FUT allows you to extract more grafts from the “safest” part of your safe zone, one reason why HnW may recommend FUT. But then you’re left with the scar. You’ll need to have your scalp examined in person for signs of miniaturization to determine the extent of the retrograde alopecia and make sure you have enough lifetime donor to meet your goals.

Yes, I agree. Start with strip. And then FUE when you’re stripped out down the road.

With your age and level of loss, and without Finasteride, you will continue to lose what is left on top. A HT into the front and midsection may very well accelerate the loss (permanent shock loss) in that area and you may end up back where you started.  If you go down the HT road, you will need a lot of grafts over multiple procedures. Donor management is going to be key. You need a long term plan with a conservative approach. I think you already know all of this. 

In your case, I’d recommend you consult with Dr Wong. Probably strip to start, and then FUE down the road.

Konior uses stick and place for diffuse thinners and does great FUT work. No mega sessions though. All of the surgeons mentioned are excellent. HnW is probably your best bet for FUT mega sessions. As close to one and done as you will get. You should strongly consider Finasteride for 6 months before surgery if you’re not already on it.

Looks fantastic so far. You’ve done everything to maximize your chances of success. Very little shed to this point but it’s probably going to happen any day now.

You can’t lose a graft at 20 days. Its just the hair.

If recipient sites are among and between existing hair then some of the nearby existing hair may be shocked out by the HT procedure. If the shocked hair was weak and miniaturized it may not grow back.  Finasteride use prior to surgery may strengthen such weak hairs and prevent the shock loss, if it occurs, from being permanent. 

 No. The best long term outcome of a HT is achieved in combination with finasteride and/or minoxidil. Finasteride may help prevent permanent shock loss from a HT. Minoxidil may slightly accelerate growth after a HT.  But mostly they slow the loss of your remaining native hair. Finasteride in particular. You can still have a successful result without medication but it may be difficult to maintain long-term without meds.

Strong progress from months 3 to 4.  It’s very early but things are looking good. No need to worry at this point.

I wouldn’t remove any grafts either. The multis aren’t your primary issue, it’s density.  If you add density between and behind the multis then they will blend into the hairline better.  1000 grafts along and behind the existing hairline would make a big difference for you. Plus a few hundred to fill in the sparse temple. Unfortunately you’ll need a second procedure to achieve your goals but I think you can achieve them. 

The picture was day 10 and the hairline was still well-defined by redness. There are quite a few multis at the hairline and it certainly looks as though there were never singles in front of them. He has other pictures from day 5 that show the same thing.

No problem. By the way, at this point, unless your doctor has instructed you otherwise, you can shampoo with more fingertip pressure and get the rest of the dry/crusted skin off.

At 22 days you aren’t losing any grafts.  They are fully transplanted. Shedding can start as early as the second week and will continue until most of the transplanted hair has fallen. You will probably have more shedding over the next 2 weeks. Hang in there.

Yes, unfortunately that’s what happened in this case. There were obvious doubles and triples up front.

The grafts receive nutrients from plasma in the recipient site and after about 3 days begins to revascularize, completing this process by about day 7.

Yeah, the next three months for you are arguably the hardest part of the whole process. Not much you can do but maintain your health and wait.

@Legend007 yeah, you won’t get through security with it. I guess you’ll have to check a bag. Either that or take a small amount with you in your carry-on  (use hotel shampoo bottles) and then send the rest UPS or Fedex or something. Probably easier to just check a bag though. 

You're probably fine. I doubt you would have have left it on long enough to damage the grafts.  You'd felt pain on your scalp and turned it off before that happened.  Just continue to use the cool setting and follow your post-op instructions.

Yes, and for the next few weeks you're sure to have more buyer's remorse.  But you went to a great clinic after doing a lot of research and multiple consultations and you're likely to end up with a great result.  Congratulations and best of luck!

That is very true. Its a big advantage for FUE and helps even out the cost.  FUT is great for large cases, when you need to move a lot of hair at once or when you're concerned about thinning out the donor. But in your case, and many others, FUE is the better choice.  Its great to go to a clinic that can perform FUE and FUT at the same level and can therefore recommend the best procedure for your situation.
