDED TREATMENT
This 32-year-old man wished to correct his previous procedures bad results. He had two "3D grafts" procedures done in another clinic in the past, that resulted in a pluggy and unnatural hairline and, also, partially depleted and heavily damaged his donor zone due to the large punches used to extract the grafts - besides focusing the extractions only on the central portion of the donor. The patient had consulted with other reputable clinics as well, who nevertheless weren't willing to operate since his case was considered too complicated and challenging to be handled.
His situation actually wasn't simple to be corrected. Firstly, I recommended him to remove most of the big and wrongly placed grafts in his hairline.
In April 2019, I extracted 95 of these grafts in order to reshape the hairline. Less than one month later we proceeded with the main surgery. Given his donor conditions, I felt that a Combo FUT + FUE procedure was the best way to proceed in this case : by means of FUT technique, we could have reduced the damaged surface and at the same time harvested some precious grafts; additional resources would have been available via the FUE technique. The surgery final count was as follows : 3936 grafts - 2415 FUT and 1521 FUE grafts - that were used to restore the whole of the patient's scalp thinning areas.
Nine months later, in February 2020, the patient undergone a third procedure with the aim of repairing the FUT scar and restocking the donor area by inserting grafts in the majority of the big, round scars caused by the previous procedures. FUE technique was exclusively employed this time, for a total of 1479 grafts.
We have recently received some photos showing his final results (1 year and 9 months after the Combo procedure, 1 year after the FUE donor repair). I am extremely pleased to appreciate such an improvement both on the recipient and donor areas and I am gladly sharing this patient's journey with the whole community.

RECOMMENDED TREATMENT
Here we present a nice example of an FUE restoration, done over two separate sessions, on a 45-year-old gentleman who suffered from hair loss since many years.
During the first session (in May 2019), we extracted and implanted 3036 FUE grafts with the objective of restoring his hairline and improving the front-mid sections density, which presented some obvious bald spots. After the procedure, the patient experienced almost no shedding of the transplanted grafts : as can be appreciated in the photos, the regrowth was already remarkable 3 months and a half following the surgery.
The patient was so pleased about the results achieved that wished to perform a second procedure to cover the crown area and slightly lower the hairline. During this second sitting (which was performed in November 2019), 2877 grafts were harvested by means of FUE, including 300 BHT grafts (taken from the zone under the chin).
Regarding the timing between the two procedures : normally, I always advise my patients to wait at least until the 9th post-operative month before undergoing a second session. However, taking into account the very fast donor healing the patient experienced, and that the second surgery would have concerned mainly the crown area (which wasn't treated during the first one), I felt that in this case we could safely proceed before the standard waiting time.
The final pictures that the patient sent one year after the second surgery show very nice and natural results, both on the anterior and crown area.

RECOMMENDED TREATMENT
This 38-year-old gentleman started losing hair at the age of 18; when he was 25 years old, his hair loss was already advanced. At the age of 27 he decided to shave his head until March 2019, when he decided to move forward and proceed with a hair transplant procedure.
Taking into account the extended nature of his hair loss, I recommended him to focus on the anterior and midscalp areas (following a fairly conservative approach) during this forts procedure; Finasteride, which the patient started taking just after undergoing the surgery, would hopefully improve the crown situation. 3545 FUE grafts were eventually used to recreate his hairline and cover the frontal and mid sections of his scalp.
One year and two months later the patient was very happy about the results achieved and decided to further improve his hairline and frontal area. Since he responded well to Finasteride, and he wasn't too much bothered by his crown hair loss, we decided again not to touch this area at that stage.
During his second FUE procedure we harvested and implanted 1985 FUE grafts in order to further lower the hairline (approx. 1,5 cm) and reinforce the frontal area density.
Six months after the second procedure, the patient has sent in some pictures and expressed his huge satisfaction about the results achieved. Moreover, during the following six/nine months, his situation should further improve, especially regarding the hair density and texture. As usual we won't hesitate to update this thread with further photographic material as soon as it will be available.

✓ 50-year-old female patient suffering from "big forehead" syndrome, with a hair loss history in her family.
✓ Taking Minoxidil 5% since the first surgery.

