Visiting WCHR 2013, UK to improve on HM – Agenda @Dr. Nigam’s My agenda for 7th World Congress on Hair Research, 4th – 6th May 2013 would be seeking answer for following questions by meeting concerned researchers and discussing with them during their presentations. Let me see if I can get video interviews, discussions for you all. All my member friends can also post your questions related to stem cell hair multiplication, DP Cells Injection, DP culture, etc which I might have missed. I will be in Edinburgh from 3rd – 6tth May & 10th & 11th May 2013, I will in London from 7th – 9th May 2013. One member from Netherland will be coming to attend the conference, he is a bio-tech and has been helping me a lot may be I will sponsor him partly. Questions:- 1. Hair follicle epithelial and mesenchymal stem cells interaction and communication to create a new hair follicle in-vitro or in-vivo. Would like to discuss more with Jahoda regarding his organ germ method culture for the same. 2. Trichogenic DP culture, Cohesive DP Structure, In capsulated DP Cells, ECM secreting aggregates of DP cells. 3. How to increase the healing of bisected follicle without the Dermal Papilla? 4. How potent are freshly isolated DP cells from body hair kept in DP culture medium? 5. The in-vitro follicle bisection, what is the minimum requirement for re growth in the bisected follicle (Bulge stem cells + outer root sheath & / or dermal cup sheaths stem cells &/or hair germ matrix epithelial stem cells)? 6. The best method in culture medium for DP culture that is 3D culture hang-in-drop method, how to get a intact basal lamina? 7. Role of repeat injection of Growth Factors in-vivo for activating dormant follicles &/or creating new follicles. 8. Better ways to counter the effects of DHT on Dermal Papilla via TGF-BETA1, how to block or slow down the follicle miniaturization? 9. What difference in hair follicle neogenesis we can expect viv-a-vis multipotent adult hair follicle stem cells vs pluripotent embryonic stem cells? How effective wil be the conversion of multipotent adult hair follicle stem cells to pluripotent embryonic stem cells? How will HM improve if at all by conversion of multipotent adult hair follicle stem cells to pluripotent stem cells and subsequent differentiation? 10. What is my favorite researcher Jahoda’s experience on trans-gender hair transplant and how to achieve optimal results with eye-brow like pluck hair transplant with or without associated Dermal Sheath? 11.   2. Recipient growths have been normally good,specially with invitro doubling. This patient is from malaysia ,he will be sending recipient and donor photos every 15 days ,will keep you all updated.
  3. Hi, This is a patient who has undergone in-vivo hair doubling. He has a very poor donor area, mostly single and thin grafts. The patient is from Malaysia and sent this pictures from there as he had purchase a videomicroscope for sending us videomicroscopic pictures. The patient will be coming after 1 month for his multiplied stem cell injection and further doubling of 6000 plus grafts. Till now he has not seen any shedding and the bisected implanted grafts are growing slowly and donor regeneration is also progressing smoothly. Donor Regeneration is up to 80% to 90% by 16 days as he has not send us all the picture of the donor so cant assess it properly. As you can see from the donor regeneration pictures there are no white dots visible and hardly any marks left there. Day-7, Donor Regen & Recep 4417 G Doubing on NW6/7@Dr.Nigam's A case of NW6 (NW7, as per the donor availability and the thin hairs), we plan to make him NW2 with donor doubling, stem cell, DP cell injections. At present we have done donor doubling of 4417 G. In next 3 days we will be doing more 3000 G, 1000 G each day, which brings the total of 7417 G and the same procedure will be repeated after 6 weeks when the patient will return from Malaysia. Recipient Please find below the Day 0 & Day 7 pictures. Today we have done 1495 G by plucked hair transplant without using any hollow needle from donor area. You can see the quick healing & fading away of the marks. We expect 100% donor regeneration in 6 weeks as the follicles grows in 8 weeks maximum.   9. Blake, For single graft I use 0.5 mm, for double 0.7 mm and for Triple 0.7 mm or 0.8 mm with hollow needle. We are the 1st clinic which uses Ultra Sound guided Partial Follicular Unit extraction. So we can see where we are bisecting it’s not blind technique. It’s a beginning of giving eyes to the Hair Transplant industry.Even fut/fue should be special usg guided. This is In-Vivo doubling plucked hair transplant with stem cells and DP cells. In this case I can go only upto 1mm in the epidermis unlike traditional procedure where they reach epidermis junction, then I tweeze and pluck the graft. As this plucked graft will not have dermal papilla and root so I inject DP cell and after 6 weeks, DP culture + Growth Factor + Stem Cells to repair the bisected follicles.

