Change In Appearance Over Two Hair Transplant Sessions
In a person with fine hair and only one session, combing the hair straight back will not take advantage of layering and will appear thin, although natural. Compare this to the photo after two sessions with the hair combed to the side. In photo after 2nd session, note that pinkness at hairline is from a recent sunburn.

What does a donor density of 2.6 mean?The donor density measures the number of hairs per square millimeter in the donor area (hairs/mm2).
Is there a scale of density? Microns? What is considered average?It is measured in hairs per square millimeter. The typical range for a normal Caucasian is about 1.7 - 2.5 hairs per mm2. Donor density of 2.2 is considered average.
Oh and how is it calculated? With a special device/machine?A densitometer is used to measure donor density. This device magnifies a 10mm2 area of the donor area. The physician counts the hair in that area and divides by 10.

Bernstein Medical - Center for Hair Restoration Patient ETC is a Norwood Class 5A with medium brown, fine, straight hair and a donor density of 2.3. Results after one session of 1,750 follicular unit grafts. Note that ETC had a mini-micrografting procedure on the top of his scalp that was far enough from the hairline to be completely camouflaged by the subsequent hair transplantation procedure.

Bernstein Medical - Center for Hair Restoration Patient RPI. Plugs placed within patient RPI’s old hairline were not noticeable – at least at first. With further hair loss, the poorly planned hair transplant became very unnatural. Fortunately for the patient, the old plugs were located high enough so that camouflage was achieved with just one session of Follicular Unit Hair Transplantation.

Bernstein Medical - Center for Hair Restoration Patient BIB had three hair transplants in the early 1990s using the old, large punch-graft technique. This resulted is an unnatural, pluggy look that was very bothersome to the patient. In addition, the improper placement of hair and poor growth resulted in inadequate coverage and a significantly depleted donor supply.
The repair strategy was to remove the large grafts, divide them up into individual follicular units of 1-3 hairs each and re-implant them at the frontal hairline (to make it look more natural) and to the top of the scalp (to give it more coverage). The after photo was taken following a series of graft excisions and two follicular unit hair transplant procedures. In total, 339 plugs were removed and 4,714 follicular unit grafts were implanted. These grafts were obtained from both the excised plugs and two donor strips.

Bernstein Medical - Center for Hair Restoration Patient AKL is a Norwood Class 5A with straight, brown hair and a donor density of 1.9 hairs/mm2. He had one hair transplant to the frontal area consisting of 1,963 follicular unit grafts. Pictures were taken before and then one year after the hair restoration. Patient AKL has written a letter to Dr. Bernstein which we have posted on our Letters from Patients page. Patient AKL had a 2nd session with an additional 1,286 grafts for a total of 3,249 follicular unit grafts in two sessions. The only photo after the second session is in the Top View section below.

Great question,
On a first hair transplant procedure, I generally place the sites/grafts symmetrically, even if a patient combs his hair to one side. The reason is that the person may change his styling after the procedure and I like to have the first hair transplant symmetrical for maximum flexibility. An exception would be a person with limited donor reserves. In this case, weighting on the part side is appropriate in the first procedure. Once the first hair transplant grows in and the person decides how he wants to wear his hair long-term a second transplant can be weighted to accommodate this. Weighting can be done into ways; either by placing the sites closer together on the part side or by placing slightly larger follicular units on the part side - or both.
If a person decides to comb his hair back, then forward weighting is used. For greater details on this, please see some "old" publications:
