  1. aim4hair, I understand, this is what I am required to do... 1) More and more before/after pics and video with markings on areas of HM with 1 year follow up. 2) Scientific presentation of published or unpublished data in a peer review journal or congress. 3) Safety approval of HM process. 4) Claim approval from the regulatory body. Regarding Pt. 1, the process of pics which can be considered evidence on internet communication or scientific presentation has already been started, but I hope you are aware, it will take 6 months to 1 year for 'after results.' I will be posting monthly pics with follow up. Scientific presentation will be done definitely mid-2013. Since Aderans has already got approval from FDA for the safety of HM process, I hope you understand that is what first phase of clinical trial is all about. Documents of US FDA safety trials of phase 1 are accepted by my country's regulating authorities, subject to this HM process or stem cell is not marketed as drug wherein even if Aderans or me will have to again go through Phase 1 clinical trial for safety in India. Regarding the claim of efficacy of HM process, Aderans will be the first to get it from US FDA by 2015 or 2017 after the end of their Phase 3 clinical trial, if they are able to grow 13 hair follicles per sq.cm after HM injections on the scalp. Till this above happens I or anybody else will be highly scrutinised, doubted and bashed by sceptics and uninformed individuals. I DON'T MIND ! Its a normal process if you look at any new development on HT. It took 10 yrs for people to shift from FUT to FUE, that too FUE has replaced approximately 40% of FUT across the globe. It took FUT to replace traditional hair-plug transplant procedures again 10 yrs. When I said my TECHNIQUE ( not the documented results which will be out in 6 months) are better than Dr.Gho's donor doubling, kindly go through the points below and experts are free to debate and form their opinions... 1) Dr.Gho uses triple wave 0.5 - 0.6 mm diameter punch to longitudinally bisect a follicular unit BLINDLY directly from the patient's donor and implants the partiial follicular unit into the recipient area. In my technique which is based on many research papers with documented findings, we first remove the follicle or follicular unit from the donor area of the scalp and under magnification (not blindly) bisect the follicular unit just below the mid-follicular false stem-cells. We precisely identify the hair morphometry by staining the follicular units with Eosin, Haemotoxylim etc. One more advantage is that, I can either use 2 partial follicular units in donor or recipient or both in recipient as per the requirement of patients scalp. And, of course donor doubling of beard hair or body hair, both the partial follicles are implanted in the recipient. 2) As you can understand, both the partial follicular units will be defecient in those stem cells which are presented in either follicular unit of each bisected part. We combine our donor doubling technique with the help of our lab by stem cell activation that is, we send few complete follicular units from the donor to our stem cell lab wherein we do isolation and activation of stem cells from mid-follicular bulge, outer root sheath, dermal papilla and matrix. At the end of the doubling procedure on the same day evening, we inject onto the area where both these bisected partial follicular units were implanted, hence we are supplying these bisected partial follicular units which were missing with the activated stem cells after bisection. Hence the quality of hair and the yield should be better as I am seeing the encouraging early results. You have wrongly quoted me, I have been working on this technique since last six months on my regular FUE patients with their consent for 50 grafts but since last 2 month, I am offering full transplant with my donor-doubling without any fees and I hope after 4 months, if results are as expected, I would be charging my fees and promoting it. How can I stop offering traditional FUE if' 1) The patient insists... 2) In areas of scalp where there is no result of HM... 3) What do I offer for alopecia totallis, scarring alopecia, on FUE and FUT visible old scar marks or bad hair transplant marks due to HT done somewhere else or multiple scars due to multiple FUTS. Also, what do I offer for Lichen Planus, Broque and other Auto-immune diseases, burnt scalp etc. Of course after 3 months testing of test dose to rule out any reaction to biofibre, I find no harm if the patient desires to offer biofibre in these specific cases after fully explaining the pros and cons for the same. The Kite of Success Rises against the Wind. More the scepticism, more the scrutiny and more the bashing inspires me further to take up the challenge. Why cant we just forget 'Dr.Nigam's hair multiplication' and focus on the 'hair multiplication' technique whether it is Dr.Nigam's or Aderans or HIstogen and see their early results. I am the only one among the three talking about confidential processes and interventions happening inside my cellular hair regeneration G.M.P certified Lab. For your kind information, Dr.Gho visited Mumbai for a joint partnership with my Dermat. friend, stayed in Taj Lands, Mumbai and this friend of mine, Dr.Anjali has also one on one seen Dr.Gho's doubling at his Amsterdam Clinic. Dr.Gho does not have any regenerative lab and neither stem cell isolation or activation of Stem Cell is legal in Europe. Also European Stem Cell Regulating body does not accept U.S F.D.A clinical trial approvals, all of us will have to do it again in Europe. The preservation medium used by Dr.Gho which lot of surgeons are thinking as 'magical juice' is nothing related to Stem Cell isolation or activation, hence Dr.Gho's technique cannot be called Stem Cell multiplication but 'Donor Doubling'. Do not confuse my 'hair multiplication' with my donor doubling with Stem Cell Activation. I partially agree with Blake that Donor Doubling with Stem Cell Activation, the documented findings for the same I'll provide in next six months could be an interesting break-through with HM.   5. Dear Deepak, I am not sure about the forum policy. Whatever little i can comment is the distance between 2 grafts is too much. Ask your doctor what is the size of needle or instrument he used for making recipient incisions and what is the size of fue punches he used for extraction. To prevent any mark on the donor area we put collagen, granules and extra cellular matrix plus arterial prp. More the density, more the natural looking appearance. May be because you have gone for less number of grafts. Regards, Dr. Nigam.