Believe it or not, it is surprise to me too that we can see on 12th day 46 growing follicle at Donor and 34 growing follicles at recipient, regenerating themselves with additions of new follicles formation. Total 80 follicles re-grown from 23 follicles extracted. Remember we have also injected stem cells, dp cells, dp growth factor, ECM + PRP. Hence new follicle regeneration also as a part of our stem cells hair multiplication therapy. In the months to come we will follow the rate of growth and also the thickness of the follicles. I will be able to comment in detail about your case once either i am told whom did you speak to at my clinic ,since i don't know ur real name, this will help me take out your details or if you can mail me your pics dr.rahul1970@gmail.com or can talk to me on my skype id dr.nigams Regarding stemcells,this is the procedure offered at my clinic- 1)Day0 -Extraction of 50 to 100 grafts and few more grafts for dp cell/dp injections and stemcell isolation and culture from your donor scalp or if the donor is weak from chest . We will give you first shot of activated stemcells on day 0 evening with dpcell/dp injection plus growth factors plus ecm plus prp. 2) We will repeat the procedure twice after 6 weeks plus dp culture injection. With this, as of now (with improved technique) you have 70 to 80% chances of success of regrowing new follicles and activation of dormant follicles with minimum density of 30 hair follicles (not graft) per sqcm of injected site. We will keep track with video microscope and macro pics. 30% chances are there, you may not get any hair or few hair that is less than the density i mentioned. Now what should we do if you fall in the category of 30% unfortunate scalps. That is when we offer with the above stemcell procedure ,the donor/hair doubling. Please understand we are moving towards creation of dermal papilla implant and subsequently hair germ or microfollicle in vitro in the lab. Till that time ,it's a good idea to use dermal papilla from chest hair by isolatimg from it or partial follicle by plucking a part of hair follicle from the donor scalp, which acts like a proto hair or a partial hair structure ,which can interact with multiplied stemcells and dp culture to create follicles with near 98% success(theoretically 100%) No more FUT/FUE/BHT at our clinic only STEMCELL AND HAIR DOUBLING.   18. We did 2500 grafts Hair Doubling to 5000 grafts on day0. We extracted 2500 Grafts from the donor with 0.5 mm punch and bisected the graft at the level of dermal papilla. Boosted these grafts with progenitor stem cells+ DP cells = ecm+growth factors+prp.As you can see the patient is complete growth at the recipient.All the bisected grafts were implanted at the recipient not at the donor.This patient is adermatologist from delhi . This patient will receive 3000 grafts doubling to 6000 in next 2 days and hence will be a case of 5500 grafts double to 11000 grafts thus will be converting from NW5/6 to NW2/1. Recipient area was also injected with activated, epithelial, dermal cup sheath stem cells & isolated DP cells from chest follicles, ECM, growth factors & PRP. Pre-implantation slits were made on the recipient area to avoid prop-out of grafts & keep the whole sticky with granulation tissue. The slits were first injected with PRP & DP cells at the derma sub-cutaneous junction. Both the recipient areas has 276 extra grafts which were 2 follicles converted to 1 follicle for aesthetic hairline purpose. We are also expecting more grafts on the recipient because of stem cell, DP cells & growth factors injections. Patient had already provided with ECM at the donor and recipient for quick healing and to avoid any mark. I do not claim but Dr. Hitzig and Dr. Gho claims ECM activates hair stem cells but I do claim injury to the plucked follicles will definitely activate stem cells. Less shedding will be seen because the plucked follicles will first try to recuperate and manufacture the other half as against going in a resting phase after the hair transplant.