RECOMMENDED TREATMENT
This 50-year-old lady since always suffered from a big forehead and wanted to find a solution to correct her situation.
This kind of scenario presents quite always good indications to undergo a hair transplant procedure. The balding areas usually do not represent a very big surface. Moreover, the donor area is most of the times excellent, which enables to get a good quantity of hair through FUT technique (preferred for women) and thus to reach a very high density on the forehead zone.
In December 2018 we proceeded with an FUT procedure. Following a preliminary estimation, we were aiming to harvest 2500-3000 grafts; in the end, we extracted and implanted 2695 FUT grafts. The evolution has been very positive and, 14 months after the procedure, the situation had already remarkably improved. The linear FUT scar was barely detectable as well.
The patient wished nonetheless to further improve and perfect her results. Thus, in February 2020, we performed a second FUT procedure consisting this time of approximately 800-1000 grafts. The final grafts count states 873 FUT grafts, which were implanted to increase the hair density where needed - for an overall total of 3568 FUT grafts.
Six months after the second procedure, the lady came to my office for a check-up consultation and was ecstatic about the results achieved. Actually, her situation already showed a huge improvement, very dense and natural results. We won't hesitate to update this thread with further photographic material as soon as we will have the chance to meet her again.

Thank you for your kind comments. I'm very glad that you appreciate this gentleman's results.
We do have reached out to him asking for some updates. Unfortunately, we haven't received any feedback so far. Of course, we will not hesitate to update this thread as soon as we get some new material.
Regarding his previous surgery : the patient was told that approximately 1500 grafts were harvested. However, as you can observe in his pre-operative pictures, it seems that just a very small amount of them survived. Probably, around 100. You can spot them mostly on his hairline and frontal area.
With regard to his donor area : at the time of his last procedure, this patient's donor laxity and density was still excellent. Most probably, there will be room for at least another FUT surgery (which could probably yield no less than 2000-2500 grafts), plus up to 2500-3000 grafts (at the minimum) via FUE procedure(s). I think we can safely assume that his donor potential is currently around 4500-5500 grafts in total - which makes it above average indeed.

This 37-year-old gentleman came to my office to find a solution for his situation. He had undergone a previous FUT procedure elsewhere, that resulted in an extremely sparse, pluggy and unnatural result, as can be noticed in the pre-operative photos.
Since he wished to cover all the thinning areas - other than correct his hairline and anterior section as a priority – in this case I felt that a combo FUT + FUE was the best way to proceed in order to maximize his donor resources. In fact, thanks to the combination of the two techniques, I would have been able to correct the existing FUT scar - by including it in the new strip - and enhance the amount of available grafts via the complementary FUE.
The patient has undergone the surgery in June 2019, when he received 4510 grafts during two consecutive days. The patient recently came back for a check-up consultation and is enthusiastic about the results achieved so far (9 months after the surgery). His hairline is now very natural and the other areas show a remarkable increase in density. During the upcoming months we expect the results to further improve, especially regarding hair density and texture, as the hair matures. RECOMMENDED TREATMENT
This 30-year-old patient underwent a FUT procedure in January 2019. The goal was to harvest as much grafts as possible in order to rebuild the patient's hairline and restore the frontal and miscalp sections. The number of grafts we eventually reached - 3920 - allowed for treating a small portion of the vertex as well, even though at a lower density.
The pictures taken at the clinic during his one-year follow-up show a very good coverage and natural results. The donor scar is almost undetectable. The patient is actually very satisfied with the outcome of his procedure.

That actually was SMP. Generally speaking, SMP doesn't alter skin tissue structure, nor influence grafts survival and regrowth.

RECOMMENDED TREATMENT
This gentleman came to my office to find a solution for his receding hairline. He had previously undergone two SMP sessions elsewhere (in 2017 and 2018) but was not satisfied with his look.
We therefore decided to proceed with an FUE surgery in order to rebuild his hairline and temple corners. Considering that the patient wasn’t taking any medications to prevent further hair loss (and wasn’t willing to start to), we decided to increase the number of grafts to reinforce the forelock and the posterior, adjacent area as well. This was done in order to stay ahead of the potential balding of these areas, that may have required the patient to undergo another procedure at some point in the future.
2950 FUE grafts were eventually extracted and implanted. I have used an important number of single follicular units to restore his hairline and temple corners, in order to give a natural but at the same time dense look. 2s, 3s and 4s grafts have been progressively placed behind the hairline to give fullness and coverage and to reinforce the anterior area.The patient is very happy with his results. The pictures he sent in one and two years after the procedure actually show a very natural and dense outcome.

Yes, without a shadow of a doubt the combination of FUT and FUE is an excellent solution for increasing the number of grafts to be harvested.
This increment can be applied in the context of one single session (4500-5000 grafts usually easily available in a two-consecutive-days session); or globally, over the course of multiple procedures, when referring to the number of grafts available in a lifetime (probably, approximately 10000). Quite simply, because the combination of the two techniques partially erases the limits of each technique performed individually.