  6. Dear I believe if Bill and Blake invite tissue engineers, biotechnologist and other doctors who are working on stem cell hair multiplication or cellular regeneration. There could be an international break through into cellular hair regeneration. I am providing through my clinic in Mumbai is stem cell hair multiplication from my regenerative lab. Since Aderans did its phase one safety trial in UK under its parent company Intercytix. This safety approval in UK was carried forward to phase two trials which is presently being conducted in the USA. Similarly Aderans and histogen phase one clinical trial for safety efficacy from US FDA. And these safety efficacy documents are accepted by Indian regulatory authorities. So we are absolutely ethical and legal with a special consent form with our in-house regenerative lab to offer hair multiplication to our patients. Only thing is we have to be careful about percentage success claim. Propecia and minoxidil were able to increase to increase hair density by 13 hairs per sq cm. This was the criteria by US FDA to clear their claim of their role in hair re-growth. Aderans, histogen and Dr. Nigam are waiting may be 2 years from now to get the claim approval. All 3 of us may differ little bit in their laboratory processing approach. Aderans and Dr. Nigam use autologous stem cell and serum free growth factors. Both of us do not plan to come up with a product. Hence our stem cell is not labelled as a drug. While histogen is coming up with stem cell hair based product called HSC. Histogen is most probably into allogenic stem cell. Since we are going to offer process under monitoring of doctors and GMP certified lab, the regulation for us will be much lighter and cure friendly especially after the clearance of safety. That is how we could become the first stem cell hair multiplication clinic in the world in 2012 in Mumbai. Of course it’s still a long way to increase the success rate further. The moment histogen gets WNT protein safety approval; my process is complete only legally i am waiting to offer the same through my clinic. Next year July- August I'll be publishing my papers. Regarding,tumour or other side effects using autologous stemcells from your hairfollicle,aderans and replicell have already cleared safety first phase clinical trials under FDA monitoring,long back.It's only than we have started offering at our clinic in mumbai.Theoritically blake is right .Only time will let us know for sure..!

Dear Brody, Let me try to explain to you different techniques of hair transplantation. 1) Stem Cell Hair Multiplication- After taking out approximately 300 grafts from back or side of your scalp, these grafts are sent to GMP certified regenerative lab wherein Stem Cells in the hair follicle in the bulge, dermal papillae, matrix, outer root sheath are first isolated from rest of the hair follicle multiplied for approximately a month, also nourished and activated by specific growth factors and injected back to your bald and thinning hair scalp in the form of a solution with a very fine needle. This is what my clinic is offering first time in the world. Aderans will most probably offer the same in 3-4 yrs from now with some modifications in the process of multiplication or will use a biopsy to extract hair instead of FUE. Aderans call their process JI GAMI Hair Multiplication process. We both use autologous (that is your own hair stem cells). We both use serum free growth factors which are antigen free, hence less chances of untoward effect. Aderans are in their second phase of clinical trials in U.S. Unlike both of us, Histogen is trying to develop a product in a while which is allogenic (that means not from your own stem cells). Histogens clinical trials are happening in U.S and pan-asia clinical trials in singapore. Dr.Nigam, Aderans and Histogen also differ a little bit in the types of stem cell they use from hair follicle and the material they use to activate and multiply stem cells. Since histogen wants to develop a product to be sold worldwide, hence their product comes under drug category, hence they have to go through long clinical trials and approval in different continents. You must be wondering, how come Dr.Nigam is offering hair multiplication in 2012 and Aderans is still few years away! This is because of cure-friendly softer regulations for Stem Cell processes offered in small scale which permit smaller clinical trials with special consent form for research and therapy under the guidelines and guidance of institutional ethics committee of stem cell research and therapy provided the process is not marketed as product wherein it has to undergo a similar drug trial. We are starting our next phase of clinical trials in Feb.2013. I have not disclosed my findings or any other thing what I just disclosed right now in the forum and fifteen days back in another forum. I am waiting to improve my results further in coming months and in 2013, I ll be presenting my scientific findings on stem cell hair multiplication. But unfortunately,due to difference in regulations in certain countries, specially the western world, I will not be able to offer SHM in these countries except in the form of 'hair tourism' with free return hair ticket to India and certain countries in Asia. 2) Activation of hair stem cells and activation of dormant hair follicles - I remove 100-200 hair grafts by FUE from the back of your scalp, isolate the stem cells, nourish and activate them and on the same day within few hours inject them back to your scalp without multiplication. A cell, a U.S based company in Atlanta uses porcine urinary bladder, extra cellular matrix and have come out with an injectable product called A Cell which is injected to your scalp and it is believed to activate your hair stem cells. A Cell has not got full U.S FDA approval but certain FDA allowances. A Cell company provides these injections only to few selected doctors in U.S that is Dr. Hitzig, Dr.Cooley and others because they want their product to be under safe and renowned hands or before it becomes popular due to improper use if poor results come or side effects occur, A Cell may lose its reputation. There is a guy in Korea who is using electrostatic machine to stimulate hair stem cells who recently visited my clinic. 3) Donor doubling technique - This was started by Dr.Gho few years back based on certain scientific research papers of other scientist whose references can be found on his scientific published papers for the same. Dear Blake, donor doubling technique has nothing to do with stem cell hair mutiplication or stem cell hair activation because unless stem cells are isolated with biological markers, magnetic beading system and special reagents in a GMP certified lab which Dr.Gho does not have in Amsterdam, stem cell just cannot be activated. What Dr.Gho is doing is cutting the follicular unit vertically due his triple wave punch and needle and cutting the follicle unit blindly wherein the major part of the follicle unit is still in the dermis of the patients scalp. So one follicular unit becomes 2 follicular unit and the other follicular unit is transplanted into the recipients site. The reason why the cut hair follicular unit grows in the recipients scalp is because it contains the stem cells and surrounding tissues in sufficient quantity and quality to regrow the broken hair follicular unit. In my donor doubling technique, I use combination of two techniques, A) I just remove the three-fourth of hair follicle from the donor's scalp and one-fourth of the hair follicle is still in the donor's scalp. B) I remove the donor hair follicle unit with surrounding tissue by FUE or mini FUT and separate distal one-fourth hair root with dermal papilla and surrounding tissue and remove proximal three-fourth hair follicle with bulge plus outer root sheath plus surrounding tissue and implant the two separately on the recipient area. Both myself and Dr.Gho use special preservation media, so that we can nourish the grafts and try to activate by whatever source we have on to these bisected follicular units before we implant them on different recipient sites.you can refer two reseach papers on my site on donor doubling on which is based my doubling.do not confuse donor doubling with stem cell hair multiplication. 3) FUT and FUE with little variations from different surgeons , dermats,and different equipment manufacturers.and other techniques are discussed in detail in the hrn forums and site. regards dr nigam

  14. 1)stem cell treatment should not be expensive only if there is competition and not monopoly. ,I do not take advantage of monopoly at present,since for me pioneering and improving results with large number of patients is a better option.btw in India volumes are better option to grow2)yes ,but results are better, if. injected in any part of scalp where some dormant telogen follicles still exist.3)beard hair are good for thickness and for those who have poor donor .4)have not yet injected a patien with beared stem cells to the face,but its a good thought, will definitely do a study on that5) at present most of the new hair growth is secondary to dormant telogen activation,so hair angle is not altered in that case.I have not seen any unnatural hair groth after stemcell HM injection even if it is blind, when all your hair follicles are formed in the womb of your mother when u were 6 to 7 months ,hair grows in respect to a particular angle to the surface of the respective area of scalp,so that to protect u the follicle muscle pull it up to 90degree angle to the surface of scalp,what is nomally called hair rasing..!.6)it's a very good option for thinning scalp. regards dr nigam

Don't be a doubting Thomas, yes i accept mistake of my staff, I do not have so much time to look daily on my website, but since i have taken challenge on this forum, i will be spending more time on before after pics. I know what i do, but remember i may also do mistakes but trust me stemcell hair multiplication upto 50% results are already there for the world to see and i will prove this with photographs, scientific presentation and forum members visit on my clinic who will post as per their experience. Any positive development in hair multiplication is good for both consumers and doctors. And the pictures which are posted today is just showing early growth of Hair Follicles and in one and one half years, he should show good growth.