  20. Blake, I agree with you that next logical step is hair/donor doubling with stemcells and growth factor and dp culture. The problem at present with hair multiplication is we are able to isolate and multiply hair epithelial stem cells and culture dp dermal cells from which a follicle is formed in a mothers womb. But the cross talk and induction of these cells is not in our control in vivo inside the scalp. As we reach to the next step of dermal papilla creation in lab and implantation in the scalp,results will improve by leaps and bound. At present in the fortunate scalps, this cross talk and induction happens and some get great results and some do not get any result. In case of hair/donor doubling we are fortunate to have a proto bisected distal follicle and bisected proximal dermal papilla intact for autologous stemcells to interact with and hence much higher and predictable success rate.

Blake,Before me ,the well known research scientist Jahoda and others have shown in several studies that 70% growth of the bisected follicular units after bisection and reimplantation.As we add to the bisected follicular units progenitor stemcells on day 0 and multiplied stemcells at 6 weeks +dp cells +ecm+prp+growth factors, We started hair doubling 6 months back. We have seen, in last 6 months regrowth of bisected follicular units to be around 90 to95%. Regarding the shaft diameter ,we will measure the same after 6months, as it takes 1 to 11/2 year for even FUT/FUE transplanted hair to achieve full thickness In our 6 months observation the shaft diameter is comparable to traditional FUT/FUE hair around this time.We have the advantage of stemcells and other factors as mentioned above.I will post the pic of 28 days followup of a patient on monday, for whom we have doubled 1150 grafts to 2300 grafts. In another forum i have opened my clinic to FREE patch test for forum members with 30 to 50grafts, for the month of march and april 2013 to those patients who are ready for semi permanent tattoo and are ready to document the same with hair counts,as an independent observer.

Stifffler, When a graft is doubled into two,this is when we call it 100% success as far as the question of doubling is considered (Jahoda and others have conducted hair doubling studies without addition of stem cellls and growth factors with 70%regrowth in both the bisected follicles.Since we add progenitor stemcells and growth factors our success rate is 90 to 95% as on today). When the diameter or the thickness of these bisected two follicles is not less than the original thickness of the follicle,than we can claim to achieve 100% success rate, as far as the quality or character of the follicle is concerned. Rate of growth of these bisected follicles is also a parameter to watch for. These bisected follicles tend to skip the shedding phase after implantation probably due to injury to the follicle and addition of stemcells, hence enters repair phase rather than shedding phase, seen commonly after traditional FUT and fue hair transplant. As you can see in the above case study, the doubling as per as the number is concerned is not an issue, The quality/diametre of hair in our last 6months experience( as we started with hair doubling since last 6 months) is not any different from the thickness achieved by follicles after traditional FUT/FUE transplant, the exact diameter we be measured after 6 months ,when our cases reach 1year growth, as in any transplant the final thickness of the follicle is achieved at 1 to 11/2 year of transplantation. Any case which is an indication for FUT/FUE is fit for hair doubling, specially the MPB patients with poor donor availability at the safe donor zone. We are in the process of converting NW6/7 to NW2 with hair doubling which is not possible with FUT/FUE except with body to hair transplant. 10000 grafts required for the same can be created by 5000 donor grafts doubling(in cases with availability of good donor or the body/ beard grafts need to be used).   23. Blake, Lauster's and Dr Gerd created the world's 1st microhair follicle in Vitro in the lab. I have visited their lab in Berlin in the month of Sep 2012. Their team along with Dr Gerd visited my clinic at Mumbai, India this Jan 2013. We had long discussions on technical issues regarding stem cell hair multiplication, dermal papilla culture and implant, proto hair implant. Till the time we perfect HM, hair doubling with stem cells and DP cells is a good option for people with poor donor or insufficient donor to cover their MPB.