Dear moopookoo, good skepticism,the guys pic ur mentioning,had shaved off his hair around vertex,where he had thinned hair,before getting injected with multiplied stemcells.That's why u see thick growth of hair in that area.Why don't u see myself on skype,and compare with my before pics of jan 2012 available on my website, I hope u will not hve photoshop skeptism possible on skype. wait for fresh photos of this and other patients ,let me see , how many r considered photoshopped, Regarding angles of side hair, please click ur own photos for next 4 days ,u will find atleast 1 pic with the same natural angle of hair ,especially on the back and side of scalp.
  20. Since epithelial stem cells are thought to have a life span at least as long as the organism, they are thought to be susceptible to multiple genetic hits. (Perez-Losada and Balmain, 2003) Till now no clinical trials has reported the same, neither have we. Even if the person does not have stem cell hair multiplication, it is the mutation of genes in resident epidermal stem cells and hair follicle stem cells are the ones which have the potential for tumorigenicity. Luckily, there is no increased incidence of the same after injecting multiplied stem cells as on today. That is the reason we do not multiply cells more than 2 cycles although great stem cell hair mutiplicationl result are possible when we multiply adult stem cells into millions. i have kept my stem cells for multiplication for next 3 months to become the study subject. Adult stem cells can differentiate only into hair follicles and can be utilized for healing of skin after a cut. Embryonic stem cells are pluripotent and can differentiate into any tissue , mezoderm ectoderm and endoderm. Embryonic stem cells are very active, thats why when an egg and a sperm meet, a whole human being is formed. Adult stem cells which we use are inactive already, myself, adreans and others are trying to activate them which in turn will activate dormant telogen follicles and will also differentiate into new hair follicles. Care has to be taken not to be over ambitious to use any technique which tries to over stimulate inactive adult stem cells in hair follicle which may lead to genetic mutation. Hence most of the GMP certified labs have anti mutation equipment through which the final multiplied hair sample is passed to discard any sample in which stem cell might have got a genetic or a mutation hit. We have a GMP certified lab with out clinic. Once few years pass without any incident reported then we can become slightly more aggressive. In developement of medical science we always have to balance between finding curative solutions with safety efficacy. Regards, Dr. Nigam.

Its being nice to see you on this forum. I m confused to go for Stem Cell or FUE , as you have said Hair Stem cell multiplication has a success ratio of 50% and i m planning to go for hair transplant in coming month or two , so at this point of time i think it is very risky to go for Stem Cell.
  22. Dear Krshonkush,yes it's true, success rate will gradually improve,but for the fortunate ones it's an amazing experience to see new tiny hair sprouting out as early as one or two months, even in NW7.

Dear maverick,this side effect has not been reported till today,niether by our team, niether adrens trialsnor histogen american and panasian trials till now,nevertheless it cant be ruled out,only with time we will come to know.Even paracetamol has side effects.All of us have mutation detecting equipment in our labs, and any such potential sample is discarded hence we can scan and prevent the sideeffect to minimise any incidence.i am using your own adult stemcells hence the possibility of the side effect is negligible as opposed to those using embryonic stemcells or allogenic stemcells.E=maverickroolz1;2326711]Hello Dr.Nigam, I am really concerned about the side effect ( basal cell carcinoma) which you have mentioned with this stem cell treatment. If that happens I would end up bald anyway due to treatments etc and more over it is life threatening.
